Hi - the good news is that you were positive for antibodies response. More important to see how it changes over time. If you have a chance to repeat the test after a couple of months (same test), that will be a good way to see if your antibody levels are stable.

Hello Hassan - It seems to me that the first test is measuring total antibodies and the second is measuring only IgM. Measured levels are higher than the reference, therefore you can conclude you have these antibodies present in your system. Keep in mind IgM develop early in the lifecycle of the infection and disappear quickly, IgG take longer to develop but persist much longer. But in your case, I cannot say if the first test is giving you IgG or not. Did you develop any symptoms of COVID and recovered? How do you feel now?

Hi Milind - it confirms your immune system response. This test is measuring both IgG and IgM and the result is higher than the minimum. It also confirms a prior infection even if you did not have any symptoms.
Hi doctor Thanks for the detailed video, the first day of covid symptom for me was on 12th April, I lost my smell on 17th April and got the smell and taste back on 26th April. I had taken the covid antibody test on 6th may 2021. Covid antibody IGG C.L.I.A = 3.43 index reference range is >=1 index is positive and 1.00 Actually during the entire cycle of covid infection I had very mild symptoms. my index value for IGg antobodies is 6. the report just says unit is index. Index reference range less than 1 is found nagitive to IGg and greater than equal to 1 is found positive.( method C.L.I.A ) The antibody test is almost taken 1 month after i was detected positive in RT-PCR. I have 2 question 1- Since there is no upper range in index that you have been mentioning in replies , what index value could still determine strong immune respone to infection 2- How do we know cutoff necessary to qualify convalescent donor plasma for use in donations based on such reports.
Hello Rati - there is an upper value for each test, but it is not generally publicized. You will need to look at the technical documentation for the test. This information is typically available in the IFU or maybe the lab knows. I will answer your questions based on my current understanding. 1- There is no general correlation between antibody level and a "strong" immune response. Each individual is different. The most important thing to understand is that if the result is positive, then your immune system has responded to the virus. It does not represent a "strong" or "weak" response. Also, note that the antibody level will change over time and may not correlate to the new variants. 2- On your question about the cut-off for convalescent plasma - again, there is no universal cutoff or minimum value. Not all convalescent plasma will be equally effective. It should be derived from someone known to have recovered. I know, they will likely want to have a high level of antibodies in the plasma, but I am not aware of any cutoff that is generally applied before plasma is accepted. So what to do? Continue to be cautious. Get the vaccine as soon as it is available to you. Let me know if this helps.

Hi - If I understand correctly, you did a antibody test on 4th Feb, which was positive with a result of 162.5. This could also be due to the antibodies in your system from the vaccine. You mention that you are due for the Covidshield booster, so I am assuming you have completed the primary vaccine series. I would strongly recommend you to take the booster dose if you are eligible, and if you did not have any serious side effects from the primary vaccination. Also, always consult with your doctor for the final recommendation. Hope this helps.

The booster will start showing its effect in about 2 weeks for continued protection. Keep in mind, you can still get infected, but you will be protected against very serious effects of COVID.

Thank you sir , very informative! I have got my sars-cov IgG test done in last week, but result came as 8.8 OD. I couldn't comprehend the quantity with this unit as there was no reference range given in my report( it was from thyrocare labs), the report only said positive IgG with 8.8 OD. But after watching ur video I can say that I have pretty good high level of antibody. But I have one little doubt, if this reference rage (1-10 OD) can vary from instrument to instruments deployed to carry out detection test??
Hi - the good news is that you have antibodies in your system. The exact value is not that important. Plus, it also changes over time. You are right that the exact number may depend on a variety of factors. It will also be different for different types of tests from different manufacturers.
Hi Marko - every test is different, therefore, it is not possible to compare them directly. Only the test developer can specify the range for positive, negative and uncertain. It all depends on the signal-to-noise ratio for the test. In this case, your result show you are unlikely to have IgG antibodies, which means it is very unlikely that you had a prior infection. You mention that you took the test about 3 months after your exposure. Since you did not have any symptoms, it is possible that you had a quick immune response to any virus you may have acquired and that you didn't really develop a high level of IgG antibodies, which typically appear 2 weeks after the initial exposure. Or, the more likely scenarios is that you were not infected. I would recommend discussing with you doctor to see if you should do a second test. In the meantime, best to continue practice social distancing and using a face mask or a face shield when in public. Best of luck and stay safe!

