    I had the sleeve in 2016, I went from 285 - 220. Over time the weight came back. I finally made the decision to seek help from a new surgeon ( I moved stated through the years) & I have my revision scheduled May 16, 2022 to the bypass. I started at 300, currently I am 273 & on a pre op liquid diet for two weeks. My surgeon said I should be able to make it to 200 lbs, but now after seeing this Im not sure. Please can I have your honest medical opinion? I do not want to do the revision for that small amount of weight. I’ve been doing really well exercising daily and eating properly and am hoping I can do this on my own

      Same like me I have my sleeve in 27 November 2017 before surgery I am 247 in 2 years hard work 😓 I lost 67 pounds I gain back 20 pounds now I am looking for some help too my surgeon call me back in two months I don't understand what to do and husband loss more then 100 pounds with bypass in one year he is going great 👍.

    Do you feel this video requires updating? I'd be interested to know what your thoughts are of Gastric Bypass revisions where the stoma size is reduced and the pouched is plicated endoscopically.

      No, endoscopic plication has been around for a while, it's not effective.

    Really only 10-25 pounds of weight loss for sleeve to RNY conversion? I’m scheduled for the conversion in July, and my surgeon told me that I could expect results as if I were having a procedure for the first time again - 60-70% of my excess weight.

      @@victoriacobarrubio5612 I’m scheduled for this July! :) I mentioned this video to my doctor and he said it’s not true at all, and expects me to lose the same way I did with the sleeve, so another 150+ pounds.

    I had rny 13 years ago & have gained it all back.. I feel so hopeless right now.. I failed & that’s something I think about all the time
    Now I have to get iron infusions and give myself b12 injections
    I have so many stupid health problems & now exercising is a lot harder
    I also eat like crap now, I’ve given up on ever losing weight and being healthy
    Will definitely find your book

    Hi, I had the lap-band in 2009. Sadly I didn’t loose any real weight, just 10-15lbs. I’m currently at 315lbs and thinking about a revision. I know that the RNY is the way to go based on your videos. I was originally going straight to VSG, because of its popularity. After stumbling across your content I see why the RNY will be more beneficial. My only concerns is the complications that can occur during surgery due to band removal and scar tissue. Are the complications from revision to RNY severe and life threatening? Thanks in advance.

    I am 12 yrs post op from gastric bypass what about the overstitch procedure. I am in the process of this revision but have to start the Baratric program over again with meeting with PH.D and dietician. Already meet with Dietician and had made significant changes in my eating habits. Hoping to get approved for the revision. Just had EGD after had upper GI both showing that my stomach pouch has definitely stretch out. There is few options for gastric bypass due to more complications and issues with malnutrition. So I am going the overstitch procedure.

    Just bought your book :) I'm not sure what to do and feel completely lost in 2008 I had gastric bypass, and went from 340 to 170. I was happy and felt amazing, now out of the blue I'm back to 280. I don't know what I did wrong and honestly the only thing that changed was going from a very active job to a sit down one. My reflux is horrible and can't swallow capsules. What kind of revision would I benefit from?

      +Allie Tayian revising bypasses is a tricky business. I recomend making an appointment with an experienced surgeon. Reflux is very uncommon after bypass surgery and should be evaluated.

    With vsg if the stomach is smaller how can you regain the weight

      The stomach is smaller so you eat less food at a time. However you are still able to graze on food all day and drink lots of calories.

    +Dr. Matthew Weiner.... I recently got a lap band to sleeve revision and developed acid reflux. If, in the future, I need a conversion to bypass, because of the reflux, what are the chances that the reflux will return? I've heard that sleeve to bypass resolves reflux immediately, but that, after a year, the reflux usually returns. Is that true?

      Band to sleeve often causes reflux which is one of the reasons I usually recommend revising bands to bypass. In my experience, converting sleeves to bypass resolves reflux permanently.

      true..I had RNY x 28 yrs ago. I had bile reflux x 13 yrs til I got a GI doc to do Endoscopy and take pix if it. Revision stopped it that day. Now, 8 yrs later, I do have acid reflux but it is in NO WAY as damaging or as frequent as the bile was. Im scheduled for a TIF procedure and an Apollo Overstitch but NOT going to do the TIF (my GERD us not bad enough for the many complications Im seeing post TIF. Also, TIF + Apollo on such a small pouch...my RN gut tells me NO WAY. I have only regained 40 lbs of the 212 I lost. I might not do the Apollo after watching your videos. Thanks.

    I was curious about revising from the old school stomach stapling surgery, vertical banded gastroplasty. Is it possible to switch that to a sleeve? My mom had that surgery in the 90's when I was a kid. It actually makes her unable to eat a lot of healthy foods without throwing up. Last year, when my sister had her VSG done my mom asked her surgeon if she could convert the old surgery to the sleeve and that doctor said he wouldn't do it, but would add a lap band on top of it, which she doesn't want. My sister has lost a lot of weight since her sleeve, and I'm having the sleeve done later this month, so my mom has been talking a lot about having a revision, but doesn't want to go all the way to RNY. By the way, your book is excellent and I plan on following the Pound of Cure plan after my surgery.

    Dr Weiner, I had a RNY in 2005 gained back about 70lbs in 16 years. I am scheduled for a revision on July 9th, surgeon is doing a Distal Bypass. Have you ever heard about this one? I'm currently 287lbs and want the Duodenal Switch which he thinks is a "nonsense" surgery. Thanks

      @@estherthompson1425Hi, unfortunately I was unable to have the Distal Bypass revision due to excessive scar tissue. The surgeon just was not skilled enough to get thru the scar tissue. I am currently seeing a 2nd surgeon opinion. The first surgeon tightened my stoma with 2 APC procedures which restricts my eating so I went from 287lbs now down to 248lbs

      Sorry you couldn't have the revision. Glad to hear your surgeon was able to help with restriction. Sounds like you're rocking it! My surgeon does a lot of revision surgeries. She's in Dallas, TX. Charlotte Hodges at New You Bariatrics if you want to look. She has a TH-cam channel too.

