3D scanner straight to CAD if necessary, then to CNC machine for mass-production. I highly doubt anyone is hand-milling these like our Dads, maybe your Granddads, did back in the 60s.
They're a niche item. I'm getting into racing, they make a huge difference on track. In terms of everyday use, they kinda just exist to flex or add and extra layer of comfort and adjustability to your ride. A full rearset kit isn't worth for most people, but folding pegs are an absolute must for the inevitable low side that bends your foot lever out of commission... not speaking from experience on that last one...

My guess is because they have to be made specifically for each bike rather than be mass produced like a chain or tires