Hi - thank you for your comment. I would strongly recommend getting vaccinated unless you have some pre-existing medical conditions that might be of concern. A positive antibody test indicates a past infection. It does give you a level of protection but when you get the vaccine, you are more protected. Of course there is no such thing as absolute immunity, and even those who are vaccinated are at risk of re-infection. But you will lower your risk by a lot if you get vaccinated. Sir I got my antibody test and the value came out to be 73.77u/ml. Is it sufficient to protect from future reinfection? Also is the antibody less or more according to what you encounter in your practice?
Hi - unfortunately, there is no "official" number yet that we can use to figure out your risk of reinfection. Compare your result to the range of the test method used (min to max). Also, you cannot compare results from two different tests. It is best to monitor the change in the antibody level over time by using the SAME test. Hope this helps.

Hi - It means there are antibodies present after COVID-19. The value of 145 is hard to interpret because we don't know the reference point. A higher number should indicate more antibodies in the system.

Hi Ajaz - you should compare the number to the reference value for the same test. Without the reference number, it is difficult to interpret. In general, a higher number means you have more antibodies in your system. Keep in mind that this level will change over time, so you may not have the same level after say 6 months or so. Current data shows that most people who have recovered from COVID are able to maintain a good antibody level for up to 6 months.

Hi Manoj - I am not familiar with this specific test. I would say that your level is higher than the min of 1, but not sure how it compares with the upper end of the range. I am surprised you had a negative RT-PCR even though you were asymptomatic. This could be the case if the specific RT-PCR you had did not have good sensitivity. Another possibility is that you got COVID again after Sept 2020 and were again asymptomatic. This could result in antibodies in your system as shown by the test in March. We cannot say much about the "level of immunity" looking at your IgG number. Practically speaking, this number is not very useful. Just the fact it is positive is more important to know. Hi Manoj - some of the symptoms you describe, such as loss of taste and smell, are consistent with reports of COVID-19. Since you are getting a reactive antibody test results, it means your immune system has responded to the infection. To what extent you have immunity against new variants remains unknown. The risk may be lower but not zero. So you are doing the right thing by using a mask and social distancing. I would advise you to keep doing that. Best wishes again and stay safe.

Dear Doctor, I also got IgG quantitative antibody test through CMIA method for my wife after 5 weeks of Covid symptoms first appeared, her antibody levels were 9952 au/ml, the test reference range is as follows , Negative 50 to 80,000 au/ml. During Covid time, my wife had mild pneumonia and was on low dose steroids for 24 days, her Oxygen levels always maintained with out any support, however she had persistent cough but there was no hospitalisation, She did all household activities and performed daily chores. After antibody test, when I saw such a high level of antibodies value, then I got her retested next day through CLIA method and the result was positive again with antibody >150 Au/ml, this time reference range was - negative if less than 1 au/ml and positive when more than 1, I think that laboratory's test range could calculate up to 150 only, so in my case result was shown >150. I showed her report to a MBBS, MD Doctor along with her latest CBC, LFT, KFT, Thyroid, electrolytes and other routine blood test values which were almost normal. Doctor said there is nothing to worry. My wife has not been vaccinated. At that time her CRP( C reactive protein value) was on slightly higher side - 8mg/litre, Please comment on antibody levels.

