@Checkpoint10na my 97 Silverado 1500 Z71 is having the same issue, in reverse will make a grinding noise but when put in drive it will only make the noise when accelerating from a stop. Could this be the issue?

What transfer case you have np243c if so i do to i need to know if this will fit
Was it all the time or sometimes? And if so did you ever had a hard thud? Mined dose this but drives fine in D 321

Mine only made the noise when put in reverse but it might be worth checking movement in the transfer case tail piece... Hope it works out for you too.
Did it take a good bit of throttle before your truck started going?
Does the fluid in the transfer case needs to be removed before attempting this job?
