    Thanks Dr. Robert! Finally someone who explains to me, and others, what the heck is going on with my blood sugar in the morning.
    I did figure out it had to do something with sleep, or better lack off it, but not the underlying processes that cause it.

    Thank you for another very enlightening and informative video 🙏
    I’m positive this is my problem! I’ve been Keto for 2 years and 7 months. Been ketocarnivore for the last 12 months, mostly carnivore, very low to no carbs 24/7. Plenty of physical activities, I’ve been removing stressors from my life for the last year. I’m eating one meal a day now. I’m fasting because at this stage of Keto it just comes naturally. It seems to me I’m doing it all right? Yet sleep evades me like the plague.
    First off I’m a vivid dreamer. Oh my dreams are colorful, loud, specific and it’s like being in another world when I have them. My dreams actually come to fruition in real life sometimes. I’m also an artist, so my mind is full of continuous ideas for art pieces. Other rare times I dream nothing as if I died in the bed. I’m lucky to get 5 hours of sleep.
    Dr. Cywes how can I solve this? I know it’s impeding my Keto journey. Don’t get me wrong, Keto has done miracles for me in these 2 years 7 months. It’s just the sleep I can not conquer and I know it’s caused my weight loss to stall and cause the sensation that Keto has stopped working.

    Wow! Dr. Cywes, thank you for sharing. I’ve had issues with my sleep since my twenties. I’m 52. I worked 12 shifts with a month of days then a month of nights and back and forth for quite some time. I’ve always needed much more sleep than it seems other people needed. I feel overall poorly on 5-6 hours a night. I’ve started getting better sleep lately and it’s making a huge difference. This all makes so much sense.

    Sleep is the most frustrating topic, because there is no magic pill that gives it to you. Every pill has a side effect that screws you in some other way.

      True. But there actually isn't a magic pill that gives you anything without screwing you in some other way... It's just easier to notice it with sleep. No such thing as a blood pressure pill, anti-diabetic pill, anti-anxiety pill, anti-infective pill or any other kind that gives you nothing but good effects without the downsides. The medical industry (that I'm in) does a horrible job of making sure people know this.

    Excellent presentation, gives me ideas on controlling my dawn effect, which is more like after-dawn for me. Glucose on waking is usually around the 7.0 mmol/L (125) but steadily rises to around 8.5 (155) by mid morning. After that it slowly reduces throughout the rest of the day.

      similar situation, 7 months keto, fasting glucose is 140, lost 40# but have stopped losing for weeks. but, strangly, my A1c is 5.3, go figure

    Well, that explains a lot. I am diabetic but I am not on medication, I am controlling it with a keto diet. I have had problems with sleep for years (late shifts). I also am not loosing weight anymore on keto. I am now just starting to sleep a little better so perhaps things will start to change around (so far still have the dawn effect). Thanks for the info.

    So much knowledge!! I truly wish you were my Dr. I know without a doubt my Type 1 diabetes would be under control. Last A1C was 8.2... Awful... lots of stress and no health insurance...ugh!!😦

    I have cured my type 2 diabetes, lost the weight normalized all of my blood work but have been confused as to why my sugars run higher than I would like after an 18 hr fast and less than 20 carbs per day. I have had sleep issues for years which I am working on with some success. Have gone from having poor sleep every night, never achieving REM or deep sleep and at least 3 nights per week where I do not sleep at all to regularly getting an hour of deep and 1 and a half of REM. I am moving in the right direction and now have a better understanding as to what is happening and how to fix it. You are an inspiration Dr Cywes. Thank you.

      I question the need for 8hrs sleep...In fact, why isn't the amount of sleep needed by each individual vary like the amount of food, water, macro or micro nutrients?

    Thank you. Once again you have solved a mystery. Years ago when my insulin was extremely high I had low growth hormone and suffered extreme muscle weakness.
    I have had lack of sleep from a new baby recently and my insulin has in back in diabetic ranges. This explains my muscle weakness again and I will have my growth hormone tested to see if it is low again. I knew all this 6 years ago from researching at the time. How could I have forgotten?

    This all makes sense. I don't have blood. Sugar issues during the day. I have high blood sugar of 95-100 in the mornings then it is fine the rest of the day. I haven't slept week of 4 or 5 years and this explains why I'm dealing with the dawn phenomenon

    Hormonal lady here …cannot remember the last full nights sleep I had … diet good low carb …no processed carbs at all …and exercise also …but a full nights sleep is elusive …..

    I've been resigned to insomnia for years which I thought was an effect of hypothyroidism, but never considered the lack of sleep may actually be contributing to the hypothyroidism. I thought my hypothyroidism possibly stemmed from oestrogen dominance. Very interesting thank you.

    Thank you for the explanation. I could not understand why BS high in the morning. Not diabetic soooo? I need more sleep! No naps to supplement lack of good nightime sleep and a bit of exercise EVERY day. Going to give it a go!!

Medications for pre diabetes dropping off down to 1. hahahhahh Sleep Baby, Sleep. you said. Has worked for me my whole Life. Great Grandmother was right. Not one of my previous docs EVER had a clue, ( eat 5 to 7 times a day just small meals :(, kept me fat and ignorant for years.

    I have been wondering for a long time what causes the dawn effect. I didn't know it was directly tied to sleep. Yet another reason to cut out caffeine. I am already on keto, but when I have caffeine I don't lose weight. I have also had to cut back on meat to only 3 oz per meal (see Dr. Berg) and not eat dairy due to inflammation and gout like symptoms (joint pain) including uric acid kidney stones.