IgM and IgG are two very different kind of antibodies. IgM is generated in the early phase of the infection and disappears more quickly. IgG appears later and lasts for a longer time. It depends on the type of test and the timing of the test. Absolute numbers are not easy to interpret unless you know the reference values of the test. Also, note that IgG does not always mean that that these antibodies are able to "neutralize" the virus, but they are a sign that your immune system is responding to the infection.
Hello Surajit - it is hard to interpret the IgG value without any reference. IgG is one type of antibody, which may or may not neutralize SARS-CoV-2. There are other tests which measure the neutralizing antibody levels. Your question about immunity is still being studied since COVID-19 is so new and not much is known yet. So far, there is data that shows that neutralizing antibody levels can persist for up to 50 days.

Hi Chris - I am not very familiar with how CT is used to diagnose COVID and what is considered normal. Also, I do not understand what the CT number of 32.47 mean in your case. My understanding of a CT-based COVID diagnosis is that the scans can show different patterns (spots, opacity, thickening etc.). So you should ask your doctor what exactly do they see and what they would consider as a normal level. Having said that, even if your CT returns to whatever "normal" is, it does not mean you have no virus in your system. It may just be circulating somewhere else. @@atlechris Oh ok I understand better now. I misunderstood CT to mean a CT scan. The CT, in this case, is Cycles threshold and 40 is the upper limit. Above 40, the test result is considered negative. In a way, the closer you are to 40, the less virus you have in your system. Does this help?

i agree tangible proof of Immunity is what we should be proving and measuring, and not just taking additional vaccines and boosters without measuring our antibody levels first. You don't add oil to the engine without first looking at the dipstick first to see how much it may need!

Are you saying that it would be better to first measure the antibody level and if it is low, or has fallen down by a lot after the 2nd dose, then we should take the booster dose? I think that makes a lot of sense. In general, you can expect to see a reduction in antibody levels 5-6 months after your last vaccine dose for the mRNA vaccines. But again for some people this may be too long or too short a time gap.

Hi Rohit - the absolute number is difficult to understand unless you know the baseline number. The reactive result 6 months after infection means you have antibodies but it is hard to understand how much. Also, note that not every test measures what they call "neutralizing antibodies". If you take the same test over time, you can compare the numbers.
FYI: you mentioned that you were focusing on the ADVIA analyzer but your data is from the Atellica analyzer. The index range for the ADVIA Centaur is 0.05-10.
@@pranshusharma7600 Ok, this is a type of a test that can give you the level of total antibody in your system. Do you know the range of the specific ECLIA test you did? Does it tell you the minimum and maximum amounts measured by this test? There is one test from Roche Diagnostics that is authorized by the FDA in the USA. I am not sure if this is the same test for you, but this one has a range from 0.8-250. Now, keep in mind that the absolute number may not mean much in terms of your "immunity". It just means you have the antibodies and your immune system has responded to the virus.
Hi Shahzad - it means you have IgG antibodies according to the test you took. Whether it is high or low, depends on the reference value of the test. Each test is different. Also, note that IgG are only one kind of antibodies in response to COVID-19 infection.

@@fredharvey2720 You make a great point. Most of the antibody testing data is in the clinical trial results for vaccines. One point to remember is that they also did more sophisticated tests to show increase in the level of neutralizing antibodies. These tests are generally not available to the public. They are showing it to prove the vaccine works but the effectiveness data comes from counting the number of infections and comparing against those who did not receive the vaccine in clinical trials. It is not possible to correlate but I am sure there are mixed messages going out in different sources of information.
Hi - it is difficult to interpret the number 0.19 without any reference. The test should also tell you a yes/no result for presence of IgG. If there IgG test for SARS-CoV-2 shows a positive result, then you were exposed.

Hello Wahid - the cutoff value means any number more that 1 is a positive result. Observed value of 175 needs to be compared to the max value of the specific test. It means you have antibodies in your system (most likely IgG).

Hi - the good news is that you were positive for antibodies response. More important to see how it changes over time. If you have a chance to repeat the test after a couple of months (same test), that will be a good way to see if your antibody levels are stable.

Hello Hassan - It seems to me that the first test is measuring total antibodies and the second is measuring only IgM. Measured levels are higher than the reference, therefore you can conclude you have these antibodies present in your system. Keep in mind IgM develop early in the lifecycle of the infection and disappear quickly, IgG take longer to develop but persist much longer. But in your case, I cannot say if the first test is giving you IgG or not. Did you develop any symptoms of COVID and recovered? How do you feel now?

