    An experimental drug from Eli Lilly helped patients with obesity lose an average of 24% of their body weight over 48 weeks on the highest dose in a mid-stage study, the most weight loss seen yet in a new class of drugs that’s revolutionizing the field.
    The medicine, called retatrutide, had similar side effects - mainly gastrointestinal in nature - as approved drugs in the class, including Ozempic, Wegovy and Mounjaro, researchers reported in a study, funded by Lilly, published Monday in the New England Journal of Medicine. The results were also presented at the American Diabetes Association conference in San Diego.

    I went through the Eli Lilly site, and got the referral to a clinic doing clinical trials. I got through today. And the trial is for Terzipetide. I tried to get it for the new retatrutude. I hope to get the approval soon. I thank this channel for this video because it made me try this approach since Kaiser won't approve it off label. Thanks doc.

      my mom did keto for 1 year as well, she lost 50 lbs but unfortunately keto is not a lifestyle- its a diet so she didn’t keep it off

    After all I learned about how drug companies bias their own research, control the academic journals, and have regulation capture of the regulatory bodies, I would recommend just eating less. COVID sure opened my eyes.

      Eating a calorie-restricted diet will slow the metabolism unfortunately. Foods are incredibly complex. I'd love to have a big vegie garden.

    This seems promising. It's sweet when you find what works and start losing that unwanted weight. I lost 42 pounds in less than 3 months and have kept it off since then. Key thing to note is that I exercised daily for months, but my issue was the struggle with my calorie intake. It was just impossible for me to eat less. It felt like I wasn't in control of my actions. Adding Phenq pills to my journey just made things easier. Eating less became possible and I also had enough energy to exercise. I talked about this extensively on my channel, though

    What happens when the body cannot produce glp-1, gip, and glucagon (just like trt reduces testoterone production over time) - and the question goes to all the products (ozempic, wegovy, mounjaro, retatrutide, …) - is there any way for the body to regain its ability to produce glp-1, gip, glucagon, testosterone, … (for any hormone replacement therapy - when the body have gotten used to whichever hormone is being injected, for months or years - can it regain its ability to produce it again, or is it game over?)

    A friend of mine lost 15 lbs on ozempic but ended up finding a lump on her neck that turned out to be thyroid cancer. Her doctor says its unlikely to be related but I'm not so convinced.

      @@narkybark Apparently in less than two months because the weight flies off with these drugs. It seems just as illogical to surmise that her weight gain was due to unknown thyroid cancer developing and was diagnosed after her weight loss. I am on Zepbound 7 weeks in and my "next size down" clothes are getting loose. 15lbs? Easy in 6 weeks with these medications.

    So sorry but the comments here about losing weight are just uninformed. It is not just a matter of how much you eat or what you eat. It is a very complex problem. 95% of all dieters fail. Are they just weak or have no self control? . No, it may start with poor eating habits yes, but it does progress to a disease that needs to be treated. Poor people and seniors very often cannot afford the higher quality foods. There is also a genetic factor to weight and body type. I truly wish people understood the problem more and were more considerate to those who have struggled for a lifetime.

      I was always really skinny my whole life until I was put on ssri’s then in 6 months I put on 60lbs. I stayed on it against my better judgement and gained another 40. I quit cold turkey over a year ago and only managed to lose 30 on my own. Now my face is swollen and I’m walking around with a pregnant belly and I’m a guy in my mid 30’s that’s never had a weight issue before.

      We only need Oprah to prove this. She has battled for decades and look at her resources! Private chef for the best nutritious foods, private trainer and gym, any and all mental health she would need and she yo-yo dieted the same as my poor self! Now she is thin, now she has quit working with WW, now she is finally where she has always wanted to be. Why? A shot. some people cut down on calories and work out and do AMAZING things for their health. And then there are the people like us, you know, me and Oprah 😃

    Thank you doctor it is not easy to make the right choice sometimes when you have kids. Also I am looking forward for a help to control my sugar and fatty liver disease.     It sounds like this is going to be a race to see who can produce the most weight loss drug. It sounds pretty confusing to me. When I was younger I took Phen Phen. It was taken off the market due to heart valve complications. I lost down to my goal weight with it. It made me feel “normal “ about food. I ate what I wanted, but didn’t over eat. I wasn’t always thinking about what I was going to eat next, but just like any weight loss without making life changes the weight returns. Very frustrating.

      I'm struggling with how to make myself a priority. Once I had my baby, ME was a non-existent idea. He was super reflux-y and so it was all about him fir the first few months. Making sure he wasn't aspirating, and getting enough food. And me trying to make milk for him that I couldn't...
      We need to figure out our way out of NOT paying attention and caring for ourselves as well as we care everyone else around us...

      @@lisacook9116How many carbs were you eating every day? “Cutting carbs” doesn’t mean anything specific. My sister cut carbs by going from 8 cookies a day to 5. Then she ate an extra big salad. She didn’t lose weight.

