I'm a fan of running a knife horizontally along the belt, (sort of cross draw) as long as it's reasonably sized it can really keep things streamlined and out of the way of the holster etc.
Does that inner belt fit through belt loops on jeans, hiking pants etc.. it looks wide in video.
The back of the belt does not have Velcro. Do you feel this affects versatility?
Not if what you want to attach there is MOLLE compatible

Inner belt should go through your belt loops it’s there to prevent the outer belt from moving it’s literally doing nothing over your tshirt
I do the same most the time as I wear shorts , gym most the time , if you do what he did with both bets it will work
