Can someone or Dr. Brewer explain to me what is the way to check if I have insulin resistance if I am on a keto diet?... glucose tolerance test then probably makes no sense and is unreliable. I don't want to come off keto because then it's hard to come back. .... but my glucose is almost 100 ....isuline 5 to 7
If you plan to remain keto, you might not want to test. But the only reliable way to test is a glucose challenge. Get a fasting glucose. Then take 75 or 100 gm glucose. Test again at 60 & 120 minutes. The Kraft Insulin Surevy protocol uses 100 gm glucose. It tests at fasting, 30,60,120, & 180 minutes. It also includes insulin at each glucose test. It is definitive. You miss about 20% of insulin resistance if you don’t test for insulin.

About a year ago, I showed my cardiologist a picture of a box of "Heart Healthy" Cheerios. He just smiled at me and said he really did not have an opinion about that "Heart Healthy" labeling!

I woke up this morning and did 300 situps, 40 pushups, walked around the block, ate 4 grass fed eggs, two strips of bacon and a fresh tomato from the garden. I learned about Carnivore almost a year ago and find it very easy to stick with. The thought of eating cereal or bread doesn't even cross my mind. I'll be 60 YO in December, 2024!

@@PrevMedHealthpeople get upset when someone has like a tomato, or some coffee, or blueberries but my cats are obligate carnivores they eat grass, and what happens to the organ meats when a wolf eats a bunny? They get undigested plant matter in theirs.
I wish more people that talked about carnivore or ketovore had these ideas. 20 years ago what I called carnivore/ketovore, was a "Nomad Diet" you eat meat and if you come across some veggies or an occasional fruit you ate some and when stuff wasn't in season you didn't.
Wish I had eaten like that back then rather than follow the standard advice and got from 220 to 370lbs and now back to 220 after keto made the lbs slip away and I can do push-ups for the first time in my life despite going to the gym as a 220lb 20 year old. Now I'm a 220 lb 40 year old that can do pushups.

Dr Brewer, you really went 'all-out" with this video. It has to be the most detailed, yet easily understood, explanation of insulin resistance, pre-diabetes and cholesterol physiology I have seen packed into just shy of 42 minutes. I will recommend this to my physician colleagues. It is a perfect 'primer' toward understanding metabolic disease.

@@johnsiekrause4109 from a olive oil store by me, the owner imports his oil from growers in Italy Spain and Portugal that he visits each year and works with the growers. Very high quality olive oil with some of the highest antioxidant levels I've seen and very fresh within a month or two of pressing. It's expensive, 25-30 dollars for a 500 mil bottle. Wine bottle size are about 35-70 depending on the verity quantity and antioxidant level. They sell out fairly fast on the high antioxidant level olive oils. The tast is unbelievable compared to store bought. All his oils are third party tested by one of the best labs and then results are shown above each selection !

Rogan asked a simple question and this Prof. went on and on and on going around in circles and he ever did answer the question. Thanks for sharing, Dr. Brewer. You're a wealth of information. I'm postmenopausal; recently turned 57. Two years ago, my functional med doc ordered a non-fasting lipid panel. My LDL-C was 191. My HDL-C A 92, and my Triglycerides A 46. Since the latter 2 are "good", my doc did no treatment. Fast forward about 3 months ago, I go to a cardiologist just to check in on things (and preventative measures). He ordered a calcium score test and said if it looks good, he won't prescribe a statin. Well, my score is 176, so a big red flag for them to prescribe a statin. I asked for a current blood panel. No orders to fast or not to fast; if it was a complete panel, or just ldl/hdl/trigylcerides, so I waited to talk to the APNP, last week, before taking the statin. Scary for me to just take 20mg of a statin. What statin? I had a lot of questions. I knew from watching a video of yours that you took Crestor (and with my own research), I felt more comfortable taking it, but still didn't want to. Well, from the fear of the plaque, plus her telling me that I have coronary artery disease and that I had to get my cholesterol down, asap, or I could have a heart attack, scared me into taking the statin (2-days in). There is paternal heart disease in my family. My dad passed at 56 from it. He was also a heavy smoker. I am healthy overall. I am 5'9" tall; weigh 107; my weight is too low at this point (went on a plant-based diet). I don't smoke. I'm fairly active, but could be more as time & my energy returns. I don't want to be on a statin for the rest of my life. I am a big believer in naturalpathic medicine; life style and wholistic healing. What do you think about Curcumin? Curamed brand?

