    As someone with proprioception issues from sensory processing disorder I love the fact that he highlighted that especially because I have the most problems with my feet in space as apposed to any other limb/body part and I also love that he highlighted the wear and tear pattern of his shoes because having had my feet mapped for the correct shoes for me it’s amazing the difference that had on my posture and reduction in pain from hereditary joint problems and also on my proprioception, great video!

    Quick question, saw a couple other videos where I thought a two point discrimination tool would have been more optimal than the monofilament sensation. Dr Gill, why do you prefer the monofilament. Thanks in advance from a learner!

    Would it be better to not inform the direction of the toe before the test to get more accurate results on whether they acually know the direction instead of memory ?

      We need to inform the patient as to what the sensation is, because we are actually holding the toe. So we’ve already added an additional variable into the mix.

    Diabetic foot examination, So does this mean we do this test on people who are already Diabetic ? Or is it more like we are checking to find if they are Diabetic or not ?

      This test should be done annually for diabetics and when they are first diagnosed

      Impaired blood supply - can happen in diabetes with peripheral vascular disease

      @@DrJamesGill its the way you stand in relation to the patient, your body language, providing that security that your there for them, it's difficult to explain but while i was seeing the patient, i felt i was behaving like you do, which by the way i praise, your voice is so calming and your sooo relaxed. However there was also a negative point 😅because when my next patient came in, off course a child, i was like ohhh DR Gill, when are you going to show us a simple way of calculating antibiotic dosage in children, in mls. I don't know if you have read my previous comments😅, i have asked for it a few times but not getting it 😅, don't know what it is but i just get confused, hope you get time to show us that too.

      I was also wondering so I googled what I thought I heard ("Shark out foot"). It is Charcot foot, a neuropathy that makes a person less responsive to foot injuries.

      Callouses can come from many sources, but it tends to be dead skin that's under high pressure that fuses together to form a harder outer shell. afaik it's more common in diabetic patients with poorer glucose control, but can have many other sources, including just not washing/exfoliating dead skin under the legs (should be careful using skin files when diabetic, if you have impaired sensation you can easily be too rough and cause a wound)

    Wish you were my doctor im a newly diagnosed diabetic type 2 have no idea what it is an im a liver patient an i have cholesterol im very young an i have tachy cardia , my doctor told me i got diabetes because most of my family members have diabetes an my mother an grandmother have heart problem as well, ps i need a good doctor who can explain everything to me as im still very young with these diagnoses 😢 help 😢

      But it is reasonable to ask your Gp for a consultation to get more information, that’s always a good consultation for me, as I’m able to directly help 😊

      @DrJamesGill thank you I'm in the US I was just wishing you were my doctor wishful thinking as from watching you you're very good US doctors like to do surgery for every thing as they recently did surgery on me they did a route en y gastric bypass surgery on me to control my diabetes because they said I became overweight because of my liver, I've never been overweight in my life but because my liver numbers were bad it made me gain 100 pounds in lesss than year so they opted for surgery rather than other alternatives I don't know why US doctors are as such 🤷‍♀️ they talked me in this surgery an I regret it as I'm not feeling very well like my normal self 🥺😭 an all they tell me is you're going to be fine an I don't feel fine

    For some reason Dr. Verghese at Stanford does not like those Taylor hammers and confiscates them from his students caught using them. He prefers the Queen's Square hammer instead. But if Doc Gill uses a Taylor then it's A-OK as far as I'm concerned! ;)

    anyone felt it contained too much info which might mean the patient could recognise themselves. We need to be very careful with public cuss things like this I not only anonymise the details but CHANGE some of the details as well so there is no chance of someone being identified.
    Please take this as a learning point, not a rebuke 😊
