- Nortriptyline or Pamelor falls into the tricyclic antidepressant family that also includes amitriptyline, imipramine and clomipramine. Anxiety related disorders constitute the approved indications for nortriptyline, however the drug is frequently prescribed for a long list of off label conditions.
Among the reasons doctors find to prescribe nortriptyline are neuropathic pain, fibromyalgia, irritable bowel syndrome, insomnia, bedwetting and insomnia. Even with newer more specific alternatives, nortriptyline still appears useful as a cost effective method of preventing migraine headaches.
Reliance on nortriptyline and other TCAs to treat depression precipitously declined with introduction of the SSRIs - selective serotonin reuptake inhibitors. While the SSRIs may cause fewer side effects in general, nortriptyline counteracts depression as well as the newer drugs such as Prozac, Celexa, Zoloft and Lexapro. Interestingly for those more severely depressed, especially with the so-called melancholic depression, nortriptyline and related TCAs appear more effective than the SSRIs.
I am taking 10ml of Nortriptyline for neuropathic (Diabetes & Fibro) pain that was quite severe and interfered with sleep. Within 48 hours the decades old shooting pains receded. So dear Doctor, thank you for a breathtaking presentation. I am now 70, have lived with the forementioned disorders and no one ever offered tri-cyclic meds...unbelievable.
I am glad to see your comment. I have the same problems that you had and have not had a good night sleep in a very long time. I will be taking 10 mg and hoping that it works with little to no side effects.

I have OCD but I take this mainly because of horrific migraines set off by light sensitivity and my god its changed my life. Hardly get migraines anymore. I thought I was having something super serious happening because I couldn't even string sentences together, couldn't walk in a straight line, memory loss etc.

@@isabelhuerta4963 Sorry for the late reply. I did feel a bit drowsy for the first few weeks and that can happen when taking new meds. Since these can make me drowsy even til this day, I take them at 8pm every night and it actually helps me sleep too.

I just started two days ago . I just feel very sleepy .
Yes!! I take 30mg nightly, and it has saved my life. I had insomnia, migraines and depression and this Helped so much. It took about a month for me to notice a difference and increased dosage for nerve pain. I also take a migraine medication called sumatriptan when I feel one coming on, works wonders!
I went to two more doctors for second/third opinions they both said I don't have migraines, something else is causing headaches. Most likely a pinched muscle or nerve in the shoulder that had a broken clavicle bone from 20 years ago that is now giving side effects after possibly healing incorrectly. For now I'm taking Sumatriptan and have to go to another "pain specialist" to see what he determines. However, I won't be taking Nortriptyline for this, seems there might be another way to work with this pinched muscle/nerve situation. @@nicoc7024

I’ve been on this for over a year to treat chronic headaches due to tbi. It was very helpful to the point my neurologist tried weening me off and it’s spiraled out of control since then. Headaches every day and anxiety can’t get back to the right dose
I had chronic headaches post concussion syndrome. I've been on nortriptyline for 1 year 6 months and it stopped migraines completely. Did you wean off successfully?
@@honeredchimp4814 No I’m actually still on it to this day. I had occipital nerve decompression surgery 6 months ago which has helped tremendously so I’m thinking that I’ll start decreasing the medication in the near future. Did you have any difficulty coming off of it?
@@B137-w2c Sorry for 7 months late lmao but I was at 50mg I've weaned down to 15mg. I had to wean down very slowly 10% per month. Still trying to taper.
I’m so glad this medicine has helped you. It totally transformed my life in an amazing way and I will never be stopping it. I had a new daily persistent headache all day, every day for 3 years. This was the only med that actually helped me. It’s done wonders for mental health. I am so incredibly thankful for it.
Pinch nerve in neck, pain from neck to thumb, can't sleep. Family member gave me a full bottle of the 75mg going to try this tonight, wish me luck.

