5 Reasons Gastric Sleeve Might Fail

  • The sleeve gastrectomy is the most popular bariatric surgery in the world today. In the majority of cases, this type of surgical weight loss will provide the patient with the greatest probability of sustained weight loss.
    So why doesn't it work for some people?
    As a bariatric surgeon, I have identified five main reasons why the vertical sleeve gastrectomy (VSG) may not achieve the weight loss volume anticipated.
    Let's address each of them, so you can be sure they don't happen during your bariatric journey:
    1. Lack of Understanding How the Gastric Sleeve Works
    Your new stomach is reduced by about 75%; you will eat 2- 4 oz. of food soon after surgery and only about 4-6 oz. later on. If you overeat, complications will ensue. The gastric sleeve weight loss surgery also pumps up your metabolism into overdrive so you burn more calories, especially during the first 6 months after surgery. If you don't follow the post-op protocol during this crucial time, overall weight loss may be diminished.
    2. Resuming an Unhealthy Diet After Surgery
    Some patients go back to eating poor quality food after surgical weight loss, just in smaller quantities. You can still gain weight eating smaller amounts of fast food, sugar, processed foods, etc., even after weight loss surgery. It's critical to partner with your bariatric dietitian to create an improved food plan that gives you energy, helps you lose weight quickly, and provides the nutrition your body needs to function optimally.
    3. Lack of Exercise
    You want to burn off the calories you are eating, even if those numbers are far below what you formerly consumed. Working-out is also a metabolism-booster to speed your calorie burn. Exercise can also help keep your skin tighter as you lose excess fat. If you ignore calorie-burning exercise, it will slow weight loss speed.
    4. Medications
    Many people take prescription medications daily and are not aware they can be 'medication positive,' meaning weight gain is a possible side effect. Beta blockers, steroids and antidepressants are examples of medications which can derail weight loss after the gastric sleeve.
    5. Technical reasons
    There is some variation about the amount of stomach removed during bariatric surgery. Surgeons who leave too much, which can be more common when a previous weight loss surgery has been performed, may not remove the part of the stomach which contains ghrelin, the hormone that produces hunger. Be sure to speak with your surgeon about the volume of stomach that will be removed and how it will affect the ghrelin in your body.
    I've decided my career to helping people overcome obesity and regain their health. The sleeve gastrectomy is a wonderful solution for very overweight people who have been unsuccessful losing weight with other diets and exercise programs.
    But the bariatric patient still has to do the work and be compliant:
    Follow your bariatric surgeon's guidelines, eat the food, beverages and water volume prescribed by your bariatric dietitian. Find an exercise you enjoy and do it regularly. Reach out for multiple sources of weight loss support, such as a live or online weight loss support group, where you can share experiences with others engaged in the same journey.
    Your weight loss surgery is not just an operation. Be sure the bariatric surgery practice you choose offers a full range of support services to help you in all aspects of your life-changing journey.

    I had my Sleeve May 21,2015..I lost a 166 Pounds.. i am post op 6 years. I have kept all my weight off.I am age 63..I am 5"2..I had my vsg surgery done UF Shands in Gainesville Florida hy A Dr.Georgio Rooiddis. I have done great with my sleeve.. my healths Improved a lots.. i am off two Insulins,3 Blood pressure meds,, a cpap machine for sleep Apnea.. i still take Armour Thyroid 60 mgs once a day for my Hypothyroidism. The Sleeve saved my Life!!

    Thank you for being honest in this video. I am in the medical fiels so I understand everything & still did my research b4 my gastric sleeve. I attended many meetings to listen to other patients who already had sleeves done & how they were doing. I felt very prepared of what to do and what not to do. I was 5'3" & 215 lbs. I got down to 170 via surgery alone and lost another 10 with hard work. I still could have lost more for an ideal result but it was like everything stopped. When you have the surgery done, the ghrelin definately goes away for about 2 years. But once you are fully healed, it comes back like it never ever left. No md's talk about this. And the other thing md's don't talk about is technical shortcomings. I would bet my life my surgeon was too conservative with my sleeve. It was to easy to go back to regular food. The full first year was a bit rough for me but you can really feel when you are fully healed. I told my surgeon I was unhappy. I regained all of my weight & she shrugged her shoulders and said "gastric sleeve doesn't work for everyone. You need a bypass." Like hell I will. Of course she blamed me and said your stomach probably stretched. Huh? Why was it too easy for me to eat food again? Can a revision be done? Or I feel like Wycovia is the way to go for me at this point at age 67. I am so very disappointed. What a hugh disappointment this experience was.

