Every death that happens in this series is either a direct or an indirect result of Walt's actions. That's why the color of Walt's sweater matches the color of the teddy bear. Creator Vince Gilligan was intentionally making a VISUAL CONNECTION between Walt's actions and the consequences of them (i.e. plane crashes, unforeseen deaths and so on).

As to whose fault Jane's death and plane crash was: Jane's death was Walt's fault. When writing the show the writers first wrote that Walt purposefully rolled her onto her back while she was vomiting so that she would choke to death, but they decided that was too dark and overtly evil for Walt's character at this stage of the show (sure he's killed before, but it's always been in self defense. Jane would have been straight up murder). Anyway, they chose for him to kill her by inaction (which is pretty grey). Jane being back on drugs is no one's fault but her own. The night Jane started using again Jesse was very respectful of her recovery and asked her to leave. His best friend was just murdered and he felt guilty. He chose to cope with those feelings in a very unhealthy manner (getting high), but that's what addicts do and Jesse is an addict. Jane knew he was a drug dealer when she helped him lie on his application to rent the apartment and she chose to get involved with him knowing this. If she was serious about her recovery she would have never let him move in in the first place. In the episode she started using again you can see the very moment she decides she wanted to get high. It was all her choice from the very beginning. The crash was on Walt. If he never went back to Jesse's, never broke in and never tried shaking Jesse awake Jane would never had rolled over onto her back where she then asphyxiated on her vomit. (This was also foreshadowed in earlier episodes. The night Jesse first shot heroin Jane rolled him onto his side and put a pillow behind his back saying, "Roll over on your side baby, incase you throw up." Walt killed Jane (let her die) and her distraught father made a fatal error at work. Also, Jane didn't die from an OD. An opiate overdose causes respiratory depression (you just stop breathing). Opiates can cause vomiting, but vomiting in of itself is not an OD symptom.
In a previous episode Jane specifically mentions sleeping sideways in case they vomit in their sleep, So they were conscious of that risk, and specifically slept sideways to prevent it, Walt's presence in that room and intervention by nudging Jesse resulted in Jane rolling onto her back... Take that for what you will and who you think is to blame.

As to whose fault Jane's death and plane crash was: Jane's death was Walt's fault. When writing the show the writers first wrote that Walt purposefully rolled her onto her back while she was vomiting so that she would choke to death, but they decided that was too dark and overtly evil for Walt's character at this stage of the show (sure he's killed before, but it's always been in self defense. Jane would have been straight up murder). Anyway, they chose for him to kill her by inaction (which is pretty grey).
Jane being back on drugs is no one's fault but her own. The night Jane started using again Jesse was very respectful of her recovery and asked her to leave. His best friend was just murdered and he felt guilty. He chose to cope with those feelings in a very unhealthy manner (getting high), but that's what addicts do and Jesse is an addict. Jane knew he was a drug dealer when she helped him lie on his application to rent the apartment and she chose to get involved with him knowing this. If she was serious about her recovery she would have never let him move in in the first place. In the episode she started using again you can see the very moment she decides she wanted to get high. It was all her choice from the very beginning.
The crash was on Walt. If he never went back to Jesse's, never broke in and never tried shaking Jesse awake Jane would never had rolled over onto her back where she then asphyxiated on her vomit. (This was also foreshadowed in earlier episodes. The night Jesse first shot heroin Jane rolled him onto his side and put a pillow behind his back saying, "Roll over on your side baby, incase you throw up." Walt killed Jane (let her die) and her distraught father made a fatal error at work.
Also, Jane didn't die from an OD. An opiate overdose causes respiratory depression (you just stop breathing). Opiates can cause vomiting, but vomiting in of itself is not an OD symptom.

So they were conscious of that risk, and specifically slept sideways to prevent it,

Jane earlier this season told Jesse to lay on your side after doing heroin so you don’t choke if you vomit. Walt went there and shook Jesse hard trying to wake him up. That caused Jane to roll over on her back and begin to choke on her vomit. Walt then let her choke and die without helping for his own gain. Now losing your only child would destroy any parent and being a air traffic control operator is already a high stress career. The fathers error is a direct result of cause and effect Walt set in motion. Not only did he knock her over and let her choke, but Walt shouldn’t have been there in the first place. Lol how was that lost in translation watching this show. Breaking bad does a great job at storytelling yet it seems to go right over peoples heads. Yes Walt, Jesse, and the father have a part in causing the plane crash. But again, this all stems from Walt which makes him the main one at fault. I rock with y’all btw I’m not hating.