I have taken 75 mg for 3 years now for severe depression, i work out 2 to 3 times a week and ive never felt better. The depression did not come back ,No side affects i can feel of😊😊
Dveapam. That’s amazing to hear! I feel as you do. I’ve been on 75mg for almost two months and the med is so incredible. My mood is miles better, I sleep better, I have more energy and yet I feel so calm and content. I still have all my normal emotions which I love too. I find it actually is very energizing and sped up my metabolism to the extent that I had to eat more to maintain my weight. I was so irritable before taking this and easily upset. Now I feel more social and cope so much better.
Out of curiosity, did you explore higher doses than 75mg?
I also have new daily persistent headache for 3.5 years and I find this med does help. I also have other autoimmune conditions and I find the med helps them too. I will be on this for life because it’s such a huge positive and necessary change.

Hello doc. I Hope you do a video on pregabalin(lyrica) sometime ,for i its use off label, as in benzodiacepine or Tramadol withdrawal . Its own potential for abuse/tolerance/withdrawal. Thank you a lot. Trying to get off tramadol(400 MG)and klonopin (4 MG) per day. Insomnia and anxiety is so insidious. Was thinking of using It during táper . Thanks a lot , BEST vídeos!

I second that. I use Lyrica, I had to discontinue it because it's physically addictive (GABAergic) but I keep it around for emergencies. I have really bad insomnia
Private groups on facebook help. People talk about how they do their taper stories. I tapered myself off Klonopin. Hope this helps.

Started taking Nortriptyline after random head pain. I think I damaged the occipital nerve somehow. The pain took me out of commission. Nortriptyline helped me massively. It calmed the nerves so I don’t live in constant pain. I’m scared that the drug will somehow become unavailable because it really saved me. I was in constant pain prior. Hasn’t affected my mental health either!

Kind of helped with my depression but was super good for quitting smoking. I had been smoking for about 20 years at that point. Weight gain was the worst I think I put on 18 kgs in 4 months (180mg)
This crap messed me up and I wasnt warned I gained 40 pounds in a month and was suicidal

I really appreciate the information regarding the current literature related between Nortriptyline and migraines and the efficacy of older and cheaper drugs compared with newer injectables. However, the presentation of this information was like reading the medication pamphlet. It was wordy, technical, and hard to digest. If I were not highly educated with an advanced vocabulary, this would have been really confusing.

Could you do please do some videos on the old school MAOIs? (Parnate, Marplan, Nardil, Emsam) If you do, could you please also do an additional video in which you discuss the commonplace bridging and adjunctive therapies with MAOIs (including adding a stimulant and/or a secondary amine TCA-those which act as selective NRIs-like Norpramin, Pamelor, or Vivactil, which are classically contraindicated in medical literature primarily out of misinformation), etc.
Agreed. MAOI + nortriptyline is considered one of the most powerful antidepressant treatments possible

I've been on this med from hell for years and reduced my originally prescribed 50 mg down to 10 mg. Slow reduction to get off of it over time is nearly impossible because of severe physical pain all over my body and hard to stand because the bottom of my feet hurt. I never had any feet issues before this drug (and deep depression). I've always had great focus, but this drug has trashed my focus even at 10 mgs. I did get off the med for 2 yrs - which were filled with deep depression and mental issues of which I never had before the drug. If any Dr. actually used this med for 1 year and tried to get off of it, they would NEVER prescribe it again. I would suggest NEVER using more than 20 mgs a day. I would also strongly suggest that you start taking 10 mgs and each week increase slightly. I found the 2nd time I went on the drug doing this slow weekly increase that 40 mgs made me feel insane.