    Thank you for your video. I had the sleeve 2/10 and it's doing it's thing and I am doing my thing to help it work the best for me. I also saw a video where the dr de-emphasized exercise but it was more to not work out too soon and too vigourously at first because the calorie intake is so small at the beginning that there's the risk of not taking in enough energy. I started working out about two weeks after surgery and I started with just a few minutes and slowly raising my workout time. I think teaching people how and why the sleeve may fail is essential for the reason you stated: knowing that it's not a magic pill and knowing how we contribute to the success (or failure) of it, will help us plan what we should be doing.

    You are very intelligent and very informative. No BS from you. My biggest problem with the sleeve is they take to much of it out and not enough to get enough nutrients into the whats left of the stomach for proper nutrition. Also Soda give up the soda first.       I never drink it. I had a Dr. Pepper the other day. 1st in well over 4 months or more. Still fat. Gave up bread and butter.

    Thank you for this very useful, kind, and informative information. All 5 are valid points which for success reasons must be considered. I was just sleeved May 25th. Although I am still experiencing the pains of surgery, I am pushing myself to walk as much and often as my body will allow. So much to RETEACH the mind and body in respect to food. I’ve got benefits of 6months to get it done. 💙

      @@TheNucMed Let me start by saying, Eric I am humbly sorry for your unfortunate experience. The thought really breaks a part of my heart. At the same time, I am grateful God has blessed me with much success and happiness after having the surgery.
      Now that I just had my 2 year anniversary of being sleeved, I WOULD NOT CHANGE A THING!!! To date my total weight loss is 147lbs… AWESOME! June 18, 2022 my father passed and that flipped my entire world upside down. I gained 25lbs from eating too much of the WRONG FOODS even in smaller portions and consuming entirely too much alcohol. But at the time it was what I felt I needed. A visit to my endocrinologist in February of this year reminded me of the WHY behind me having the surgery and all of the hard work I’ve put in since. I was my father’s primary caregiver. My mother is disabled. I know there was no way I could have cared for him and worked as a public school educator at 347lbs. I REFOCUSED and RECOMMITTED myself to living a healthier lifestyle by allowing my “TOOL” to do its part while I do mine. WITH ALL OF THAT BEING SAID, the sleeve has worked wonders for me physically and mentally. To date I still see my therapist and exercise 4 or 5 times a week. My benefits have allowed me to experience things I only dreamed of doing while I was overweight. I’ve learned how to swim. I’m taking horse riding lessons now, which is also great therapy. I’m going zip lining at the end of this month. I’ve been featured in a weight loss documentary for the hospital’s, where I had my surgery, weight loss clinic. As a “success” story, the same hospital is scheduled to film me today during my riding lesson and again when I go zip lining. I DO THINK ANYONE who has had the surgery does have a certain responsibility to share their story be it GOOD, BAD, or OTHERWISE. Each individual has the same responsibility to do their own research, pray, and make a decision with the understanding that their outcome may or may not mirror someone else’s outcome. Be well and my the Lord heal and restore your body.

    This was a very informative and great video. Knowledge that the doctors don’t tell u. I’m scheduled for surgery next month, I’m 50 pounds over weight and I’m seriously thinking of pulling out from the surgery. Help I don’t know if I should just diet or go for it and have the surgery.

      I had 60 lbs to lose, I did do the sleeve surgery, I’ve lost 35 lbs since August 20th, 2021. I’m not sorry I had it but you don’t lose as fast as someone that has a lot more to lose just an FYI. Good luck 🍀

    I think I was playing with fire before covid hit. I lost 100lbs from the sleeve but over time I ate "fun" foods again. The gyms closed and I gained roughly 25lbs. Its a terrible feeling.