      I find that exercising daily makes me sleep well at night.

      Have you tried some nootropics? I tried Ashwaganda (sensoril) it works really well, as most of my problem with sleep were stress related, so Ashwaganda on an empty stomach, a light dinner, 45 minute walk seems to work for me...even those very very heavy stressful days (work + personal, those shit storm days). Best of luck!

      I also sometimes find it hard to get a continuous 8 hrs of sleep. I found that, whenever my life permits, short naps (or siestas as we do here in S Europe) help me get enough sleep during the day.

    My middle name is dawn effect. Waking maybe 110 then peaks at 130 or more. OMAD, In ketosis. 1000 mg metformin at night. Well, Bikman says when you are producing ketones, you are also producing glucose. Only time it goes away is 2 days into a fast. In the 80s. Right back next morning after refeeding one meal. I only sleep 6 hours, but feel stimulated and energetic when I wake.

      @@shoegal7 no I haven't. I do believe my cortisol is elevated.
      It's funny that I can fall asleep within 30 seconds of lying down but sure as clock work I wake up and my mind gets active. Even if I fall asleep in this 3am state it still feels like I am awake. My solution has been to take a melatonin to bed with me and pop it when I wake. But then I wake up groggy.

    Hi Doc I have been T1D almost since birth. I have never been unable to develop muscle mass at all. Have used Insulin for 73 years. I have pushed weights, cycled 1000's of kilometers. I still only weigh 66 kgs, with no muscle development at all. Why does this happen?

    How to Omega 6's play into the inflammation ? I have noticed a substantial decrease in aches and pains since going ketovore about 4 months ago... now I'm wondering if I should up my Seafood to increase the Omega 3s to mitigate any Omega 6 exposure I get...

      I am doing the same thing and have noticed less pain. I prioritize protein, focusing on more Omega 3 sources. Under 10 grams total carbs daily.

      Yeah canned sardines are staple for me- improves my mood considerably, pretty sure it's associated with the omega 3 content

      Also, for some people too much meat (& sometimes dairy) can increase uric acid & purines to give gout symptoms which includes inflammation.

      I lost the weight with serial 10 day cold pressed juice (limited to one fruit) and water only intermittent fasting. At the end of the juice I switched over to eating a clean paleo/keto one meal a day within a 4 hour window until I am pleasantly stuffed. Over the week or so I learned how to eat and then I went back to the juicing. In 3 months I lost roughly 60 pounds. Now in the last month I’ve only dropped 5 pounds but I’m in control and this maintenance phase is an adventure.
      I pay particular attention to my omega 6 to 3 consumption. I’ve never been much on the sleep thing and now that I’m retired I sleep when I want. My theory is we all have a set genetic sleep need. To balance the hormones, the yen and the yang thing, your insomnia is because you slept too much. If your balance needed sleep hopefully you won’t be driving.

    So inject 4iu of human growth hormone per day and inhibit somatostatin for maximum anabolic benefits? That's what you said right? And blast IGF1-LR3 for a few weeks now and then to reset insulin sensitivity and build even more muscle. I'm pretty sure you said that too. Thank Dr

    I am desparate to get sleep. I have not slept more than 1-2 hrs for 4 months from anxiety. Glucose is out of wack! Pain and inflammation flaring up. I have done keto for 5 years, so no sugar or starches here! Have always had problems sleeping from childhood, am now 63. Have not found any relief. My pre diabetes was reversed with keto with the exception of high cholesterol, had tons of energy felt great, exercised, but now everything is out of balance again in just the last week or so. I am barely able to function. I am very concerned. What do you suggest to get sleep when your body or even drugs won't let you? Please help!

    This is off the subject but I stopped taking Metformin, cold turkey, 500 mg once a day because it was causing me bathroom issues and I was uncomfortable going anywhere. Do you think I made a mistake? I am taking Berberine, Ceylon Cinnamon and some other supplements to help control my sugar. I also do Low Carb/High Fat and moderate exercise. However, I have been off the wagon for a few days now and plan on going strict again starting Monday (Aug 9)

    I can’t get my dawn effect below 200 and on a strict low carb carnivore diet. I sleep well but get up to pee 2-4 times a night. Go right back to sleep fast and dream (rem) most of the night. Very frustrating. A1c climbing.

      Share some info: age, sex, exactly what you eat, and how many hours you sleep. If you know your most current lab results, make sure they seem to make sense. With some more info, people, including me, might be able to advise you. 😊

      You may not be achieving deep sleep during your nightly naps. Google Dr. Stasha Gominak's talks on youtube or her website. Specializes in educating/treating exactly this problem.

      FWIW all my labs are decent except climbing A1c and insulin like 15 instead of 6 where I was doing.better. I’ll check this out. I’m pretty sure this is my issue to solve.

      Sedation jk lol glycine/gummies after supper would be my first try and they may like them! I make my own with jello as a dirty way and can buy gelatin in different qualities, and raspberry (and other berries) juice powder. Just search keto gummies. I love them too :)

    I honestly wish GHB did not because this rape designer drug .When I was able to buy it over the counter as nutritional supplement one scoop and you were in the deepest REM sleep for 4 hours with a dopmine surge to boot when you woke up not a sedating hangover.