I was on this medication for 5 weeks. 100mg. I gained 12 kg’s. I think it started to help but the weight gain was too much. And I couldn’t pee. :(

After hearing all your arguments i seems there's no point taking the antidepressants because both ways the problem isn't going to be resolved as instead taking them poses a greater threat.....

anafraynl helped me a great deal with OCD. Even if only taking it to break a cycle. It helped

I was prescribed nortriptyline and pregabalin for peripheral neuropathy, and nerve damage from a car accident, (a car literally ran over my head after hitting me) my feet up until my knees burn like they are dipped in hot chip fat.
The most excruciating pain I've ever been in. Pregabalin and nortriptyline have numbed the pain. That's all they do, is mask the pain and make it more bearable. I can't feel my feet when I walk its so strange. I'm on 30 mg of nortriptyline and at the beginning I was on 1200 mg of pregabalin but now I'm on 125 mg three times a day. And it does work, but as I said it just masks the pain. I've had this for 6 years now.
What about a sleeping pill ?
I told my doctor I had a terrible time sleeping and he prescribed me THIS?! WTF?! I am grateful I saw this before I took any. He can take these pills and cram them up his ass.

I may have to take this (and possibly gabapentin) for “neuropathic corneal pain”. I’m taking autologous serum tears to regenerate the corneal nerves. That helps with the “peripheral sensitization”. Hopefully I don’t have too much of the “central sensitization”. Would love it if you did a video on neuropathy and central sensitization.

I am thinking about taking this for chronic headaches and allodynia on my scalp, but the possible effects on the heart is what is scaring me from this, I would only be on a low dose once a day so I imagine the possible negative effects should be minimal. Has anyone had heartburn issues at startup? Did anyone manage it or have it go away successfully? I have tried it before bed or in the morning with and without food and no matter what I do, I have constant chest heartburn pain from it. It’s the one antidepressant that leaves me feeling so much like my old self. If not for the heartburn I’d be on it.

Did you find an answer or did it end up going away? I assume omeprazole would work if you're still experiencing it. I would talk to your doctor first.
Sadly I never found a successful way to tolerate this medicine because of the heartburn. I have had very severe bouts of c diff in the past and my doctors will not put me on an acid suppressor. I would never want to take a ppi indefinitely either. It’s amazing as I never haveheartburn pain yet I take one nortriptyline 10mg pill and within 12-24 hours I develop constant pain. It lasts all day and goes away within a few days of stopping the nortriptyline. I don’t have a large intestine and cannot tolerate any ssri due to excessive diarrhea. I found the same with snri’s like Effexor and pristiq. Trazadone made my heart race even in modest doses, remeron worsened diarrhea and left me lethargic with high triglycerides despite a strict diet and very sore dry eyes. Wellbutrin worsened diarrhea too and kept me awake all night. Amitriptyline left me feeling drugged which was a horrible feeling. The nortriptyline felt miles ahead of all of them if it wasn’t for the chest discomfort.
Now I find myself struggling with insomnia, and I know I have underlying anxiety and am clenching my jaw quite often. It’s tough. Most antidepressants are so disappointing in terms of the side effects.
hmm based on my research ken and experience nort. is the same as elavil, so you are supposed to have less side effects just as cetirizine is of hydroxyzine and just as lexapro is citalopram or prostiq is venlafaxine but less side effects
i think you have sensitive stomach like me! no eating before bed and drink lime or lemon before bed! omprezole even ken landow said he is right i trie dmany times ends up causing more reflux! try eating with it? if not then try an anti acid but not the calcium ones are dangerous theey cause kidney stones like dr ken landow said (yes he teaches alot of stuff his videos are super long)!

Someone please tell me how to get off this horrible medication. I take 10mg every night which was to help with pain and to be able to stop taking pain meds. I've tried going cold turkey on the nortriptyline but the withdrawals seriously feels worse than when I stopped taking the oxys. My entire nervous system starts wigging out and I'm scared it's triggering my epilepsy. Can I half the 10mg pills? I just want to get healthy and be clean from all this rubbish medication 😔

Why would you go cold turkey? Talk to your doctor, you don’t simply go cold turkey with a tricyclic antidepressant, it’s not a game. Talk to your doctor, ask them how to get off the drug.
Also, 10mg is a super low dose. I didn’t see a palpable effect until I took 30mg and I’m only now after almost a year on it considering leaving it for a different treatment.
This is not a game, dude. Your mental chemical balance is not something to play with, it can permanently mess you up.
