Hi. i wanted to ask about the peak levels if they were high. We know this could lead to toxicity, and PK factors could have influenced it. But is it correct to say that the trough conc also could have influenced the peak to be high, meaning the trough conc was higher than the appropriate range
There is no general answer to this question as both also depend on the half life of medications and individual patient factors such as age, kidney function etc.

So peak & trough test shows drug concentrations in specific tested organs? Or is this test showing amount of the active drug working physiologically-pharmacologically in serum levels in specific organs? It’s not indicative of effect on perception at any time?
Peak and through measure the serum concentration of a specific drug to determine steady state, dosing, and metabolism of that specific drug in a person's body.

Might be a crazy question, So how do you know if it has reached the toxic effect?
The patient will exhibit signs and symptoms of toxicity plus their peak levels will be high.

The peak would be measured right after the infusion is complete and the trough right before the next dose is due.