Hi Milind - it confirms your immune system response. This test is measuring both IgG and IgM and the result is higher than the minimum. It also confirms a prior infection even if you did not have any symptoms.

Thanks for the detailed video, the first day of covid symptom for me was on 12th April, I lost my smell on 17th April and got the smell and taste back on 26th April.
I had taken the covid antibody test on 6th may 2021. Covid antibody IGG C.L.I.A = 3.43 index reference range is >=1 index is positive and 1.00
Actually during the entire cycle of covid infection I had very mild symptoms. With low level of antibody like 3.43, does that mean I’m asymptotic?

PCR tests are to determine if you HAVE covid. Antibody tests are for if you HAD it.

Hello Rati - there is an upper value for each test, but it is not generally publicized. You will need to look at the technical documentation for the test. This information is typically available in the IFU or maybe the lab knows. I will answer your questions based on my current understanding.
1- There is no general correlation between antibody level and a "strong" immune response. Each individual is different. The most important thing to understand is that if the result is positive, then your immune system has responded to the virus. It does not represent a "strong" or "weak" response. Also, note that the antibody level will change over time and may not correlate to the new variants.
2- On your question about the cut-off for convalescent plasma - again, there is no universal cutoff or minimum value. Not all convalescent plasma will be equally effective. It should be derived from someone known to have recovered. I know, they will likely want to have a high level of antibodies in the plasma, but I am not aware of any cutoff that is generally applied before plasma is accepted.
So what to do? Continue to be cautious. Get the vaccine as soon as it is available to you. Let me know if this helps.

Hi - If I understand correctly, you did a antibody test on 4th Feb, which was positive with a result of 162.5. This could also be due to the antibodies in your system from the vaccine. You mention that you are due for the Covidshield booster, so I am assuming you have completed the primary vaccine series. I would strongly recommend you to take the booster dose if you are eligible, and if you did not have any serious side effects from the primary vaccination. Also, always consult with your doctor for the final recommendation.

I couldn't comprehend the quantity with this unit as there was no reference range given in my report( it was from thyrocare labs), the report only said positive IgG with 8.8 OD. Hi - the good news is that you have antibodies in your system. The exact value is not that important. Plus, it also changes over time. You are right that the exact number may depend on a variety of factors. It will also be different for different types of tests from different manufacturers.

Hi Marko - every test is different, therefore, it is not possible to compare them directly. Only the test developer can specify the range for positive, negative and uncertain. It all depends on the signal-to-noise ratio for the test. In this case, your result show you are unlikely to have IgG antibodies, which means it is very unlikely that you had a prior infection.
You mention that you took the test about 3 months after your exposure. Since you did not have any symptoms, it is possible that you had a quick immune response to any virus you may have acquired and that you didn't really develop a high level of IgG antibodies, which typically appear 2 weeks after the initial exposure. Or, the more likely scenarios is that you were not infected.
I would recommend discussing with you doctor to see if you should do a second test. In the meantime, best to continue practice social distancing and using a face mask or a face shield when in public.

Hi - thank you for your comment. I would strongly recommend getting vaccinated unless you have some pre-existing medical conditions that might be of concern. A positive antibody test indicates a past infection. It does give you a level of protection but when you get the vaccine, you are more protected. Of course there is no such thing as absolute immunity, and even those who are vaccinated are at risk of re-infection. But you will lower your risk by a lot if you get vaccinated. Hi - unfortunately, there is no "official" number yet that we can use to figure out your risk of reinfection. Compare your result to the range of the test method used (min to max). Also, you cannot compare results from two different tests. It is best to monitor the change in the antibody level over time by using the SAME test. Hope this helps.

Hi Ajaz - you should compare the number to the reference value for the same test. Without the reference number, it is difficult to interpret. In general, a higher number means you have more antibodies in your system.
Keep in mind that this level will change over time, so you may not have the same level after say 6 months or so. Current data shows that most people who have recovered from COVID are able to maintain a good antibody level for up to 6 months.