      It's an addiction, like alcohol. And nutritionally. They're really similar sugars.
      Be careful you don't go into booze after you give up sugar. Many drunks can go either way. You might lose a finger if you put sugar in front of many drunks/addicts.
      Maybe look up Overeaters Anonymous?

      never start up with sugar/high carb food after the surgery... it's like an addiction... there is a reason there is so much sugar in ALL food these days = addiction... sugar is more addictive than cocaine...

      Happens when you get old. 50 ish. Menopause. Try Trader Joe's nutritional yeast. Should stop the falling out part, at least. I use as a sprinkle on vegetables.

    I am really scared i will get major reflux problems and have to go for a GBP after all. I really do not want that..So i hope i am one of those patients who never has a problem with reflux..Because i hear a lot of people getting that problem after a sleeve.

    I’m worried about the last point you made. I swapped from a band to a sleeve. So far, I’ve lost 20lbs in 25 days, but I’m worried because I can’t feel a great deal of restriction. I’ve been eating extra healthy, and I’ve just started exercising… but I’m really hoping that restriction kicks in soon, once I’m off liquid/purée foods.

    My sister had the sleeve done 5years ago and this year she gain excess amount of weight. She has acid reflux bad and they told her the only way to relieve that is the bypass. Since the bypass a month ago she has ended up in the hospital because of the hiatal hernia. Now today she is in the hospital again for the same thing, can't keep nothing down n have surgery again to repair the hernia. My question is if I'm having the same issues far as the hernia goes up under my rib cage n I suffer from reflux which doing better with medicine. Should I get the hernia taken care of if it's bothering me?

    I failed with sleeve to lose enough weight. I changed what I eat. eat healthy food and didn't stretch my sleeve but get hungry every 2 or 3 hrs. snacking every 3 hrs. those damn calories add up!

    Hello doc after gastric sleeve surgery is there any pain around the area?

    Since having my gallbladder taken out that's all I experience is the dumping syndrome, I guess having the surgery won't stop it either

      Good question.. i am the same. Had my gall bladder removed 4 yrs ago.. im having my gastric sleeve next week.. having this surgery to minimise food intake and not eating a lot of fats and sugers because the sleeve wont let me, hopefully the will lessen or stop. Fingers crossed

    I’m scared to get it. Can the staples come loose and puncture anything else inside? Especially with vigorous exercise? I have too much fear to get it but had a consultation appointment about it last week 😓

    I'm a "Technical reasons" patient. I'm 11 months out and I've only lost 33 pounds. I'm so upset. They said my sleeve healed in an hourglass shape. It loads at the top and slowly piles to the bottom. Needless to say, my sleeve stretched from all the piling at the top and gathering at the bottom. Now, I'm basically eating the same amount as before the surgery. So, they are going to convert me to bypass. I declined.

    I had the surgery about 3 years ago now I'm gaining weight fast . I know some of the reasons but my question is when I get back on track is my stomach go to shrink back up or I have to get a revision surgery to tighten up the stomach again ?

      They won’t do a revision to tighten your stomach. They will do a gastric bypass though.

      Pouch reset is a gimmick according to my nutritionist. Do research. I dont know for sure. I think you would have to have a revision to have the smaller stomach again.

    I had my surgery over a year and a half ago and it was a fail. The surgeon wouldn’t let me drink water for the first 2 months and that should have been my red flag, but then during my 3rd month I got really sick to the point where I had no fluid/food intake for about 10 days and he wouldn’t let me get IVs (ended up getting them though because I spoke to other surgeons who thought my surgeon was insane).. during the end of the 4th month, I somehow went from not being able to eat or drink to having an appetite and eating (had covid at the time). And it all went downhill from there… would a CT gastroscopy show me if the fundus wasn’t completely cut off?

    I would like to know if the dilema of choosing Gastric Sleeve or Gastric Bypass procedure it's normal to come down to the pacient or the surgeon? Can the pacient choose? In my case 185 KG at 175 cm height my surgeon told me a Bypass would be better but I read a lot about it and I feel a Bypass would be something I never could agree since I am 35 and right now I am pretty decent with my health, if you factor in my age and weight. I don't want to offend him and act like I am so smart but I just wonder how should it be: my call or his call? It's a private clinic and I pay 100% for the procedure. Thanks a lot!

