    Why would the finasteride group be ashamed of saying they had side effects and the placebo group wouldn't? The difference in % between sides in the placebo vs finasteride is what matters, since shame it not related to drug use

      Doesn't even matter, these statistics provided by Merck are complete nonsense. Their claims that this drug is safe are ridiculous. Look into the released documents by Reuters to see how far the deception goes.

      You’re wrong. What this means is that both groups might have underreported sexual side effects, meaning that the difference between placebo and finasteride group would also incrementally increase.

    This is the most easy to understand explanation about MPB in youtube. I watch a lot of MPB videos here and they just throw around medical terms like 5 alpha reductase and dht and the role of finasteride but its never clear to me how it actually works until this video! Thanks mate!

      From what I’ve read, it takes 14-21 days for your DHT levels to normalize (remember that Finasteride only inhibits up to 70% of your DHT conversion, it doesn’t block everything).
      And it takes about 90 days for Finasteride to be completely out of your system.

    Advising men with MPB to first use minoxidil/dermarolling as a first line defense is not very good advice. The main cause of MPB is DHT and as a result men with this common problem should find ways to block the DHT whether that be from ingesting finasteride/dutasteride or applying some kind of topical DHT blockers such as topical Finasteride. Blocking the DHT should be the first line of defense. Using minoxidil is only a bandaid and the hair loss will continue.

      It depends on the patient's risk tolerance. Many healthy men aged 20-40 would not be willing to risk prolonged sexual dysfunction (even if the risk is very small) for a cosmetic condition. We're talking about hairloss here, not heart failure - safety is paramount. My recommendation prioritizes safety.

      In addition, data from double blind randomized placebo controlled trials indicate that the majority of men will be able to maintain their hair on minoxidil monotherapy. If the patient is dissatisfied with minoxidil, they can always add on finasteride down the line without losing ground.

      A few years ago I would have agreed with you. If you read merck's product monograph for propecia, you would be under the impression that finasteride 1 mg causes sexual side effects at a rate of 1 or 2 percent greater than placebo and that these side effects are fully reversible after discontinuation. However, new research suggests that the frequency and severity of sexual side effects may be underestimated. We saw this happen with ssri antidepressants a while back - the trials were not calibrated to detect sexual dysfunction and the true rate ended up being severely underestimated. However, depression/anxiety are potentially debilitating conditions, risk factors for a multitude of diseases and therefore this risk may be warranted. Male pattern hairloss is a cosmetic condition and topical minoxidil is a safe and efficacious alternative to finasteride. Some men will respond very well to minoxidil. Exposing men to the increased risk of finasteride without first trialing minoxidil is not appropriate in my opinion. That being said, some men (myself included) are willing to tolerate more risk. As long as these men know the risks and benefits of dht suppressive therapy, I would not be opposed to starting with finasteride or dutasteride.

    Hey Doc, can you shed some light on this please.
    Finasteride is out of your system in 8 hours but the 5AR is inhibited for 30 days? I'm confused, if DHT returns after 7 days, how does it do this if 5AR is still inhibited for a whole month. Could you please elaborate on this? This is what the doctors on the hairloss show channel said.

      I have taken finasteride for 4.5 years (started at 20) without any noticeable side effects. Was able to regain a substantial amount of hair with finasteride 1.25 mg daily and minoxidil 5% applied twice daily - im still above baseline I believe. Had diffuse thinning that I noticed at 19 (dad and grandad on both sides went bald very early). Hope this helps!

      @@sankalpsharma8 When I first started noticing hairloss at 19, I went to a walk-in clinic and was recommended minoxidil 5% twice daily by the doctor - I wanted finasteride but the doctor did not want to prescribe it due to the possibility of side effects. I did not use minoxidil at this point as I was skeptical of its efficacy. Fast forward to 20 and I was losing hair fast noticing 50+ hairs each time I took a shower. I had a new family doc at the time who agreed the prescribe me finasteride 1.25 mg daily (I split the less expensive 5 mg tablet into quarters) as I explained to her that I was familiar with the risks and benefits of this medication. My hairloss stopped 3 months after starting finasteride but my objective was to get regrowth as well - particularly at the crown where I had been experiencing most of my hairloss. I began minoxidil 5% twice daily about 6 months after starting finasteride after reading this study - onlinelibrary.wiley.com/doi/abs/10.1111/dth.12246 - demonstrating superior efficacy of combination finasteride + minoxidil therapy to either treatment by themselves. This study was conducted in 450 Chinese men - I am half Chinese so I believed these results would translate well to my situation. I noticed really good regrowth from finasteride + minoxidil about 1 year into therapy (12 months and 6 months from when I started finasteride and minoxidil respectively). I have been able to maintain these results for the last 3.5 years. Unfortunately I did not take any pictures documenting this transformation so you will have to take my word for it!

      I also have CFs. I started fin and am not sure whether i’m experiencing sides. I sleep shorter and feel depressed sometimes. But that could be totally unrelated to fin.

    I've heard that if finasteride doesn't produce results, then the name brand "Propecia" often is recommended. Have you found that this does make a difference? Also, is there any difference between Propecia and Proscar, which I understand are just two name brands for finasteride? As for ketoconazole shampoo, is this only effective for those who have a scalp condition?