Hi Manoj - I am not familiar with this specific test. I would say that your level is higher than the min of 1, but not sure how it compares with the upper end of the range. I am surprised you had a negative RT-PCR even though you were asymptomatic. This could be the case if the specific RT-PCR you had did not have good sensitivity. Another possibility is that you got COVID again after Sept 2020 and were again asymptomatic. This could result in antibodies in your system as shown by the test in March.
We cannot say much about the "level of immunity" looking at your IgG number. Practically speaking, this number is not very useful. Just the fact it is positive is more important to know.

Also i lost my taste and smell perhaps in sept and thus doubt that the sampling done by test person was not correct . do i have some protection from new strains (i mean not to get severe case result if again getting reinfected) , although i think thanks to my mask and other safety protocols i followed the viral load in first infection made me asymptomatic , again thanks doctor for your comment
Hi Manoj - some of the symptoms you describe, such as loss of taste and smell, are consistent with reports of COVID-19. Since you are getting a reactive antibody test results, it means your immune system has responded to the infection. To what extent you have immunity against new variants remains unknown. The risk may be lower but not zero. So you are doing the right thing by using a mask and social distancing. I would advise you to keep doing that. Best wishes again and stay safe.

I had Covid Mild symptoms and after 15 days Got IGG Anti Spike protien done.
However the result was 35634 AU/Ml quantitative antispike Sars cov igG antibody @CMIA. I haven't heard of this much antibodies.
Is this normal or good immune response or something to deep dive on.
Hi - I am not aware of this test. Also, I would not expect IgG to be so high only 15 days after the infection. Please check with the test manufacturer for clarification.
Dear Doctor, I also got IgG quantitative antibody test through CMIA method for my wife after 5 weeks of Covid symptoms first appeared, her antibody levels were 9952 au/ml, the test reference range is as follows , Negative 50 to 80,000 au/ml. During Covid time, my wife had mild pneumonia and was on low dose steroids for 24 days, her Oxygen levels always maintained with out any support, however she had persistent cough but there was no hospitalisation, She did all household activities and performed daily chores. After antibody test, when I saw such a high level of antibodies value, then I got her retested next day through CLIA method and the result was positive again with antibody >150 Au/ml, this time reference range was - negative if less than 1 au/ml and positive when more than 1, I think that laboratory's test range could calculate up to 150 only, so in my case result was shown >150. I showed her report to a MBBS, MD Doctor along with her latest CBC, LFT, KFT, Thyroid, electrolytes and other routine blood test values which were almost normal. Doctor said there is nothing to worry. My wife has not been vaccinated. At that time her CRP( C reactive protein value) was on slightly higher side - 8mg/litre, Please comment on antibody levels.

IgM and IgG are two very different kind of antibodies. IgM is generated in the early phase of the infection and disappears more quickly. IgG appears later and lasts for a longer time. It depends on the type of test and the timing of the test. Absolute numbers are not easy to interpret unless you know the reference values of the test. Also, note that IgG does not always mean that that these antibodies are able to "neutralize" the virus, but they are a sign that your immune system is responding to the infection.

Hello Surajit - it is hard to interpret the IgG value without any reference. IgG is one type of antibody, which may or may not neutralize SARS-CoV-2. There are other tests which measure the neutralizing antibody levels. Your question about immunity is still being studied since COVID-19 is so new and not much is known yet. So far, there is data that shows that neutralizing antibody levels can persist for up to 50 days.

Hi Chris - I am not very familiar with how CT is used to diagnose COVID and what is considered normal. Also, I do not understand what the CT number of 32.47 mean in your case. My understanding of a CT-based COVID diagnosis is that the scans can show different patterns (spots, opacity, thickening etc.). So you should ask your doctor what exactly do they see and what they would consider as a normal level.
Having said that, even if your CT returns to whatever "normal" is, it does not mean you have no virus in your system. It may just be circulating somewhere else.