    Doctor, can you tell us how much of the stomach should be removed? I hear different amounts either 75% or 80%, what is the correct amount?

      There is not an exact measurement of the stomach removed, hence the range. What best calibrated is the amount of stomach left behind. This depends the size of the bougie and the size that should be used is in the 36 to 40Fr range. 40Fr is most commonly used and is roughly the size of a garden hose. Hope that helps

      @@collin669 This is not a prerequisite for surgery, however, losing some weight prior to surgery is a good indicator for losing significant weight after surgery. This is in keeping with the principle that the surgery is a powerful tool that still needs to be used with lifestyle change for maximum effect

    I’m wondering if breastfeeding is impacting my loss. My surgeon said it wouldn’t. But I’m… not losing. Since surgery almost 10 weeks ago I’ve lost less than 20 lbs. most of that was the first 4 weeks. Scale hasn’t moved in like 4-5 weeks. I’m working out daily, drinking about 90 oz of water, eating Whole Foods, not doing sugar or many carbs. Not on medications. I just feel so defeated. Prior to surgery I lost 40ish in just under a month and a half.

      Did you have gastric sleeve or bypass? Dr John pilcher on TH-cam said for some women gastric sleeve doesn't help. But he has never noticed this problem in men who have had gastric sleeve.

    many thinks that exercise means they need to run a marathon... they simply just needs to WALK... 2x30min a day is good and as they get more exercise they get more energy and often want to do more than that because now they can... they don't need to be in a boring gym with other sweaty grunting people

      They go all out because they're feeling lighter and like they could do everything,but, they need to keep in mind that they could be burning too much calories without realizing and as well nutrients and minerals too that they can't easily replace with their food intake,so you're right!. People should take it easy and do try to conserve their energy because they're gonna need it since they can't get all the needed calories like before..

    Hi I had gastric sleeve and it has been 5 months. I weighed roughly around 140 kg and now I weigh 117 kg. I'm not losing anything for a month now :(. I've started snacking on crisps and chocolates and I believe this is why I haven't lost the weight i was suppose to lose

      Stay focused. There are weight loss plateaus after the surgery. Give yourself a chance. Try cucumber s for chips and limit the chocolate. Just keep these out of the house.

    I had my surgery August 20,2021. I started out weighing 225, I’m down to 195 and for the last week only list 1 lb. Not sure what to do I walk 1&1/2 miles a day still drink 2protein shakes and cottage cheese or steamed vegetables . Anyone have any suggestions??

      I walk at least 5 daily. Hit 45-50 and you're effed...at least I am. I'm way too fat. A good 60 pounds too damn fat. I'm like 188 at 5'4. I know it's not where you were, but I still hate every centimeter of my body.
      Good news, i feel so gross, I don't have to worry about stupid people I might date. I've got 0 interest. So, bonus.

    Would a gastric sleeve be appropriate for someone who can’t exercise due to disability? I still want to lose weight but because I can’t move much (but still feel hungry like anyone else) it’s insanely difficult. I don’t even mind a prolonged recovery period due to this I just don’t want to be this overweight anymore…

      In general, yes its appropriate and I have done this in the past. I still try to get people as active as they can be with sitting exercise, resistance bands for instance. a consultation with your local bariatric surgeon will help with specifics

      if you can't move your legs but can use your arms, then try an arm pedeling bike (for home training) 2x30min a day should do wonders and can be done while watching telly or while enjoying the view in the garden or whatever options you have... just have something you can "watch" so you don't sit there staring at the timer... then it is over before you know it

    Dr Seun, hi I am hoping you can help me. I am a kidney transplant recipient for 7 years now. I never had a weight problem I have always been active. I was on dialysis for 3 years and during that time I gained 60 pounds because my activity level changed no more going to the gym I was too weak. My appetite didn't change so I gained the weight. I had a kidney transplant and gained and additional 30 pounds. I was at risk of losing the kidney because my bp started to go back up. I exercised, watched what I ate and couldn't lose the weight. Finally, I did the vsg surgery. I lost 60 pounds but I have not been able to lose the last 30pounds. I keep going back and forth with my weight. I do take 2.5mg of prednisone. I don't know what to do. Any advice? I keep going for appointments with the bariatric nurse but I get pills that end up affecting my immunosuppressant medications and do anything.

