An Important tip to consider with underhand grasp is making sure the beaks are with the long axis of the tooth and so the rotation movements will be in horizontal plane on the tooth otherwise it wont luxate or even might fracture , the 11 o'clock position is needed for sure but it gives a direct upper view on the Forceps so you cant assess the horizontal plane of the grasp so I always bring my head from the left or even go to 1 o'clock position to correct the plane of the grip , I do have a mandibular cowhorn forceps in American style so I always use the underhand grasp with it but all my other forceps are English style which are difficult to use an under grasp with... one more thing I don't think we can use American style forceps with mesially tilted teeth since it cant be fully seated cuz it would be hitting on the anterior teeth . whats ur opinion doctor Jason with all whats mentioned ? thanks !

Underhand still kinda feels a li'l difficult. But I guess it will get better with practice.
I hate extracting teeth I always get stuck on some cases
