I hope you’re able to see this. I have a 28 day old newborn. He’s EBF since birth. When my milk ‘came in’ I pumped off 7 ounces for pain relief. Since then I only chest fed and would pump upon waking. That morning pump averaged about 4 ounces. I alternate breasts with feeds(I’ll feed him from left this feed then right the next feed time). He falls asleep after about 5 minutes but won’t eat anymore even when Im successful in awakening him (he’s full). My concern is he’s extremely gassy. I give him gas drops a few times a day, brought him to ER due to his severe belly pain. His KUB xray showed gas and dialated bowels. Physician mentioned possibly a virus or just a gassy baby but this has now been 2 weeks since onset and no improvement on gas. He has explosive gas/poop, it’s yellow, increased spit up(not projectile), he gained 2.5lbs in his first 3 weeks of life. I’m starting to think we may have a foremilk issue. Any suggestions?
It sounds like you have a Forceful letdown/over supply. Most moms have this, a little bit in the first couple of weeks postpartum but if it seems to continue after a few weeks, then mom probably has the condition “forceful letdown over supply”. Here is a link for my three part video playlist on FL/OS. The first video will just talk about the signs. This will help you to determine if this may be your situation. Keep in mind that you and baby, probably won’t have all of the symptoms but if you have many of them, then you probably have this condition. The second video will walk you through how to get it under control. Having this condition is something that always needs to be managed. It can get out of control very easily. Normal breastfeeding guidelines don’t apply to moms with a FL/OS. An example would be that most moms need to feed from both breasts each feeding in the first few weeks but when a mom has this condition, she needs to “Block Feed”. The way that you described how you are feeding baby is block feeding. You are doing an amazing job mommy! You are on the right track! Hi Mary, I found your videos to be super insightful! I’m having issues with my 4 month old who has been constipated since turning 2 months (he has gone 14 days without pooping at one point). I’ve also noticed that I have a lot of foremilk in one breast than the other. Could this imbalance be the cause behind his constipation? How can I fix this imbalance?
@@ann3614 If a baby was constipated, their poop would be firm and pellet like. It’s rare for an exclusively breastfed baby to become constipated. Breastfed babies who are older than one month old, may skip pooping for several days. Sometimes up to 2 weeks. This is because breastmilk is more completely absorbed and utilized. There is less waste. If a breastfed baby isn’t pooping and seems to feel fine, their body is still using those nutrients. Making baby poop when they’re not constipated, is taking nutrients away from baby. Babies who skip poops for longer stretches, usually continue to do this. If a baby is ever receiving anything other than breastmilk (like formula or solids), they should be pooping every day. Definitely talk to your doctor if you’re concerned but it’s normal for some breastfed babies to skip poops *All babies should have 6 or more wet/pee diapers every day. ❤️
Very informative 🙏🏼 How much fore milk does it usually take before baby gets an upset stomach? If he gets to much fore milk in one feeding, could that cause stomach issues? (Sometimes, especially in the mornings, my breast seem really full/hard, so I have been wondering if he gets to much fore milk then)
It would need to be a large amount of fore milk on a regular basis, like throughout the day, every day to bother baby. It’s very normal for our milk to be higher in fore milk in the morning. It still has plenty of fat in it, there’s just slightly more fat in your evening milk. (And more lactose in the morning but less lactose in the evening ) Our bodies were designed to make the larger amount of milk in the morning. This is like baby’s main meal in the day time, then in the evening it’s more like baby’s dessert. Unless you have a forceful let down/oversupply, there’s no need to worry about it.❤️
Thank so much for sharing this. I have a 6 week baby and I would use haakaa (No suction is used )to catch leaks from the other breast when I am feeding. In the first few weeks I would have about 3-4 oz in haakaa but now I guess as my milk supply regulates I get nothing or less like 1 oz sometimes. I would still save it and I wonder if I collect enough from multiple days and give my baby later, would it be too much foremilk from that collection ? since its all from the let-down reflex? The milk at the end of a feeding is slightly higher but either way, there’s not a huge difference in fat from foremilk to hind milk. The only moms who need to watch foremilk/hind milk, are moms with a forceful letdown/over supply. It sounds like you’re doing great! ❤️
I believe I have a foremilk/hindmilk imbalance. I also have a fast letdown . My 6 week old has really bad tummy issues , gas, spitting up , reflux. She screams in pain! She doesn’t stay long at the breast, she pulls away and throws her head back in frustration. So how can I fix this imbalance , I’m totally lost on this 😢 pls help

