Itachi could have been the first and greatest Uchiha Hokage ever. The old bastards should have had more trust in his love for the village and then perhaps the Uchiha clan might have calmed down seeing how their future clan leader would also have become the Hokage! Unfortunately he suffered from some kind of terminal illness. He wouldn't have lasted much longer in position than Minato. So for whatever's worth, it would have been a brief hurrah for the Uchiha and then a quick decline back into resentment. Besides, there's no way he would have become hokage. Not only do I doubt he even wanted it, the rest of the village and the fire nation wouldn't have been much for it either. Before he slaughtered his village he wasn't much more than a very promising ninja. He had no impressive accomplishments, no reputation that would peak the attention of the fire nation lords, and he was way too young. Minato was 21-22 when he got elected; Itachi was 13 (!) when he killed his clan - I doubt the fire nation would pick a 13 year old boy as hokage. Might even have been against the law. Also for reference, both Naruto and Kakashi became hokage at 30'ish and Hiruzen at 22-23.

Itachi kept his illness a secret too, so unless the Hyuga could somehow see his illness with their Byakugan, I doubt they kept it a secret from him specifically. Maybe if Itachi's illness was an Uchiha bloodline thing, which the Hyuga randomly stumbled across a cure for, and as such kept it a secret to maintain some degree of power over them, I could see it. Otherwise I'm not sure I buy it.

oh man, what a great character he was. not only did he save his village by massacring his own kind including his father and mother, and assuring his little brother's safety and also potentially making him stronger and branded as a hero; when he was revived, he saved his village and the ninja world, potentially saved his little brother again by not killing kabuto. he really was the greatest shinobi ever lived.
I just realized that itachi can have a EMS in one eye with shisui's eyes. But instead he chose to use the power of the eye (kotoamatsukami) to sasuke if ever sasuke tried to destroy the konoha and go to a dark path. Also one of the reason why he let sasuke survived and sasuke to want more hatred (except for being a brother) is for his little brother to become stronger and be killed by his own bloodline. Truly tragic fate!