@@jamescalifornia2964, yes, but it caused flushing. I do take a Bcomplex that has niacin in it.
@@LaKayDailey ~ I use SloNiacin. Sustained release, no flushing. I sometimes also take small doses several times a day. 👌
Have you tried taking Niacin within a meal? I had bothersome flushing taking it even after a meal. I found that if I take in the middle of the meal, I don't generally get flushing at all, and if I do, it is much less intense. Hopefully that doesn't mean it is being negated by the food. (I don't think so... it appears to have raised my HDL by 10% and lowered my LDL by about the same amount.) That was taking 1000 mg per day of the Nicotinic Acid variety. Also, I have seen in multiple places that if you take an aspirin just before the Niacin, the flushing is eliminated or reduced, but I haven't tried this.

I recently went on 250 miles of backpacking trips and got off LCHF diet for about a month during and between the two trips. Total C went way down but Trigs/HDL ratio suffered a little (stayed under 2 though); hba1c went up. For me, diet clearly beats exercise as far as metabolic health goes. Or maybe better to say the foundation of metabolic health starts with diet. Next year, I will go through the effort to stay on LCHF out on the trail.
@mystatinfreelife, I find I can hike backpack much easier staying low carb fat adapted. No energy bonks! Just steady energy through the day and can hike like a mountain goat, love it.
I'm 71, haven't been healthier in a couple of decades, am probably slightly underweight (for the first time), have a reasonable blood pressure, and my overall cholesterol is ... 300(!) I've been on statins for about five days, but...I'm going to go OFF of them, thanks to MANY doctors (including YOU) letting me know that the Lipid Panel used today seems...obsolete. I WILL likely get an apoB evaluation, or a more detailed study of my LDLs. I use ghee, cold-pressed sesame oil (NO other seed oils), MCTs in my coffee (1or2 per day) and a high grade olive oil. Very LITTLE sugar! I really learned a lot from this presentation. THANKS!

First time I ever heard anyone describe APOB. Betcha even most MDs can't describe it!
I'm SUPER happy to have found Dr. Brewers channel in the midst of my struggle. And while I'm slightly different than the average due to heavy metal poisoning and hypersensitivity/allergies yet despite my documented proof, my primary consistently gaslights me and focuses on ldl & cholesterol.
I've lost 43 lbs on carnivore but that's been my only benefit. I'm feeling well, in a LOT of joint & muscle pain and a crp that's now through the roof.
I just wish I could get a doctor who does carnivore or keto and takes insurance
And the crazy amount of iron they add to it is ridiculous

Thank you for taking your time to really help people Doc. Five years ago I woke up at 3am with PMR. Six months later I started a keto life style. Today I played tennis for 2+ hours in hot and humid Florida, then went to a diner and had 3 scrambled eggs with 5 slices of bacon just like I eat every day. Sometimes 6-7 slices. I'm 78 and feel better than I did when I was 50. My PCP sends me to everyone he can think of for further testing. My numbers are pretty dang good-- better than his, I bet.

Also, if you are a carnivore or truly keto; don't, do the craft test unless you consumed carbs prior to the test for at least 72 hrs. If not, your pancreas will be so sluggish, it will not know what to do with 75 mgs of carbs, and you will look like a type 2 or 1 diabetic.

"ldl cholesterol concentration" is positively linked with "systemic microbial load", and serves as an "antibiotic function". Accordingly, reduction of "ldl cholesterol concentration" would reduce systemic antimicrobial capacity, thereby accelerating rate of biological aging and progression of aging associated diseases (like sarcopenia and dementia).

