NTA. She's throwing a fit and making ultimatums because her ex moved on more than 3 years after their break up. OP made the right choice. The "friend" is a grade A piece of work.

You made the right choice. You had no problems with your brother. She had no problems with your brother until she created one. Anyone that would make such a demand about your wedding has lost sight of the friendship. Stop stressing about it.

OP, it wasn't really your choice - she MADE you choose. So it became her choice to force you. You did the right thing. Since they both dated other people after their breakup, I'd surmise that Kate wasn't really over your brother, and maybe saw your wedding as an opportunity to reconnect.
She made an unreasonable demand and you made the right choice, it's not that you have a bad relationship with your brother but invited just for being family. If you were uninvited your brother the situation would have been much worse and eventually everybody would have knows the reason was her who suppose to never have been in a relationship with your brother.
Your ex friend is obviously not over your brother, while sleeping with others...damaged women like this have tendencies to destroy your relationships and drag you down to their misery...you did good with your decision and untill your ex friend reflect on her own stupid behaviour and come back with sincere apology leave her where she is, in past
