  • Out of all the different antibiotics, aminoglycosides are one of the few that a lot of people may not feel comfortable dealing with.This is because of the different dosing methods, renal adjustment parameters, and drug level monitoring. Being the drug specialist, it is crucial that we get comfortable because all other health care providers will turn to us for recommendations. In this video I provide a brief overview of aminoglycosides and dosing. I've included clinical practical questions to help us apply what we learned because sometimes despite giving the right patient the right dose, the response may not be what we expected. Its important to know how to approach that situation.

    Just wanted to clarify for patient #5, question 2. The peak and trough is normal. There's no need to dose adjust. The trough is on the lower end but all we can do is monitor the patient closely for improvement in their clinical presentation. Patient is at q8hrs currently so the frequency can't be increased hoping to increase the trough slightly. In clinical practice aminoglycosides are not normally dosed q6hrs. Hope this helps.

    Please, explain what formula do you use to calculate Ideal body weight ? Is this one correct for males 50kg+(0,91 x [ height in cm - 152.4 cm])?

      Hello I use --> 50kg + (2.3kg for every inch over 5 feet). Females are the same but use 45.5kg instead of 50.

    I was prescribed gentamicin for 6 weeks. My dose was everyday at 10pm. I don't recall peak and trough levels being monitored, perhaps peak and trough were measured once in that first week but in the remaining 5 weeks I only had blood samples taken every morning around 6 hours after my dose. No trough levels taken half hour before dose or peak levels after my 60 minute infusion. Is this normal? No dose was skipped until I refused my final dose due to symptoms. I'm left with irreversible damage to my vestibular system and vision.
