I had a diagnosis by an orthopedist with an x-ray, an MRI and 2 months of physical therapy. My initial examination wasn't nearly this detailed and now I wish it were just to satisfy my interest in learning about it. Diagnosis was supraspinatus tendonitis, two small tears in the supraspinatus muscle, impingement of a bursa and osteoarthritis. Probably an anomaly (round shoulders). Still have some pain due to my stubborn refusal to stop working out and playing sports. I did have a good doctor .
Amazing video. Im a Primary Care Physician, almost everyday patients arrive complaining Of painful shoulder pathology. It will be very useful to take better decisions. Thanks a lot.
@@scotthowieson1191 Good evening! Yes indeed sir they did. Evidence of torn ligaments throughout the trapezius, old fractures in both hand bones (x-ray), my right leg was 1/4" shorter than the left, lower back pain cause by weakened abdominal muscles. I didn't realize how torn up I've been, still am actually, currently nursing yet another swollen lower back muscle. Was instructed to see massage therapist in addition to some unusual but effective exercises to be done daily. I see a massage therapist every 3 weeks now. The worst was when the therapist asked me to resist his ONE arm with both my legs and my legs lost! He just casually subdued them didn't even feel like he was trying, said my abdominals had "turned off" and my lower back was doing most of the work. A bunch of other technical notes but following the exercises put me in a far less painful state.
I really appreciate your effort and comprehensive examination! It helps me a lot as a new medical practitioner here in US . More power and God speed! 😊
Thorough and systematic shoulder examination presented by an acknowledged and beautiful doctor with a marvelous british accent and soothing voice. Doesn't get any better than this. Thank you.
This has helped me SO much in learning these different tests. I appreciate the anatomy pics, on-screen labels and the unhurried explainations! Studying for my PA summative exams.
Much appreciated your video because of its combination of demonstrations and words followed by images indicating the anatomy of concerned parts thanks a bunch
Me too! I have pain in my shoulder for over a yr and all my doctor told me was “I’ll get X-ray, I see nothing on X-ray.. u are getting older, getting older isn’t fun, it hurts.” I didn’t go back to her.. now I had a car accident and it seems a bit worse. But I don’t have a regular doctor yet
Time?It is up to you, choice... To be or not to be a good doctor. A doctor passionais about the profession and with empathy for all patients. You guys get paid more than any other professional. Please do not give me excuses... You are paid more them any other profesional.
Right on! My “doctor” was pathetic, 1 abduction movement; arm at 90 degree then push down. Conclusion: “you have a weak arm.” Really doc? I knew that, tell me what the problem is. “Let’s do an MRI.” What a scam. I’ve heard others complain also, so it’s not just me. Seems too many doctors are lazy. I learned more about my shoulder & how to help myself with exercises. Like everything else in this country, medical profession is not what it should be.
Summarised notes. You're welcome😊 Supraspinatu: Empy can test, forward flexion and 30 degree flexion then check power for supraspinatus, External rotation with power (IS n TM), Internal rotation lag sign, gebers lift off test (subscapularis), ROTATOR CUFF PATHOLOGY: Supraspinatus impingement: neer's test, Empty can test, Hawkins Kennedy test. Patients with supraspinatus impingement can have scapular movement asymmetry. Palpate the anterior joint line and internally rotate the shoulder and palpate subacromial/subdeltoid space. SPECIAL TESTS: FOR BICEPS: Speed's test ( resisted flexion), Yergason's test (resisted supination), papate the biceps tendon while doing this. ADHESIVE CAPSULITIS: painful ROM. Reduction in ROM both actively and passively. ER is the first movement to be affected in adhesive capsulitis. There's scapular motion asymmetry due to limited mobility of glenohumeral joint. AC JOINT DISEASE: Localised pain over AC joint, might pain when rolling over that joint to sleep. Palpate the joint for any pain, crepitus, step off deformity. Scarf test (cross body adduction) and palpate the AC joint. Painful arc in last 30 degrees of abduction. SHOULDER INSTABILITY: sulcus sign: pull down on humerus and notice for any sulcus on lateral side of shoulder. Apprehension and relocation test. LABRAL TEARS: Common in athletes. Speed's test. O'brien's test. Crank's test (internal and external rotation whilst palpating for pain and clunks) Acute lesions: anterior joint line fullness. Loss of power on resisted movements.
I used to watch this for ASMR... Then I injured my left shoulder 3 months ago. The speeds test for me was severe biceps tendinitis for a long time. Very frustrating. I'm still waiting for an Ortho appt with the VA. Meanwhile I bought Dr John Kirsch's book about the shoulder. I've followed his book to a T. Now I have no pain in any of these movements and my rotator cuff is stronger than it's ever been. I'm 60. I work out for 2 hours daily again with no shoulder pain. .
Did you take any medication for pain, how did you fix the issue, my pain is serve and I'm on pain killers and I'm really scared the pain doesn't allow me to use my arms at all ans any tiny moment really pain full
No imaging tests. Still working out but I stopped doing military presses out of caution. I hang from a bar for 10 minutes a day and do external rotations. I do upright rows, lateral and front raises along with rear felt work.
Try not to sound so much like a commercial next time. BTW, each doctor looks as an ailment for the perspective of his specialty. Ex. A neurologist will say one thing, and an orthopedic will say something else. Then there's the chiropractors who can fix everything with an adjustment. All can be right, or wrong.
This is the most clear and simple education and knowledge to understand, she is amazing , she dont talk too much and make the subject complicated like other videos , she go to the point directly, i would say on you tube she is a teacher number one. her charisma get your attention with that confident and voice level. been watching her video many times a day for one week. So many things that can go wrong... I have shoulder pain (probably from swimming) but none of these was hitting the pain spot exactly (AC joint was possibly closest). Guess it is time to see my physio and have an expert check it out! Really good to know what they are doing with those tests, though, thanks for the video.
Now I have finally found the correct issue with my shoulder, I'm currently going to Primary Health Care, but only doing stretching exercises for now. I have reason to believe they are just Milking the Alberta Health government until they decide that stretching exercises are not a good treatment, I have seen other videos that I need an MR Arthropathy done. What I know is that it is an AC joint issue, I have done this myself @7:28 she is doing the scarf test, my right shoulder I can do full arm out abduction, my left shoulder it jams @ 90° with acute to moderate pain. I forgot to mention if I do not pinch where the AC is I can lift my arm but at ~160° it starts to pinch at the AC also the tendon between the shoulder and chest muscle is sore to the touch. I just need to get the proper care instead of getting the run around until they figure out that there is more wrong than initially assessed. I can be sitting at my desk with my hand or in a fist position arm @ 90° and I feel pinching in between the shoulder and chest muscle(front) just near the top but just below the long bone part. The most part it sometimes feels like it's separating from the Glenoid Cavity (air pocket). I even went to Insight Medical Imaging, then My Doctor said there was nothing wrong (but there is) I have lost trust in Albert'a Health Care, it's like they don't care because I am over 51 and not working, so they are the ones paying.

The best video so far (saw about 5 of these) on diagnosing shoulder pain.

I'd like to have a doctor like that! Really accurate
