Give the perception that you are not from Africa if that is a road block. Don't mention it in your outreach, make sure that you look professional on video with a good webcam + background that doesn't give any indication that you are outside of the USA and it will probably come up less than you think. If it does come up, get a mailing address in the USA and say that you have offices in the USA + a team in Nigeria to keep costs down.

Definitely, keep it simple. Name, title, company website link - make it look clean. logos help high volume cold email land in spam, so leave those out of your cold email
Depends, medical niche is a very broad category. Some will be terrible - like specialists that are referral based (cardiac, brain, etc. specialists), but others like chiropractors etc. are proven to be consistenly good niches to work in
When you mentioned even offering services for free, I hid my face behind my palms, lol. But almost everything you've said here resonates with me. I definitely hesitate to contact potential clients again because I often worry that I may be bothering them. In your opinion, do you think Facebook is suitable for reaching out, or should I stick to Instagram when targeting those in the real estate field?

500k- 1 mil USD in revenue minimum is a good place to start