I hear Texas has some very talented and skilled surgeons, I have a friend that had excessive scar tissue and her surgeon got thru it with ease. I just chose the wrong surgeon who sadly to say is only after the money and not for the patient's best interest.

    I am looking for revision from sleeve to bypass solely for GERD. I've lost 170 pounds with sleeve from 315 to 145 and am in the high end of normal BMI for my height which my surgeon is happy about considering where I started. I don't really want to lose much more because of how much loose skin I have so am glad to see that there isn't a large amount of weight loss expected from sleeve to bypass revision. What's interesting though is almost 6 years after you made this video that revision groups are still full of people wanting to revise from sleeve to bypass for weight gain or not losing enough. I don't really want to have surgery at all right now. I love my sleeve but the radiologist told me this week after a barium swallow test it's probably my only option to resolve the issues with my esophagus and GERD

    I had the VSG done 3yrs ago, my acid r. is so bad my dr strongly suggested the revision. However that to me is too drastic that is why I chose VSG instead. However I have been told, the damage to the esophagus can be very bad that you can develop cancerous cells. what is your experience with that? thank you

      +Robin Marshall There are not a lot of options for revision after RNY gastric bypass. In the past, I've placed lap bands over the gastric pouch and lengthened the limbs on the bypass, but have abandoned both techniques because they don't seem to work that well. Aggressive dietary modification, possibly with the addition of weight loss medications is probably your best bet. See an experienced Bariatric Surgeon in your area, but do so with your eyes wide open - there are no simple answers here.

      Depending on where you started, this may be an expected result. This is largely the result of your genetics.

    When u convert from band to sleeve because of having a prior surgery and the body is used to the new life style and new pouch is the weight loss slower with converting to the sleeve ?

      +jadoredior04 What I've found is that you will lose weight according to your weight before your band surgery. If you've lost 50 pounds from the band, this will be 50 pounds that you won't lose after your sleeve. If you're back at your pre-band weight, you will lose weight like any other sleeve patient. This makes sense when you think about weight loss after surgery as being driven by your set point, not restriction and malabsorption.;

    Just bought your book. 😊 I have watched your videos and they give me hope...
    I am six weeks post sleeve surgery. I have been nauseated every day from when I get up until I go to bed. I went back into the hospital to get my esophagus stretched but nothing changed. It's been so hard to get in enough protein although I've been drinking skin milk and eating beans which go down okay. Also can eat blueberries and bananas which don't help with protein...... I usually chew on ice when water is upsetting....
    Any suggestions or medications that might help. Thank you so much.

    Doctor, how safe is a roux n y revision to a roux n y? I had the surgery five years ago. went from 369 to 240. the scale won't budge. I know I stretched out my pouch. if I go through with it, will I lose the remaining weight?

      Not sure exactly what they're planning, but trimming the pouch or lengthening the roux limb rarely causes more weight loss

    Hello Dr i had gastric bypass since 2009 and i been having bad gas and it really painful and use last 2 weeks and i was down to 162 how i gain weight now I'm 185 pound I'm really thinking about about having surgery again. What do you think

      No sensible revision options for gastric bypass. Most gas is caused by bad food choices. Give yourself a one month trial on the Metabolic Reset Diet in my book and see what happens to your weight and your gas - I bet you'll be pleasantly surprised.

      ThisYEARiWill Very few people in the US offer the mini gastric bypass, because many patients have problems afterward. As i mentioned in my video, conversion of a sleeve to a gastric bypass does not offer much weight loss.

    Hello Doctor, I started off at 650 pounds.. Had a sleeve.. Down to 430.. Now they said to get me around 250 I would need the RNY.. Do you think the revision will get the remaining weight off me?

      +Jordan L probably not that much. Usually a sleeve is revised to a duodenal switch as a staged procedure, not a rygb. There's also an exciting new procedure called the SADI that will likely be performed more and more in the coming years. I'd be surprised if revision to rygb got you more than 50-100 lbs

      what if someone needs the revision due to severe acid reflux? is there possiblilty of weightloss as well being that the pouch would be much smaller than a sleeve?

    It seems like all the WLS return back to setpoint weight after 5 years. I am 6 years non functional lapband/severe GERD but unable to do a revision due to no health insurance and just overweight now. I never felt this lapband was totally functional for "hormonal" long term WLS results. Revisions up to 3 WLS in many people so .........there is more to this puzzle.

    So happy I watched this video... was trying to figure out how I could afford self-pay for revision but for only 15-20 pounds lose it is not worth the struggle and money

    Hi Dr Matthew..
    I am wondering.. I had a lap band in 2011... convert to sleeve in 2015 and now looking at bypass. I have been told my Sleeve is Fast Emptying which is causing insufficient weight loss, Will Bypass help with Fast emptying sleeve and possibly result in Further weight loss??

      All sleeves are fast emptying - that's one of the mechanisms that causes the weight loss. Revision from sleeve to bypass is a tricky decision, I'm available for skype consultations if you think that would be helpful. You can call my office: 248-413-2670 for more info

    Can u tell me why when got the gastric bypass after 3 months later I got hypotension and with ih 2 year and 9month i developed hypoglycemia and how can I treat hypoglycemia it keep dropping it feel ugly ...😢😢😢😢