      @@valerieann2751 Lipo is no cure. A good doc will tell you to lose the weight FIRST.
      I had full body lipo with a top surgeon. Believe me. It doesn't do ANYTHING to make you lose weight or keep it off. Wish I had the 10k back, and that was a BARGAIN. I would have just invested it. Mr. Right didn't pop out of nowhere and nothing else changed. So...
      Full body lipo, boob lift and chin lipo. We're now right back where we started 15 years ago. So...not his fault. I'm just a disgusting food addict. I'll eat anything in excess.

    Hi there, great video. I am 42 years old, have been vegan for 6 years now. I’m a hairdresser so on my feet all day, play tennis once a week and go to the gym 3 x a week, legs, back and bis, chest and tris. I have gradually put in weight over the years and I’m at my biggest now. I have tried so much and nothing is working. Do you think that it’s a good option for me? Also if I’m eating a whole food plant based diet will I still possibly get dumping?

    My fiance just got this 3 weeks ago. His recovery is rough, as he only drinks soups, and yogurts. He tried very little bites of tuna fish or shredded chicken and he immediately gets sick and vomits. He calls it vapor lock.
    Is he going to experience this forever and hes stuck only eatting soups and yogurt for rest of his life?
    Prior to the surgery he always worked out, lifting and hikes every week. Hes very active. He only wanted to lose about 40lbs.
    How long will he have to continue with liquids ? Hes very very frustrated and at times depressed because he regrets getting it done since he's been on right track prior to surgery.
    Btw, he still gets hungry so the gremlins are still there!!!

      How is he doing now? For the first 3 weeks I was on purees. After that I added soft foods as tolerated. Took months before I could eat most things.

    Hello I have a question I had the Sleeve in 2017 and I have gain weight after having two different major surgeries since my Sleeve. So exercising is hard for me to do like I did after my Sleeve.     I am getting my surgery in 2 days but I was told that it works for PCOS. Is that true? I don’t eat a lot to begin with. I was told the surgery stabilizes your hormones.

Nobody ever replied to my question, but I’ve lost 92 pounds, but now I’m at a standstill and am being told it’s still due to the PCOS. It doesn’t take away your PCOS but it takes many of the symptoms that PCOS causes. I would ask your doctor.

Before the surgery I had abnormal cells every time I went to get my pap-smear which means I would have to go every six months instead of once a year, and since I’ve had the surgery I no longer have abnormal cells when I get the Pap smear. Thank goodness 😅

      I feel the same. I’m 10 days post op and I can already get 4 oz, though it takes me about 45 min. 😫 I’m afraid I won’t lose. I am now purposely eating 2 oz and drink 2 oz of water with hopes this will help me lose

    I had a gastric sleeve 4 weeks ago. Ive eaten hardly anything in 4 weeks under 600caloried a day but ive not lost anywhere near enough weight and im so worried its not working. I cant eat much, and am not hungry so i know its not stretched.

      It does happen, usually after losing the first 40 lbs. be sure to take your Biotin every day.

    Mine isn't going well I eat my three meals I eat about 6 oz of food sometimes less and 1 snack two hrs after my last meal and I'm just always so hungry I get really bad hunger pain.

