Here are the 10 mistakes that can cause inaccurate CGM readings, according to this video:
    10. Insertion trauma: Applying a new sensor right before the old one expires can help to avoid inaccurate readings during the first 12-24 hours.
    9. Not understanding that CGM measures interstitial fluid, not blood sugar: CGM readings are always a little bit delayed compared to blood glucose readings.
    8. Sleeping on the sensor: This can put pressure on the sensor and cause false low readings.
    7. Using the wrong brand of CGM: Different brands of CGM may work better for different people.
    6. Taking too much vitamin C: Vitamin C can falsely raise CGM readings.
    5. Inserting the sensor into the muscle: The sensor should be inserted into the fatty part of the back of the upper arm.
    4. Comparing CGM readings to glucometer readings too often: This can lead to anxiety and frustration.
    3. Using CGM in extreme temperatures: Hot temperatures can lead to false high readings, while cold temperatures can lead to false low readings.
    2. Not being hydrated enough: Dehydration can lead to false high readings.
    1. Not understanding that no CGM is 100% accurate: All CGMs have a margin of error.

I got IV vitamin C in a megadose (like 15 grams) and my blood sugar readings on my libre were crazy high!!! Nobody thought to tell me, and I was so confused!!! Apparently vit C is structured very similarly to glucose so the CGMs read it as such.

Thank you again for sharing your experiments. I am pre diabetic and have experienced exactly what you have personally experienced/ experimented with. Thank you also for experimenting with LFWFPB. My mother and mother in law are both typeII and I have always asked them to be sure and finger stick before taking insulin or drinking OJ when CGM shows too high or too low. I helped my mother in law lose 100 pounds and she was successfully off of fast acting insulin! She can't walk too far so I told her to just move her arms up to the side like sitting jumping jax and it brings her glucose down, as well as when she drinks more water. Thank you for posting this very important video.

If you catch the CGM on a door frame, it's possible for it to pull enough to dislodge the filament without quite pulling loose the adhesive. This will leave it looking good but you may get disastrously low readings.

So might the consistently lower readings be the sign that the sensor isn't inserted properly? Is it then lower by some value (like 2 mmol/l) or a fraction of the real value (say, two thirds, for example)? It's my first experience with a CGM, and the mean near 5 mmol/l and never spiking above 7.5 seem not right, and I would whether in such a case one could apply some correction or might as well just discard the sensor.

Another tip is stated in the paperwork and people don't realize it: Don't calibrate the CGM if your glucose is falling or raising, and only if it's a greater deviation of 20 mg/dL compared to a finger check.
Thanks for some of these tips I didn't know. My life has been better with a CGM (Dexcom G6), hardly ever check my glucose with a meter anymore. Makes me feel more human when complimented with an insulin pump.

The libre does not take its glucose level from blood but the interstatial area around the blood cell and this lags the blood by approx 10 min. The change in the blood ALWAYS leads the chang in inerstatial whether it is rising or falling, this can sometimes cause confusion at the end of a trend.
Say the number show you at and a falling arrow you wld expect the blood to be even lower as it always leads. However when you check the blood just after the sensor reading it is higher NOT lower, the reason is the status has changed and glucose ids rising but that change is not yet being shown in the interstatial layer. This why the instructions tell you on a low to always confirm it with a finger prick before reacting with something to push your glucose up.

A few months ago, I switched from the FS2 to the DG7. I was able to prove what I feared by wearing both devices for about 10 days. The FS2 was alway 35-75 points off. Because of that, I was having serious vision issues to the point the eye doctor had me take a brain scan (it was clear). The G7 for me is incredible and I love it. Also, on point 2 of your video, I recently discovered how true and correct you are about hydration and the effect it has on accuracy. You are right on that one as well. CGM makers are not held to a high enough standard. That is the real issue. They make a fortune from us and they get to blame the users for being too skinny, too heavy, irresponsibly eating an orange, exercising, not exercising, etc. And we actually let them do it. The wildly inaccurate readings are used by algorithms for Omnipod and others. I have to do numerous fingersticks daily and manually input correct glucose levels to achieve any kind of control.

It’s frustrating but I’m not sure what they could do better. As for the unreliability, there are limitations with the technology, and the manufacturers generally will replace the sensor or out goes wrong - even if it is your fault. As for the algorithms, I agree they could be improved, but given the implications on people’s health and the dangers of low hypos, I imagine they have to be really cautious on how intelligent the algorithms can be.

I’m an EE, I tore down the Libre 1 the total bill of materials is ridiculously low. The main microprocessor costs $1.40 in 1000 pieces. There’s a few passive parts that cost literal pennies, a thermistor, and a battery. The circuit board is 2 layers maybe 5 cents. I can’t account for the filament cost or the inserter but it if I had to guess the entire Libre 1 system costs $5 all up with packaging.