@@atlechris Oh ok I understand better now. I misunderstood CT to mean a CT scan. The CT, in this case, is Cycles threshold and 40 is the upper limit. Above 40, the test result is considered negative. In a way, the closer you are to 40, the less virus you have in your system. Does this help?

i agree tangible proof of Immunity is what we should be proving and measuring, and not just taking additional vaccines and boosters without measuring our antibody levels first.
You don't add oil to the engine without first looking at the dipstick first to see how much it may need!
Thanks for sharing your thoughts. Are you saying that it would be better to first measure the antibody level and if it is low, or has fallen down by a lot after the 2nd dose, then we should take the booster dose? I think that makes a lot of sense. In general, you can expect to see a reduction in antibody levels 5-6 months after your last vaccine dose for the mRNA vaccines. But again for some people this may be too long or too short a time gap.

I see it this way - there is a public health policy issue at the overall population level, then there is an individual health issue at an individual level. I don't talk about policy issues and decisions on my channel. But I think we should understand the data in a general sense to figure out what decisions we should be making at a personal/individual level. The best course of action is to talk to your doctor with a full understanding of general information and how it may or may not apply in your individual situation.

@@James_DahlenHi - so far, I have not heard any advice or guidance based on antibody levels alone. At an individual level, I always advise to look at your whole situation, including your exposure level and the transmission rate in your area, your medical history etc. This decision is best taken after a carful look at your individual situation and talking to your doctor.

Hi - you need to compare it against the reference level for the test. Each test is different and the absolute value does not mean much unless it is compared against the reference value. In general, a higher number means more antibodies, but remember there is not direct correlation between the level of antibody and immunity against the virus. It depends on the type of antibody.

Hi Rohit - the absolute number is difficult to understand unless you know the baseline number. The reactive result 6 months after infection means you have antibodies but it is hard to understand how much. Also, note that not every test measures what they call "neutralizing antibodies". If you take the same test over time, you can compare the numbers.

@@pranshusharma7600 Ok, this is a type of a test that can give you the level of total antibody in your system. Do you know the range of the specific ECLIA test you did? Does it tell you the minimum and maximum amounts measured by this test?
There is one test from Roche Diagnostics that is authorized by the FDA in the USA. I am not sure if this is the same test for you, but this one has a range from 0.8-250.
Now, keep in mind that the absolute number may not mean much in terms of your "immunity". It just means you have the antibodies and your immune system has responded to the virus.

Sir i got SARS-COV2 (covid-19) IgG antibody test done 14 days after second does of vaccine and the result came 6.73 positive reference range being >1.0(index) positive but on 23rd day o got the same test done again and result came 1.82(positive) why did the numbers decrease pls explain
Hi Nakul - it is not uncommon for antibody levels to change over time. Are you sure it is reporting only IgG or does it also include IgM antibodies?

@@NaveenAgarwalPhD My employer explicitly states it will not take antibody tests as proof of immunity. It's either get the jab, get an exemption (I got a religious exemption, actually) or get fired.
Due to this, I haven't bothered discuss with my doctor.

To me, this reeks of politics and I strongly suspect Big Pharma is paying off politicians to ignore natural antibodies to market more doses. @@fredharvey2720 You make a great point. Most of the antibody testing data is in the clinical trial results for vaccines. One point to remember is that they also did more sophisticated tests to show increase in the level of neutralizing antibodies. These tests are generally not available to the public. They are showing it to prove the vaccine works but the effectiveness data comes from counting the number of infections and comparing against those who did not receive the vaccine in clinical trials. It is not possible to correlate but I am sure there are mixed messages going out in different sources of information.

Hi - it is difficult to interpret the number 0.19 without any reference. The test should also tell you a yes/no result for presence of IgG. If there IgG test for SARS-CoV-2 shows a positive result, then you were exposed.

Hello Wahid - the cutoff value means any number more that 1 is a positive result. Observed value of 175 needs to be compared to the max value of the specific test. It means you have antibodies in your system (most likely IgG).
