  • Today, Alexander presents how Endotrophin can be used as a novel biomarker in Heart Failure with preserved Ejection Fraction (HFpEF), using the PRO-C6 biomarker assay.
    HFpEF is predicted to become the predominant form of HF worldwide. It is a very heterogeneous clinical syndrome with a very high degree of morbidity. The syndrome is determined by age, comorbidities and fibrosis, and there is an unmet medical need for better diagnostic and prognostic biomarkers.
    Endotrophin is a deleterious molecule released during the formation of collagen type VI and has been implicated in fibrosis and inflammation. Nordic Bioscience has developed a prognostic biomarker for disease activity, PRO-C6, which measures the concentration of endotrophin and the formation of VI type collagen. PRO-C6 is elevated in severe HFpEF, and HFpEF patients who die have significantly elevated levels of PRO-C6 at the onset of their disease. This suggests that PRO-C6 is a relevant biomarker for risk assessment in HFpEF patients.
    PRO-C6 was analyzed in a subset of the TOPCAT study, and it was found that adjusted PRO-C6 levels were significantly associated with the primary endpoint - stronger than NT -proBNP and the MAGGIC risk score. This demonstrates that PRO -C6 adds value to currently used risk assessment tools in HFpEF.
    Furthermore, patients with a high-risk endotype can be identified by their PRO-C6 scores - patients with high PRO-C6 scores have a 60% reduced likelihood of survival at 3-5 years, compared with a 10-30% reduction in patients with low PRO -C6 scores. These data indicate that PRO-C6 can be used as a strong prognostic biomarker in HFpEF studies.
    The data show that:
    1. PRO-C6 is associated with HFpEF disease severity and outcome;
    2. PRO-C6 adds unique value to biomarkers in the field of HFpEF;
    3. PRO-C6 can be used as a biomarker for risk stratification.