    Can you talk about whether reoccurrance of weight gain happens upon discontinuation of these medications? Even when following an appropriate diet?
    There is a reason the weight gain happened to begin with. For example, myself. I've been eating a 700-900 cal for over a uear now, and am stuck in the 250s, on a frame of 5'8"... If I were to take one of these medications for weightloss, and increased my intake to say 1500 cals/day. Would I regain?

      Wow, that’s so few calories. Your metabolism has adapt. You should check your hormones. I have similar problem and my hormones are off. I do eat more like 2200 calories per day but I want to lose a few pounds. My doctor is helping me adjust my hormones now and I am doing HIIT.

      @@ftmrivas3043 yep, I've just recently met with an endocrinologist, she is collecting all my information from my PCP. I also just met with a nutritionist, and she says with all the different diets I've tried in the last 5 years, my body just doesn't know what to expect so its clamping down and keeping whatever it does get. Esp with the low calorie intake. So, she told me quantities of food groups to eat. And I'll try that..., While waiting for my endocrinologist to get back to me. This weekend I will be food prepping again according to the ratios she gave me. Hoping I don't jist keep gaining.
      I need to figure out how to eat on a consistent basis...how to infuse it into my busy life. I had a little boy and I don't know how to make myself a priority, js my biggest problem.

      Homeschool him by day and housework
      Then I get ready to go again at night.
      And in between all that I am trying to learn how to use Adobe Illustrator and Procreate to start my own business, and doing all the social media posts, and built my own website. I have to do this, because this is my way OUT of the hospital.

    I am a type 1 diabetic and use Mounjaro to help control my glucose levels. It does a great job in reducing my use of insulin too. I've lost some weight, but not a high amount.

    I've read that private insurers in the U.S. will not pay for any of these drugs if they are used off-label and not for their intended use in the treatment of Diabetes-type 2.

      It depends on the insurance company. However you are right, most private insurers are starting to deny Ozempic if there is not a diagnosis of T2D. Wegovy and Saxenda are FDA approved for weight loss but some insurance companies have them as plan exclusions.

    Glp-1 stimulates trpv1 and while trpv1 is an incretin, the insulin then up-regulates trpv1 receptors that put glucose into the cell independent of insulin and the insulin receptor. This creates insulin resistance and you will see this resistance after you stop the drug and do a combination 5 hour glucose tolerance test/c-peptide, insulin curves. Additionally you will find that while TRPV1 is force feeding the cells, hyperinsulinemia is taking place from over stimulation of trpv1 with the risk of beta cell dysfunction and loss. With the hyperinsulinemia and force feeding the cells glucose the cell’s antioxidant reserves run out and the mitochondria are damaged and too much reactive oxygen species are produced leading to atherosclerosis and other inflammatory diseases.
    Try eating the low carb diet instead.

    All these quick fix drugs…….I am waiting for the long term studies. Intestinal blockages (impactions) which can go undetected for years is now becoming a common cause of death in those that have had weight loss surgery (shortening the intestines).

      I'm on Zepbound, I have issues with calorie restrictions because my brain screams for too much food. 7 weeks in and I have to remind myself to eat. It isn't appetite loss for me, it is a lack of the overwhelming, loud hunger that I've always had. Eating healthy is SO easy now. I feel like other people who can push a plate away now.

    Lost 115 pounds via diet in 2020. Still need to lose 35 but I'm hitting a wall. Have t2d treated my metformin only. I eat 1200 or fewer calories per day (OMAD.) Was on Ozempic for 8 months after losing 115 pounds. Could not tolerate the pancreatitis. What's worth a try of all the drugs in pipeline?

      From what I've read, as T2d you should be able to qualify for Mounjaro. If you have a history of pancreatitis, you should probably discontinue if you feel pain in your stomach. Definitely work with a doctor about it.

      Yes you can :) I have only one working kidney and my nephrologist said it was fine when I was prescribed Ozempic:)

    I do understand that being obese is a national epidemic, but I am concerned about the mentality behind just popping a pill (a very new one at that) to lose the weight and thereafter being tied to it for the rest of one's life. A friend of mine started losing weight at the beginning of this year through diet (eating one meal a day) and exercise. The last 30 days he has fasted, and in total has lost a total of 96 pounds (32 of which were from the fast). It can be done!

    Thanks for the update on these meds, Dr. Mike. From the research I’ve read, it seems these GLP-1 meds have awful side effects. The risk of pancreatitis is the dealbreaker for me. I am glad my doctor steered me away from medical intervention for NAFLD, insulin resistance and pre-diabetes. I eliminated processed foods and limited my carbs to 25 grams per day on a clean keto lifestyle. My weight loss at one year exceeded 26 percent and has now passed 31 percent. If I can do it, anyone can. I wish I had done it sooner before my health was at risk.