Good point. However, for those immune to "placebo effects", statins may reduce their cognitive and physical healthspan. There is already a growing body of published academic research to indicate the possibility of such a phenomenon. For example, convincing research published in the "journal of nuclear medicine" by Dr. Silverman's team at UCLA reveal that statin use is associated with more than double the risk of converting to dementia over eight years of follow-up compared with statin non-use, and with highly significant decline in metabolism of posterior cingulate cortex -- the region of the brain known to decline the most significantly in the earliest stages of Alzheimer’s disease.
I went to steel cut organic oats. I limit my sugar and carb intake and avoid highly processed foods. I think grass fed animal protein is healthier and more nutrient dense. same with pasture raised eggs and chicken.

Interestingly, Dave Feldman's latest preview of their Cleerly CCTA results show that LMHRs and the MI Heart matched cohort not using LDL meds both have about the same soft plaque. For some reason LMHRs' plaque isn't calcified as much as those in the MI Heart cohort. This raises even more questions about the traditional Lipid Hypothesis. But it also begs the question about how plaque progresses without getting calcified.

Regarding A1c screening missing diabetes, the opposite can also be true. A1c can be elevated due to causes other than average glucose. This isn’t discussed much. I’d love to hear a deep dive on this.

I have been sloppy keto for several years. Many of my markers are improving. (-30 lbs. Trig/HDL = 1.5)
I will continue to inch toward carnivore to combat my insulin resistance. (my 2hr insulin = 76 µIU/mL)

Crushed put in glass of hot water, ran a magnet on the outside and the metal followed the magnet!

I have over 7 months lost ~17lbs and down a couple pants sizes. My pants size is 32 now so I was thinking my waste was ~32 inches. Just measured the circumference inside my pants and where my pants sit on my body. It is 36 inches. At my belly button it is 41 inches. Wow! I am a little shocked. I had figured waste at belly was about 36 estimating my pants. Whoever came up with the pants size measurements system need a little talking too. 😅 - So simple to measure and I actually had never done it before.

Insulin resistance = end point glucose saturation without cellular fasting. Fasting/High intensity exercise triggers cellular rejuvenation pathways, and makes cells insulin sensitive again via turnover of cellular components (replacing damaged glycosylated components with newly synthesised components and upregulation of catabolic processes that burn energy). Between the war on saturated fat (which satiated hunger) and promotion of snacking ultra processed carb junk (for profit) our cells never get the fasting they need. You only need fasting of 12-14 hours 3x per week or 3x HIIT training).

This is easy to digest and makes so much sense. At age 28, my LDL was 160-155, and in my 30s, it's 181-190. Since earlier it was around 160, I don't have FH, right, Doc? By the way, my HDL is always around 58-65. Thanks, Doc, for the lipid energy clarification.
People that hit an LDL of 180 or greater have an 80% chance of having FH or LMHR.

I got my annual wellness checkup Thursday, Lab test results Friday. According to my lipid panel I probably won't make it another week, but I feel pretty darn good. I'm expecting the doctor's office to call Monday and tell me to get on down to the pharmacy right now for you know what...... Follow the money? Nooooo follow the blood work.
At this time we do not have the technology to measure the cholesterol in the blood, chemically or physically by using light, color or transparency. Based on agreed knowledge of the last few centuries, there are NO minerals, crystals (like sugar and salt), fat bobles/balls (like cholesterol), oxygen (as gas) or CO2 (as a gas) in blood plasma.
There are NO Clots, it is only cartoons and photoshops and criminal fake doctors. Fake doctors in Providence Saint John told me that I had a clot in my RCA and they placed a metallic stent which dissolved and disappeared in my blood, later I learned that it was all fake and was about charging thousands of dollars.

I stay away from the simple carbs but also the saturated fat. I eat mostly plants especially rice and potatoes. Eliminating the fat and unclogging my receptors has my insulin resistance reversed and under control. A1c 5.3 and more importantly my insulin is 4. No junk, complex carbs only!

