What is COPD? Clearly explained for patients and carers

  • What is chronic obstructive pulmonary disease (COPD)? In this video I explain it in a simple way. COPD is a very common lung condition. The main symptoms are excessive breathlessness on exertion and a productive cough (clearing sputum).
    COPD is a combination of two other conditions: emphysema and chronic bronchitis.
    Emphysema means that the lung tissue thins out and there is less surface area for the lungs to exchange oxygen (O2) and carbon dioxide (CO2).
    Chronic bronchitis means that there is frequent coughing with sputum production, usually on most days, for at least three consecutive months each year (at least 2 consecutive years). This is the standard definition.
    Both chronic bronchitis and emphysema coexist in people with COPD, but in different proportions. Therefore, not all people with COPD have the same symptoms. Some may have more breathlessness, whereas others may have more cough and sputum production. Some COPD patients also have more frequent flare-ups (exacerbations). There can also be an overlap between COPD and asthma.
    The main cause of COPD is smoking or inhalation of noxious fumes and dusts, either at home or in the workplace. Some people are genetically more predisposed to getting COPD, and will be more sensitive to the effects of inhaling noxious fumes or smoke.

    hi again Dr Stefan , I do so very much enjoy your videos you always explain everything in laymans terms so to speak { plain english} so that people like my self can understand you I my self have copd, I own it meaning I smoked and did this to my self I was diagnosed with copd about 5 years ago or so and I quit smoking immediately it was hard to quit but I did it, please keep your videos coming, you are doing a great service to all who want to know more about what you spcialize in lungs you are an awesome DR. keep videos coming please

    HI Dr Stephan. You're the only doctor talking about lung disease that I can relate to. Others are hard to figure out due to difficult accents etc. I got lung scarring at the beginning of the Pandemic in 2020 having been close to someone who had a fit of coughing.Bad flu,feverish for months later. CT scan indicated lung scarring. No treatment for 18 months due to confusion etc. Now diagnosed with IPF. Taking Esbriet. I believe I'm misdiagnosed. Arguments with doctors.Not good. They won't accect my details about the origins of my disease. Here's my question. Would appreciate it if you could answer it in some future video. Are steroids ineffective after all this time in my case ? Im very concerned as I believe a lot of time was wasted.Thanks in advance.

    Hello Dr. Stefan, I discovered I have Alpha1 Antitrypsin Deficiency affecting my lungs through a 23andme genome analysis. I have never smoked in my life but I’ve had the usual exposure to second hand smoke and air pollution, living and working as a California native born in the 1960s… I now have some emphysema in my lower lungs. It’s considered moderate by PFTs. But I don’t have many of the classic symptoms that accompany this condition…
    I do get replacement AAT infusions but the inhaled bronchodilators with combination medication cause me so many side effects, my quality of life has declined. I’ve been told that I need to use them to both maintain and to prevent worsening of my lung condition. I have found nebulizing reduced l-glutathione to be extremely helpful at eliminating the small amount of wheezing I do have as well as breaking up some mucus I didn’t even realize I had.
    Can you please address why my specialist insists I need to use the inhaled medications to keep my lungs from getting worse? I am also liver affected (F3-4) and I’m wondering if I can be extra sensitive to inhaled meds because of liver insufficiency… Thank you!

    Bună ziua!
    As vrea sa va adresez o întrebare medicala și sper sa nu va deranjez.
    În iulie 2022 am avut o intervenție pe inimă, în urma căreia s au înlocuit valvele aorta și mitrala, iar tricuspida a fost reconstruită.
    De atunci, ma confrunt cu o pleurezie recurenta, mai mult lichid este pe partea dreaptă și moderat pe stânga. A fost scos lichidul de 3 ori din partea dreaptă, aprox 2 litri de fiecare data, însă nivelul s a refăcut de fiecare data în câteva zile.
    Am încercat și o schemă de tratament cu dexametazona și mai mult diuretic cel mai mult 2 luni. Mi a fost un pic mai bine pe perioada tratamentului, însă rezultatul nu se menține.
    Precizez ca nu am edeme în alte zone ale corpului, cum ar fi picioare, mâini.
    Toate modificările care au necesitat aceste intervenții, vin din tratamentele cu chimioterapie și în special radioterapie pe care le am efectuat în anii 2000-2001. Atunci am fost diagnosticată cu limfom nonhodgkin și radioterapia s a aplicat și pe zona pieptului și la momentul respectiv era foarte invaziva adică ataca tot ce era în raza de acțiune, inclusiv plămâni, inimă.
    În ultima discuție pe care am avut o cu dr cardiolog, mi a spus ca nu mai are alte resurse și ca ce a mai rămas de făcut ar fi pleurodeza, însă nu știe pe termen lung care pot fi riscurile. Pentru ca de obicei se face în cazul persoanelor cu neoplasm pulmonar, iar speranță de viață fiind mică, aceștia nu sunt urmăriți pe o perioadă mai lungă de timp.
    Ce vreau sa va întreb este dacă ați auzit de cazuri similare și care este protocolul în Anglia? Și dacă pleurodeza este o opțiune validă pentru mine. Precizez ca am 38 de ani și am fost nevoita sa ma pensionez.
    Mai Precizez ca lichidul scos din pleura a fost analizat și nu este de natură canceroasa sau infecțioasă, e curat. Ca și consistenta este apos și gălbui. Seamănă cu urina.
    Ma scuzați pentru deranj, însă încerc sa iau informații și din afara țării, poate ca există o expertiză cu situații similare. În România nu este sau cel puțin n am găsit eu. Sau poate știți în România un medic care are experiență cu așa ceva.
    Va mulțumesc și în măsura în care va permite timpul, aștept un răspuns de la dumneavoastră.