    • @petervanholland6667
      @petervanholland6667 9 หลายเดือนก่อน

@@JamesReedyyup. But that’s not the way medical consumables work. You could tear down some new drug to prove the base chemicals cost pennies instead of the hundreds of wherever currency you use. It’s all about recouping the original development cost and of course driving a profit.
Hopefully because they are easy to replicate (easier than discovering a new drug) some competitor can drive down the price a bit. I guess, the analogy with drugs is different as you can’t make a “generic” version - the tech is only useful when there is an app, an integrated pump etc.

@@petervanholland6667 ohh for sure, I did a brief stint in the medical devices sector (hearing aids) and the costs are atrocious. They said diabetes is hyper prevelent and even non-diabetics would do well to try a CGM for information about their own patterns. The costs could be driven way way down.
Also the risk profile of the CGM is low…not so much for chemical compounds.

Well, there was a lot of very specific and real-world information in this video, you covered a lot of perceptions people have about the accuracy and the numbers they see on their meters both the CGM and the glucose monitoring systems we all have. You provided some great real-world reasoning and research into why there are real reasons why these CGM sensors read differently. And it is probably a surprising revelation to the average person that the CGM sensor is not actually reading your blood sugar level from your blood, it has a different method. After 35 years of being an insulin-dependent diabetic, the disease seems to have progressed to a point where I do need a CGM, but, the ongoing expense puts this device out of reach at this time. but seeing this video and others that you have created, is very important to me in information in order to make decisions and have a greater understanding of these devices and some interesting things that you have expressed about having diabetes in your past video history. I approach my diabetes and administering insulin in a very analytical way, which I believe you are also doing when you create your videos and express your findings on subjects like the CGM and other things concerning diabetes and its management. This video really gave fantastic information about these devices.

I had trouble with the Freestyle Libre 3 while I was on a backpacking trip. TWO of the sensors fell off within an hour of application. It appears (obvious in hindsight) that applying a new sensor when I got up, then starting my day's hike, was not a good idea. After discussing it with the doctor's office, we determined that I needed to put the sensor on at night before going to sleeping bag (like going to bed, except out on the trail) to allow the adhesive time to bond well before I started sweating. After that, I had no problem. (And Abbott Labs did replace the two sensors.)
Of your tips, the one about compression seems to be the one I may have been guilty of. Since m doctor only has me using 1 sensor per quarter year, I can always put it on the back of the arm that I'm less likely to sleep on, since I do often sleep on my side. Great tips! Thanks.

I have only used the Libre Freestyle 2 which gives me consistent readings that are 80-90 points too high every time. 80-90 points high reading could cause major problems for users making treatment decisions based on what they read off of a Libre 2. All seven of these Libre 2's were new and inserted correctly as per instructions.
I drink at least 100 oz of water daily and use zero Vitamin C. The Libre 2 is the only CGM covered by my insurance so I have not been able to try any other brands of CGM's.
Strangely enough I had a Libre Freestyle 2 that was 20 months old. I decided to try the old meter when a new one failed. The old Libre 2 turned out to be the only one that was spot on accurate. Go figure?
Not sure if the issue is my body or a failed product. I called Abbott and spoke with a Rep who was following a flowchart he was reading. The Rep was absolutely clueless. After 45 minutes I made the decision to terminate the waste of time phone call.

Have you experienced issues with high readings after exercise? My readings can be 6 mmol/l then 20 minutes after exercise can go around 20 mmol/l. The wierd bit is normally I would have to take 4 units of rapid insulin to correct this but if I do nothing after around 4 hours it comes back down to where it was and lower. This is normally in the evenings when I exercise a couple of hours after dinner. If I exercise in the morning after breakfast this does not happen. I normally cycle around 90 minutes each time

    • @rttm
      @rttm 11 หลายเดือนก่อน +2

I ride my bike usually after lunch and I get back to home before dinner. I have few times had it to rise after exercise. Before meal I measure always from fingertip with real glucometer too. One time the dexom one was showing something 7mmol/l, but the gllucometer showed 17 mmol/l. Then I measured again after about some minutes and it had already dropped to 12mmol/l.
But it is known that exercise can bring blood sugar up. So I guess it's normal.
I think it has happened more when I had not bicycled for a week or so. Or if it was shorter ride.
When I was cycling on cold weather and stopped halfway for a while. I started to feel chills in wind, then the CGM started to show low and then it just displayed a error message, "problems with sensor" I knew it has to be wrong, because I did not feel low sugar symptoms, also usually I won't have chills when it's so low. Rather feel hot. I was shaking with whole body from chills, so can't be low. When I started ridding it started to show blood sugar values soon again.. and probably they were correct too.
I had this happen few times. One time also after ride, because I did not go to shower or change sweaty clothes away right away.. Again I had terrible chills and then it just displayed low on screen at first and then the error message about sensor problem.
My initial thoughts were, that it's similar to compression error, because body temperature dropped so rapidly, the blood circulation to arms get cancelled to keep core warm. And with no blood supply to arms it has to go low, but not sure.. Later I read that very hot and cold can affect the reading. Yes when I go to hot shower it always go up a little bit and then after shower lowers, but I onetime compared with blood sugar from finger tip and also there it was higher right after hot shower and went down together with CGM.