Hi! That’s good that your doctor said that you should be able to breastfeed. Over the last 25 years or so surgeons have performed augmentation, keeping in mind that a mom may want to breastfeed. I would find a local Lactation Counselor. Call your hospital and ask specifically for an IBCLC. They will do a breast exam and give you more advice. Be sure to ask if you can get in to see her right away after the baby is born also. ❤️
Hi Mary, I’m feeding my baby from my breast and by pumping my milk into a bottle throughout the day. Is it alright to keep switching like that? Also, my baby is almost 8 weeks and mostly has frothy poop that is mixed with green at times. She is fussy it’s hard for her to pass gas :( I have noticed my milk start dripping randomly throughout the day. I understand that when I feed her from my breast I should let her drink from one side until she is full. However, how can I do that when I bottle feed? Do I keep pumping only from one breast until it is empty? Also, can I pump from both breasts into one bottle?
Hi mommy! You can pump both breasts at the same time and mix the milk into one bottle. Just when feeding from the breast, it’s necessary to nurse as long as possible on the 1st side before switching. She has explosive diapers, has gas but is not able to release it unless it’s while pooping. She mostly poops when I hold her in a ‘sitting on the toilet’ position. She mostly has frothy looking poop which is mostly yellow but at times green and yellow. Mary, do you think this problem would get fixed if I just always bottle feed her?
She also cries a lot in the evening especially. She takes my breast when I try to feed her but then starts squeezing her knees into her tummy and moves around and starts crying. But then wants to continue drinking milk again and the same thing repeats. Does that mean she is still hungry? I’m a first time mom and I feel so overwhelmed when she cries 🥺😔 please help me
@@hadaislam You are doing an amazing job momma! Work on getting your supply under control but go slowly. It will help her a bit if you go to exclusively pumping but only if you also lower supply. If you want you can exclusively pump or just partially feed from the breast. You might want to pump in the morning and chest feed in the evenings when supply is naturally lower. Your supply will also adjust more as time goes on. At about 12 weeks, most moms see an improvement. Having this condition can be more work than having a low supply. It’s something that you’ll always have to keep an eye on. He will have this condition with every baby, but every baby will handle it differently. Some babies have extreme problems and some barely show any difficulties. Just keep it in mind if you have more babies. you may need to come back and watch that second video every once in a while just to remind yourself how to keep it under control 🥰
Hi, I have a question. My baby would get a dark green diaper about once a day but today had 3? I have been making sure to leave her on the 1st breast like 30 minutes until she comes off and now she had more today. I still take an iron supplement from pregnancy and eat alot of spinach and beans? Also I started adding an extra pumping session worried my supply would go down since I am at week 8 on Sunday and was trying to be as plentiful as possible while I can? Thanks.
Do you know if you have a forceful letdown/oversupply? That could be the cause. Especially if the extra pumping is increasing the green poops. An occasional green diaper is normal. If there are a lot of them, it may be showing a problem. Vitamins and supplements taken by baby or mom can sometimes cause green poops. How many wet diapers is baby having every day? When you add extra pumping, how many ounces are you getting?

I was doing 1 to 2 pumping sessions after nursing a day and now went up to 3. I think for each one after nursing I got 4ounces, 3 and 2.5 ish ?

It sounds like you’re seeing much more than average. I would cut your extra pumping down (slowly so you don’t get a clogged duct). If it’s not causing baby lots of painful gas or causing low weight gain, you could continue to do some pumping if you want but it can increase problems for baby and increase your chances of clogged ducts. Keep in mind that when you have a FL/OS, you’ll have it with every baby. Some babies handle it better than others. If you have more babies, keep it in mind. Hi Thanks for a quick reply. He is 5 weeks old. I usually collects the maximum upto 40ml in the morning when I have good supply of milk. The rest of the day can be 20 ml or sometime even less.

If you like the Hakkaa then keep at it. I never cared for the Hokka, I personally preferred collection cups. I would wear them at nursing and throughout the day, in the first couple of months when I was leaking more. The Haakaa just seemed bulky and in the way. If it works for you that’s great!