      I’m not an expert in anything, but observed differences in name brand vs generic for any drug has generally seems like a placebo effect…unless MAYBE the requirements for generics are loose in a given country (i.e. looser standards mean generic might be less potent). I’d guess in many cases, claims that name brand is better than generic have more to do with timing of the switch than the switch itself. Someone starts generic for 6 months, feels it isn’t working, switches to name brand, then notices a huge uptick in response in the second 6 months, but it has nothing to do with the branded medication and everything to do with the duration of continued use finally producing positive results. Whether Propecia, Proscar, or generic, they are all finasteride. Again I’m not an expert, but this is my intuitive/logical observation shaped by what I’ve heard experts say (and if you refer to online anecdotes, many people use any of these 3 with positive effect). Some people are just non-responders, or haven’t waited long enough, and again these situations affect their anecdotal observations of what worked and what didn’t.
      Ketoconazole, in my observation, is primarily advertised per its main use case: scalp conditions like psoriasis and dandruff, but people obviously suggest it for hair loss due to its anti-androgen properties. Notice a bottle of Nizoral doesn’t mention hair loss on the bottle. That should tell you something. When I saw a hair transplant surgeon with tons of experience (who has YT videos btw) he seemed to downplay its importance. It seems that bang for buck in efficacy and research are finasteride and minoxidil, hands down. If keto has an affect, it seems mild. I view it like rosemary and pumpkin seed oil…”some” efficacy, and sure, you can include it in an “everything but the kitchen sink” approach, but the big 2 are the big 2 for a reason.

      Hey! I think about 1 year to recover a significant amount of crown hair. Never taken long breaks from minoxidil but I did reduce to once a day and dont seem to have lost too much ground. Hope this helps!

    Dude u have great hair my father went bald around 35 so I'm 20 with Norwood 1 and very slight hairline is thinning will I be able to recover and remain it ?? I think u have great hair !!!

      Thank you my friend! I'm more of a diffuse thinner so it's a lot easier to hide, especially after gains from minoxidil + finasteride. It's entirely possible that you will continue to lose hair. Not medical advice, but If I was in your position, I would start minoxidil 5% twice daily right away and start thinking about the risks and benefits of oral finasteride. If you are serious about maintaining your hair long term, finasteride is the best option. But if you need some time to think about whether or not to start finasteride, I would use minoxidil 5% twice daily +/- dermarolling +/- ketoconazole shampoo in the meantime. You are young and could probably regrow some density around the hairline and maintain your norwood 1 well into your 30s and 40s if you started oral finasteride + minoxidil right away. But again, talk to your healthcare provider about the risks and benefits of each treatment before starting.

      I'm on 1 month daily finasteride with no sides and taking oral minoxidil low dose 1.25 everyday and having no sides at all instead I feel more energetic my erection quality is on another level I usually last long on bed but I'm better than before I don't know why 😂. I'm Indian( Asian ) so I think I might regrow my hairline back which has thinned out a slight.

    I m balding from 17 and i m 19 . My left temple is more receded than right temple and i used to have a little spot on my crown area . I have family history of hair loss , i m the type of guy who care about his looks so much and i said i should fight for my hair . Also i want to be a steward and hair loss could be a problem for that . I started minoxidil in january this year , dermarolling my hairline and a protocol with scalp massages from march till this month ( but i stopped ) and i gained some results but not enough and my hair is still falling pretty much . I would try finasteride 1 mg 3x a week along minox and dermarolling . Man , hair loss really sucks ( especially at these ages )

      Sorry to hear my friend. If you care about your hair to such an extent, I would talk to your doc about oral finasteride. I started taking it at 20 but feel like I could have saved some density on the crown if I had started earlier. That being said, dht is involved in penile development. Should be ok if you are above 18, but there is a theoretical risk. Hope this helps

      @@catalinn6924 if i was on your position, I would find a new doc and take 1 mg per day and then decrease the dose if I had side effects. From a medical perspective, finasteride 1 mg once daily is completely safe for individuals 18 and older as this dose has been approved by the FDA for treating male pattern hairloss in individuals 18+.

    Took finasteride for close to 5 years and developed post finasteride syndrome in 2019 at the age of 23. Totally destroyed my life. It’s not just sexual stuff either. I ended up with severe physical/mental sides as well which is far worse.

      @@therealcrazymonkey1251 Hi man I started getting sides after a couple of years of finasteride use. I got referred to very helpful endocrinologist who told me about how many patients he treats with HRT because of their finasteride usage. My symptoms disappeared after he put me on hormones. I got PFS after I quit the drug in 2019 and I haven’t gotten any better since.

      @@Raptor-oi5cx doctor who gave it to you at age of 18 is insane, since in some cases hormonal changes last untill age of 24. My doctor send me off when i asked him in my early 20s, he told me to come back when i am at least 25. I did that and no problems here.

    I wanna try finasteride, but i'm very afraid of the side effects, especially gynecomastia, is there any specific blood or semen analysis (hormones maybe) that i can take before starting finasteride to make sure that i'm safe from developing gynecomastia while taking finasteride ?

      You can get a full hormone panel but it won’t help you really. You don’t know how your body will treat the extreme testosterone. The odds of that happening are extremely slim the sides are rare and you can always start off at .25mg every other day and titrate up to determine if you will get side affects.

    So uhm why on earth are we not discussing possible side effects on chest and breast cancer? My doctor warned me that it might increase the chances of lumps in the chest and in extension breast cancer. Not sure how many recent studies have been made on the topic but one from 2010 had linked 5 cases of Finasteride 1mg a day to breast cancer.

My doctor told me the same, when i started taking finestirade, i was experiencing nipples sensitive and getting bigger, and my breast was growing as a women's, so doctor told me some men coudl experience breast cancer. My horrible side effect was more depression and i quiet after 2 years of treatment. Now im doing only topical minoxidil .

    I've had no sexual side effects so far and the results are promising but recently Im getting a lot of cystic acne around and below my jawline. Do you think finasteride could cause it?