Just subscribed to your channel, because I've just started using a CGM. I've been Type 2 since around 2014. I stopped using Metformin in May, substituting it with Inositol and Berberine. I feel so much better without the Metformin - no more GI distress, despite using Berberine. I want to see how effective the inositol/Berberine combination compared to Metformin. The numbers weren't great during the first three months. On the high end of test results for Metformin. This -seems- to indicate, that my inositol/berberine combo was working, but not "better" than Metformin. These last 2 weeks seem to confirm thisb - the GMI of 2 weeks was +/- my HbA1c from 3 months ago. So we'll see how this goes. My GP is "new" to CGMs, so is giving me FeeStyle Libre 2 for free. And because I'm a geek, I'm paying for a Sibionics for 6 weeks.

    • @curiouskitten

I clean the area with alcohol pad, then apply sensor once the area has dried. Never use waterproofing for showers. Just make sure I'm gentle and careful rubbing around the sensor with wash cloth. Libre 2 sensors used to come with 2 alcohol pads. Abbott stopped including them😮

For Dexcom G6 the trend arrow is not as useful as it too lags. I always estimate my own arrow by using the graph as it is always after eating or exercise not very reliable.
Sometimes it can be two arrows down, but if you look at the graph it has basically cut the drop and levelled out.

I use the libre view 2. It does automatic updates to my phone every minute. But it is often wrong and doesn't catch up with the blood meter monitor. I wouldn't rely on it totally and definitely wouldn't want one hooked up to an insulin pump. Knowing it's short coming it's still very helpful.

I made almost all these mistakes as well. I have also recently discovered that covering the CGM with a full overpatch also affects the readings significantly. Those holes on top need to be exposed.

I am trying to learn the lesson, don't freak out every time the cgm alarms go off. I am about 40% in target range, so the high blood sugar alarms go off frequently. I hope I can figure out what I am eating wrong that is leading to these high numbers. I basically do nothing when the readings are high. I know I should go out for a walk, but I am not there yet. Other than drinking water and skipping meals, what can be done for high blood sugar?

    • @esatuzundemir9321

Take into account the glysemic index of the food and the time it takes the insulin the be absorbed in the body. If you are eating something with a high glycemic index it might help to inject insulin at least 15 minutes before you eat. Certainly more if your blood sugar is already high to begin with.

    Another great video!
One of the most recent issues I've been experiencing is crazy alarms, i.e., the librelink app sends me a hipo and/or hiper alarm (normally significantly high or low values), which I'll run fast to correct. But after 5 minutes, when I check the app the values are normal and those peaks that I received an alarm about are not even registered in the graph. As anyone experienced the same?

If I I place the sensor on without the transmitter, prior to expiry of the initial unit, then the chance of foreign body entering the empty space where the transmitter is pleased?

Anyone use the freestyle Libre 3 with the following issue? I have gotten completely inaccurate readings of 30-40 below actual finger sticks readings of below 110. It’s only 1-4 off when above 110. This is day or night with none of the mentioned mistakes and has been happening with my last 6 CGMs used. My first couple seemed fine but at that time my glucose levels weren’t within my target range. It’s only when I started getting in range that I notice it reading waaay lower than actual and it keeps screaming at me that my glucose is critically low when it’s not.

I had a message concerning a sensor failure this morning (G7), and it said not to remove the unit, but to wait. It only lost 2 readings, I was sleeping on that side and it sounds like the compression failure that you mention in your videos. I have a question, Is it acceptable to inject insult (rapid acting) near the sensor? Or will it cause false readings?

I now understand much much better what make glucose go up or down, and especially, how long or fast food, exercise, meds, etc kick in. I've had 3 "defective" sensors out of the first 3 I used. I've been getting WILDLY inaccurate readings off of my DEXCOM ... including low sugar alarms and loss of signal alarms. NONE of the "supposed" causes for all of this have NOT been the causes of my Dexcom's unreliability and inaccuracy. This is supposed to be a "Medical Device" ... a scientific instrument. Based on it's lack of reliability and accuracy, it is NEITHER! One thing you can say about this device, is that it is merely a very expensive toy. I won't be using this anymore ... too old for "toys".

Issue I have with the CGM and hypo events is that it screams at you but it’s indicating what was going on in your blood 20-30 mins ago. For me, a T2D that is working my way out with diet. I’ve gone from A1C of 6.9 to 6.1 in 5 weeks with diet alone no exercise. That said my carb intake is 85-125g per day and so I’ll get these hypo events sometimes.
But what happens is even tho I dip down my liver I guess dumps sugar and I come back up so the issue has passed by the time I know about it. It’ll oscillate a bit, but I don’t stay low for more than 10-15 mins.
I guess a T1 or more serious T2 would stay down longer to the extent they need sugar? Or people who are on glucose suppressing drugs are at risk of being driven down when mine is just reactive hypoglycemia?
