    Sir I have a doubt. As you said acidic drugs are better absorbed in acidic medium but in reality it is absorbed more in intestinal mucosa but as we know medium there is alkaline so ionisation will take place and drug will be excreted. So how does it get absorbed in small intestine?

      Thanks for writing back to us. It is not that 100% of a weak acid is present as ionized form in the intestine. The ratio of the ionized and nonionized forms at a particular site is dictated by the pka of the drug and pH of the site. For weak acids, the ionized form is present in greater amounts than the nonionized form in intestine. Even though less in amount (as compared to the ionized form) the nonionized form is (easily and well) absorbed from the intestine. On the other hand, nonionized form of weak acid is the predominant form in stomach but they are not well absorbed because of the thick mucus covering and less surface area of stomach. So in reality the influence of pH is overcome by the increased surface area and good blood supply of he intestine. Hence all drugs are better absorbed in intestine and not in stomach. Hope I have clarified your doubt.

      With regard to PH, weakly acidic drugs are better absorbed in the stomach and weakly basic drugs are better absorbed in the small intestine because they remain largely unionized (more lipid soluble) in the similar environment.
      But the other important factor(s) for absorption is surface area and blood supply. When an acidic drug reaches small intestine, it starts to become ionized gradually, not at once. So whatever that is still remaining unionized will rapidly get absorbed because of the large surface area due to micro villi of small intestine. That is why an acidic drug gets absorbed in the acidic environment of stomach (same medium) as well as small intestine, because of large surface area and good blood supply.

      Thanks for your comment and the request for suggestion regarding pharmacology textbooks. The best book for general pharmacology is Clinical Pharmacology by Bennett and Brown. The first two sections (first 200 pages) are 'must to read' for all medical undergraduates. Everyone will surely enjoy reading this book. For individual systems, Basic and Clinical Pharmacology by B.G Katzung is good. If you are scared about the size of this book, you can follow Brenner and Stevens' Pharmacology. The fifth edition of this book is the latest one (2018).
