@@scotthowieson1191 Good evening! Yes indeed sir they did. Evidence of torn ligaments throughout the trapezius, old fractures in both hand bones (x-ray), my right leg was 1/4" shorter than the left, lower back pain cause by weakened abdominal muscles. I didn't realize how torn up I've been, still am actually, currently nursing yet another swollen lower back muscle. Was instructed to see massage therapist in addition to some unusual but effective exercises to be done daily. I see a massage therapist every 3 weeks now.
The worst was when the therapist asked me to resist his ONE arm with both my legs and my legs lost! He just casually subdued them didn't even feel like he was trying, said my abdominals had "turned off" and my lower back was doing most of the work. A bunch of other technical notes but following the exercises put me in a far less painful state.

I had a diagnosis by an orthopedist with an x-ray, an MRI and 2 months of physical therapy. My initial examination wasn't nearly this detailed and now I wish it were just to satisfy my interest in learning about it. Diagnosis was supraspinatus tendonitis, two small tears in the supraspinatus muscle, impingement of a bursa and osteoarthritis. Probably an anomaly (round shoulders). Still have some pain due to my stubborn refusal to stop working out and playing sports. I did have a good doctor .

    Summarised notes.
    You're welcome😊
Supraspinatu: Empy can test, forward flexion and 30 degree flexion then check power for supraspinatus,
External rotation with power (IS n TM),
Internal rotation lag sign, gebers lift off test (subscapularis),
ROTATOR CUFF PATHOLOGY: Supraspinatus impingement: neer's test,
Empty can test, Hawkins Kennedy test.
Patients with supraspinatus impingement can have scapular movement asymmetry.
Palpate the anterior joint line and internally rotate the shoulder and palpate subacromial/subdeltoid space.
SPECIAL TESTS: FOR BICEPS: Speed's test ( resisted flexion), Yergason's test (resisted supination), papate the biceps tendon while doing this.
ADHESIVE CAPSULITIS: painful ROM. Reduction in ROM both actively and passively. ER is the first movement to be affected in adhesive capsulitis. There's scapular motion asymmetry due to limited mobility of glenohumeral joint.
    AC JOINT DISEASE:
Localised pain over AC joint, might pain when rolling over that joint to sleep.
Palpate the joint for any pain, crepitus, step off deformity.
Scarf test (cross body adduction) and palpate the AC joint.
Painful arc in last 30 degrees of abduction.
SHOULDER INSTABILITY: sulcus sign: pull down on humerus and notice for any sulcus on lateral side of shoulder.
    Apprehension and relocation test.
    LABRAL TEARS: Common in athletes.
Speed's test. O'brien's test. Crank's test (internal and external rotation whilst palpating for pain and clunks)
Acute lesions: anterior joint line fullness.
Loss of power on resisted movements.

I used to watch this for ASMR... Then I injured my left shoulder 3 months ago. The speeds test for me was severe biceps tendinitis for a long time. Very frustrating. I'm still waiting for an Ortho appt with the VA. Meanwhile I bought Dr John Kirsch's book about the shoulder. I've followed his book to a T. Now I have no pain in any of these movements and my rotator cuff is stronger than it's ever been. I'm 60. I work out for 2 hours daily again with no shoulder pain. .

Try not to sound so much like a commercial next time. BTW, each doctor looks as an ailment for the perspective of his specialty. Ex. A neurologist will say one thing, and an orthopedic will say something else. Then there's the chiropractors who can fix everything with an adjustment. All can be right, or wrong.

Right on! My “doctor” was pathetic, 1 abduction movement; arm at 90 degree then push down. Conclusion: “you have a weak arm.” Really doc? I knew that, tell me what the problem is. “Let’s do an MRI.” What a scam. I’ve heard others complain also, so it’s not just me. Seems too many doctors are lazy. I learned more about my shoulder & how to help myself with exercises. Like everything else in this country, medical profession is not what it should be.

As someone trying to self-diagnose shoulder pain from over-training in the gym, it is staggering to learn how complex the shoulder and its various pathologies are. My symptoms are as follows: dull pain which gets worse on exertion/stretching, crunching/snapping sound when performing over-head exercises, no loss of range of movement in any direction, no loss of strength. I think it could just be mild tendon inflammation, but I think I need to book myself in with an expert :)

My shoulder pain has gone- and I don't see many follow up comments, so thought I would do one. If anyone has the above symptoms, I did the following:
      1) Shoulder/rotator cuff stretches everyday
      2) 1 week abstinence of upper-body gym work outs
      3) 1 week upper-body re-load (slowly reintroducing arm, chest, and then finally shoulder exercises)
      4) Occasionally taking naproxen before work outs

Similar story with me, injured both my shoulder and hip at some point in the gym and got a dull pain which flared up and clunking and catching in the shoulder and hip. Started studying Sports Therapy and got diagnosed with Labral Tears in my shoulder and hip, so is likely you have a labral tear too. Hope this helps!

My problem is similar, with mild pain and clicking sound from right shoulder during abduction (150°-180°). It's been one week since I started physiotherapy, it's a little better now.Lets see what will happen.I would recommend bob and brad channel for shoulder pain exercises.I am following those in addition to the ones physiotherapist gave me.

2 shoulder surgeries on my left shoulder in last 14 months. Be careful with whose grabbing your repaired shoulder. I had a young physical therapist who thought he could get my last 10% range of motion by jerking my shoulder aggressively. It popped the tendon and I had to have it repaired the second time.

No they don't do that. When pain occurs no more streching, no full movement to that direction.

Thank you very much. Should the Obrien's test not be performed with internal rotation? I have seen these demonstrated with internal rotation in numerous other videos.