I looked it up. I see conflicting information. Some says rolled oats cook quickly. Another says it doesn't. That is critical to me. If it cooks quickly, it seems the body could get the sugar out quickly. What are your thoughts??
my doctors surgery has been phoning me and pushing me to take statins for the past couple of years. That's without any kind of testing at all, or even seeing a doctor. I've declined so far, because he hasn't given me a good reason other than age. I've just turned 60.

It was good until minute 39, neither oatmeal is good for insulin resistance
Just had bloodwork done at Cleveland Heart Lab and my Insulin Resistance Score is 4. They list optimal as below 44. So I definately do not have insulin resistance. I avoid eating as much as I can any sugar (no baked goods) and that includes high fructose fruit (eat mostly blueberries and strawberries). Having said that I eat oatmeal every day with flax seed, chia seed, hemp seed, a teaspoon of brazil nuts (selenium), pumpkin puree, bit of walnuts, cacao powder, some organic and greens plant protein, some almond milk and rest water sprinkled with cinammon. Oatmeal is not bad. Depends on how much is your total sugar intake.

Hi Dr. Brewer, I found for me that it worked lower my LDL having 1 tbsp of oat bran and 1 tbsp of psyllium husk in 200ml of Kefir. Plus I took Red Yeast Rice supplement twice a day. In 40 days my LDL dropped down from 275 to 97 , triglycerides from 51 to 35 , serum cholesterol/HDL ratio from 5.1 to 2.3 .
Red yeast rice is lovastatin. It’s Mevacor. That’s why it works. Same side effects as prescription statins. Doesn’t matter if it’s found in nature. Be careful out there. When you take it, you don’t know the dose.
my triglycerides are 32! my ldl is 10.8! crestor and ezetimibe for 1 year and i feel great

What causes high blood pressure 2hat to do to keep it at your recomendations
Excessive high-density and low-density cholesterol will not cause cardiovascular diseases or stroke, but oxidative foods with excessive cholesterol especially low-density cholesterol will. For example, storing a lot of gasoline in the house will not burn the house down but sparks will, please connecting the dots. If you have high cholesterol try to avoid oxidative foods like high-temperature fried foods.
As an empowerment health researcher, our body are not made to consume excessive cholesterol because the body can generate them according to what we need. I hope my research can save lives.

So how do you get off statins? I was 'borderline' but couldn't modify, so they put me on low dose, I stopped and numbers were back to borderline, and they told me the 'risks.'...so I went back on, low dose. I think I'm all clogged up... Can you repair arteries if you had high bp (not high, but elevated a little), then managed with meds. 81mg aspirin therapy? I would like to see cgm video. I don't need one, but I got a new one from someone who upgraded to a newer model and wanted to try for data.
have you discussed the 2015 study published in the journal Cell (PMID: 26590418), that found non diabetic participants responded differently to high glycemic foods? while a cookie would spike most people's blood sugar levels, eating a cookie does not in some people, in part because of their particular microbiome

Probably missed the last part of the video, we are not saying that is necessary healthy, however check lean mass hyper-responders, there are those who might have a situation where LDL doesn't associate with plaque, we encourage people to find their own individual risk factors, again LDL can be a problem, but most people, more specifically 88% of americans are not metabolically healthy

almost carnivore for last four years. Now have a bad hip everything seems to hurt particularly my back so exercise has become difficult. My weight is going up what is the answer in my case re exercise. Even breathing sends excruciating pains through my back often.

Would it be fair to say that HDL-ApoB as factors/markers (not cause) is much more important as get carbs get higher in the diet? So, if a person is "not willing" (similar to an alcoholic not willing to stop drinking) to modify their lifestyle & carb diet (hopefully most folks are not so stubborn) then taking statins and managing HDL ratio and ApoB are the thing to do, let say as a last stand fight? When it comes to one's life, depending on a "last stand" fight against stroke and heart attack is foolish and will eventually fail leading a defeat.