      I get hungry too! Some ppl in the sleeve fb groups that I'm in swear it's only head hunger. They say even if our stomach growls, that it's the head hunger causing that. I'm like "no! I know what hunger feels like!" My hunger never went away. BUT, (idk how true this is) but I recently heard that whether we are hungry or thirsty that it sends the same signals and has the same feeling in our bodies(hard to explain, but pretty much that its easy to confuse hunger from thirst because it feels the same post op, so many times we can just be thirsty but it feels like we are hungry) again, idk how true that is, I probably didn't explain it too well, but yeah- either way, I still feel hunger too! Makes me feel better hearing that I'm not alone in that because some ppl in the fb groups make ppl that feel hunger feel wrong, like we are making it up and it's impossible to feel hunger! Maybe the part of our stomach that produces the hunger hormone (ghrelin? I forgot how to spell it) maybe that part wasn't taken out of us(?) 🤷‍♀️

    I follow Keto. I am 9 years post op. If you have high protein, low carbs, low calories and low fat. You can have chicken or turkey. That is crazy. Your brain needs fat. You need protein, you need fat.

      @Margie Perkins how far out are you from surgery? Have you lost since you've been at 220? (Maybe it was just a stall?) I'm almost 4 months out. So scared I'll fall at this. My main goal was at least to just be under 200lbs. (Even if my body stopped losing and left me at 199lbs, I just wanted to be under 200!) But I've seen ppl say that after 3,4,5 months they just abruptly stopped losing weight. Maybe thinking it was just a stall, but then turns out it wasn't a stall, their bodies just stopped losing. I'm so scared of that. (Which brings me to this video! That's why I'm watching this, to see what makes the sleeve fail) hopefully you got closer to your goal by now and you were just in a stall🤞

Maybe (hopefully) you were just going thru a stall when you commented, and thought you were done losing? I have heard ppl that did say their sleeve failed, some that hardly lost any weight, and/or their bodies stopped losing only 3,4,5 months post op😭 which I'm scared that'll happen to me. But hopefully your sleeve didn't fail and it was just a stall?

      @@Catherine-nv7wc hello. Still feel like it failed. Never felt the restriction others felt. Lost 40 lbs in 1 year. Something I’ve done with dieting alone in past . I’m trying my best to realize it’s about me changing my habits so starting to loose on my own now. With eating choices and exercise. Regret telling anyone about my surgery as when I do loose all my weight they will think it was surgery only but I know it was me changing and doing the hard work. I wish you much success. I know many that have had it and it’s been successful. I’m thankful I never felt sick and didn’t loose so much so quick that I looked like them which is sickly at times. And since I’ve lost slow my skin doesn’t say. Definitely a learning process. Wish you much success . Don’t worry and Remember true surgery is on your stomach not your mind and emotions. So if you don’t change those and that’s were you ate from, then it will come back.

      @Margie Perkins did you change your eating permanently and did you change your activity level do you drink a lot of water how long ago did you have your surgery I was 218 when I started. I went down to 128 through diet exercise and lifestyle changes and a lot of water not just the surgery. I did go back up to 160 100% my own fault but I'm now back down to 136 and am determined to get to 125. I am 60 years old

    I'm getting worried that mine is failing. I'm 3 months out and I've love 46 pounds, but I haven't lost anything in about 4 weeks. I've been eating well and I exercise 4-6 times a week. I've been walking about 6 miles a week and doing mild weight training. I'm worried that I'm not going to lose anymore.

      I got mine in august 26 2019 it’s been a little almost 7 months around there,and I haven’t been losing weight. I started at 221 and I’m at 190lbs not losing weight anymore it just stopped them pounds I lost in the beginning. After that no more weight loss. I know a lady she lost already 150lbs she’s 6 months out and still losing on a daily. Just don’t understand why I’m not losing.

    i just want it to stop, i was 170lbs and now 115lbs, lost so much that I’m forcing myself to eat to regain at least 20.

      I would kill to get to 170. I was 358 on surgery day may 25 2023 got down to 313 on labor day and back to 325. Epic fail.

    Very informative. I am close to someone that underwent this surgery and I was horrified and saddened about the decision. I think it needs to b a last resort cuz of side effects and radical lifestyle change and all the vomiting and dumping. That cd nvr b a positive correction to me. Thats depressing! I think what morbidly obese ppl
    Need to try first before surgery is intense therapy, nutritional counseling
    And get rid of the enablers. Surround urself with people that support you not sabatage u! Stop eating fake food and candy and cake and limit bread. Believe in yourself and meditate or pray for strength and will
