To answer your question : In America, the date is formally written in form of "mm/dd/yyyy". But if you want to avoid any confusion you can mention like "Jan 1st , 2011". The same way, I have mentioned in both Annexure E Form and Affidavit for change of appearance and signature form.

@Aansi does ups require a witness for notarization? also the new and the old passport needs to be manually picked up from the consulate?

If your Aadhar and driving license do not have the new address, you may want to consider using any other one document that do have your name and current address, such as
-Recent Bank Statement from Government bank (Should
have affixed photograph)
-Utility bill not older than 3 months
-Voter Card ID
-Copy of Ration card which has the parents name - Should have names of applicant and
parents

Copy of Photo passbook of Public Sector Bank
Copy of Parents Passport copy as their name is on the applicant passport
or

When they say that, they mean "FedEx pre-paid labels are not acceptable to return your documents". It's for the return of your documents. You are good to post your documents through FED-EX since they generated it.

Yes you have to submit one new Indian address proof, you can submit any one document which is given in Adult checklist under “supporting documents section” like Adhar card/driving license with your correct current address Indian address details

Thanks for this video. I have my appointment in 2 days, but suddenly see that the checklist is different from the mandatory list in the website. change of appearance and signature form does not say it has to be notarised. For adults there seemed to be only the visa status for notarisation. Hope someone who has submitted recently can plz clarify which checklist I should now see.
