I know that animals (probably) wouldn't recognize/understand a smiling or sad cartoon face, but the idea/intention is pretty fresh and smart.
Also, from what the other commenters said, it can also be a rather educational (?) toys for kids/toddlers, that if they (somehow) break it, it'll reveal the "sad face" to show that their toys wasn't "happy" to be treated like that.

It might be possible. I have seen plastic horse figurines with the junk. They tend to be the really realistic ones. Maybe a hobby store is a good place to look.
