The disparity in the two observations on whether H2S leads to diarrhea or constipation arises from the fact that H2S induces loose stools that move at a slower pace than usual. Typically, constipation is linked to hard stools rather than loose ones.

Dang, you nailed it. We have all of the above sensory sensitivities, three kids diagnosed with autism level 1. I’ve had H pylori from when I grew up in Ukraine, I went grey at 25, which I always thought was due to hydrogen overgrowth. I’ve managed it for over 40 years till Covid messed even up and put me into menopause. Now I have to figure out what to eat, but yeah, you are on to something, thank you! I will be following.

Hi Doc...you mentioned the discrepancies between H2S leading to diarrhea or constipation. One of the primary ways that endogenously produced H2S is generated is by the microbiome through the sulphur-fixing bacteria (SFB) action. As with methane (CH4) production, H2S production is a means of trapping molecular hydrogen (H2) produced upstream. So SFB uses 5 molecules of H2 to produce 1 molecule of H2S, and the methanogens (methane-producing archaea) use 4 molecules of H2 to make 1 molecule of methane. So they compete for the H2. This is ok because this way, the methanogens and the SFB maintain a functional balance with the H2 producers while simultaneously supplying the body with a crucial gasotransmitter, H2S.
Ok, now, the SFB has a substantially stronger affinity for molecular H2 than the methanogens, which means that H2S is more likely to be produced than CH4. All these gas-producing creatures need to be in our bodies, but at levels that benefit our physiology. Ok, so here's the scoop...individuals with excess H2S (proven by symptoms and a good doctor) have the propensity to have diarrhea, but less commonly, constipation, but sometimes, neither. There's a tendency to see constipation in methane producers because methane interferes negatively with the MMC (powerful muscular movement downwards of the GI tract). I hope this makes sense. The problem with H2S is that most health practitioners treating SIBO are now all over eliminating H2S. But this is a big mistake because we NEED it for many physiological functions. An excess of H2S will cause 1 or more symptoms you have discussed. But most importantly, the cause(s) of SIBO need to be identified and addressed first and foremost. 👍
Great job, by the way! 🤜🤛

      This is very interesting.
If i understand correctly, if one has SFB (H2S SIBO) or methanogen (methane SIBO), hydrogen from any source (bacterial or Hydrogen water etc) will act as the substrate for the production of H2S/methane?
This is super interesting to me because many people react badly to hydrogen gas therapy.
Any further info, sources, details would be greatly appreciated.

This is a awesome breakdown! A couple things to add maybe? Watching some updates with pimmentel group and dr. Nigh h2s looks like an adaptation to some type of sulfate deficiency.
I have the dizzy brain fog unsteady version (like I been on a roller coaster) no tingly, but lots of smelly gas and of course ribflaxmin worked for like 10 days and it came back (x3)
My Gi map is pretty classic Sibo both times without any changes: kleb, strep, ecoli.
High on the breath tests. Black stools when I take bismuth.

Hi Dr DiNezza, thx for the valuable info, Hydrogen Sulfide is mitochondrial poison, but is there a connection between H2S produced in the gut affecting the brain? Or just a reduction in mitochondria in the gut can be enough to cause this overall body fatigue?

    • @gutmicrobiomequeen
      @gutmicrobiomequeen  2 ปีที่แล้ว +4

Great question! There is actually an abundance of research showing that H2S is neuroPROTECTIVE 🤯🤯🤯 I was thinking of doing a video titled "you should LOVE H2S" but I wasn't sure if anyone would watch it 😂 so I've tabled the idea for now.
🍗 But, of course, neurons need ATP, so mitochondrial impairment isn't going to do your brain any favors. The take-away here is that low to medium amounts of H2S are anti-inflammatory and protective, but excessive levels are harmful.

Is it common for H2S sibo to cause feeling of being poisoned? I wake up in the middle of the night and feel disoriented, poisoned and it goes away only after I pass gas. Then the same thing happens in the morning. If I go long hours without eating then I don't have the poisonous feeling. It clearly stems from my GI area, accompanied with gas, belching, nausea and so on. Feels like the gas is reaching my brain.

What would your recommendation be if I've gone to my doctor and he did tests for ulcers, an endoscopy with biopsy and gallbladder scans and everything came back negative? All he did was say wait two months and kept me on protonix and karafate (which haven't helped) I've read a lot and think sibo could be a cause of my painful bloating symptoms. Should I just suggest it to him as a possibility? What should I expect him to do next if he thought that was behind my issues?

Latero Flora by Global Health for gas, bloating, and flare + inflammation
I don't know if you went through strain-specific reviews and info videos? that would be amazing.
I have anti-resistance gene from antibiotics overuse since I was a child and I have struggled a lot with bloating, gas, severe constipation, horrible slow motility (only one meal a day), and allergic reactions to most if not all food. my GI MAP is complete chaos

    Dear Dr. DiNezza,
first of all thank you for your very interesting videos. I noticed in this video, you talk at the 4:45 min. mark that you either have too many mast cells or mast cells that are too active in visceral hypersensitivity. Too many mast cells or too mast cells that are too active is an important marker in the diagnosis of "MCAS" - mast cell activiation syndrome.
Do I understand you correctly that visceral hypersensitivity only occurs in Hydrogen Sulfide Sibo, not in Hydrogen or Methane SIBO?
That would mean that mast cell activation syndrome could be linked to Hydrogen Sulfide SIBO. I think that would be a VERY important fact for those docs that try to cure MCAS.
Do you have any links of the research you talked about regarding the visceral hypersensitivity and the mast cells that I could give to my doctor?
    Best regards
    Stefan

I’m assuming you forgot to mention it in this video, but I guess it’s theoretically possible that you changed your mind in that year-long period, but I’m guessing that that’s unlikely and that you just forgot to mention that symptom in this video.

I had sibo treatment, hydrogen dominant and methane positive. After the first antibiotic the hydrogen disappeared but the methane remained. After the second antibiotic, my symptoms decreased but did not completely faded out, 6 months later I had a sibo test but it was negative. Now I'm wondering if it's hydrogen sulfide. I have extreme tenderness in my lower abdomen, the pain is not always there, but the bloating happens even when I eat the slightest thing. I also have a symptom, for example, every time I get cold on my feet, my stomach swells like it's going to explode with pain. Could this be an indicator too?
