  • Anterior cruciate ligament (ACL) tears are a common injury to the knee, especially among athletes. How can you tell if you have torn ACL or not? Today I wanted to share three easy tests I use as a physical therapist to assess the integrity of your ligament and tell whether or not you may have a tear.

    The four major ligaments that stabilize your knee joint are the anterior cruciate ligament (ACL), posterior cruciate ligament (PCL), medial collateral ligament (MCL) and lateral collateral ligament (LCL). These ligaments each serve to stabilize and support the knee in different directions.
    What is the ACL?
    The anterior cruciate ligament (ACL) is one of four major ligaments that hold your knee together. The ACL attaches from the front of your tibia to the back of your femur and is key in providing stability to the knee. Specifically the ACL limits the motion of forward translation (forward "sliding") of the tibia on the femur. It also serves to limit rotation of the tibia on the femur.

    How do you injure your ACL?
    Most injuries to the ACL are actually non-contact; no trauma from an outside force is involved. ACL injuries are typically sustained when your foot is planted and then you try to cut laterally or rotate. These motions apply maximum stress to the ACL and, with sufficiently high forces, can cause it to rupture. Another common cause is a blow/trauma to the outside of the knee when the foot is planted.
    What are the signs of an ACL injury?
    The most common signs of an ACL injury are
    - Hearing or feeling a "pop" in your knee
    - Joint effusion (swelling in the knee)
    - Bruising (depending on the severity of the rupture)
    - Joint tenderness
    - Instability (feeling like your knee could "give way" while standing, walking, going up/down stairs, etc).
    How can I check my ACL to see if it's injured?
    If you suspect you may have an ACL injury, you should go get it assessed by a medical professional (either a physical therapist or orthopedic doctor). These individuals are extensively trained in performing tests and measure to assess whether or not your ACL has been compromised.
    There are three tests you can perform at home with the help of a friend or family member that will give you a better idea as to whether or not your ACL has been injured.
    1. History - one of the major predictors of ACL injury is the history of the injury. As mentioned previously, ACL injuries are typically accompanied by a "pop" that is either heard or felt, swelling, bruising, joint tenderness, and instability. These injuries are often traumatic in nature and happen suddenly as opposed to gradually over time.
    2. How to perform Lachman's Test
    - Lay the subject on his/her back with his/her knee slightly bent (20 degrees).
    - Stabilize the person's femur with one hand and pull the tibia forward with the other.
    - If the ACL is intact, you should feel a "catch" at the end limiting forward motion of the tibia on the femur.
    3. How to perform an Anterior Drawer Test.
    - Lay the subject on his/her back with his/her knee bent to 90 degrees.
    - Sit lightly on the person's foot and wrap your fingers around to the back of his/her knee.
    - Pull the tibia forward towards you to assess how much motion there is.
    - If the ACL is intact, there should be very little motion with a "firm" end feel.
    - If there is an ACL injury, there might be excessive motion and an "empty" end feel (no catch limiting forwards translation.
    If any of these tests are positive, chances are you've injured your ACL. The next step would be to go see a physical therapist or orthopedic doctor/surgeon to discuss your injury and your options. Please don't hesitate in taking this important next step!

    Recently suffered a twisted knee during my soccer game. Planted my foot, and twisted. Didn't hear the *pop*, but felt bone and bone friction. Great swelling and immobility, of course. Took about 2 weeks to calm that down with anti-inflammatory. The doc did both tests and said "a bit of a give on the ACL". A few days later, my Physio did the same tests and more. Didn't notice any tear and passed all my ACL/MCL and LCL tests (except bending of the knee in full, really painful and not as flexible). 3 weeks after the first incident, I balanced on the leg trying to kick the ball (helping the team warm up), and the twist happened again. This time a lot more pain, down the outside of my shin to the outside of my ankle. Lasted 2 days. X-ray the following day indicated no bone damage and no torn ligament was suspected again. Swelling wasn't as bad. 3 weeks after that, when the knee felt good again, again another twist under the same circumstance (think I'd learn, right??). This time, the twist didn't cause a lot of pain, and it felt a lot less give. Swelling a bit, but nothing significant. Throughout this whole thing (today is 1 week from last incident), I've been able to jog and even sprint. All forward motions are good. Still afraid of sideways and backwards movement. Stairs are good too. Squatting isn't good, still. Swelling behind my knee noticed after doing my knee-strengthening routines. RICE does help. EVERYONE: get a reliable knee brace, don't baby your knee and do your knee strength routines. Take anti-inflammatory and ice it/rest it (RICE). The toughest part of the recovery is to gain trust in your knee again. Took me an entire season (didn't play) to regain trust in my ankle after a really bad sprain with interior bleeding (no surgery needed). Take your time. A sprained knee (which is most likely what I have, still waiting for an MRI) takes between 4-8 weeks, and then rehab. Everyone thinks I've "cracked" or partially torn my ACL. Something that heals by itself.
    PS: Second hardest thing is to not play, when you are the manager/player of the team :(

      LycanPaw that is extremely simplistic advice to what could be a complex issue. Go to the doctor, get tested, get an MRI. Often times simple physical tests don’t reveal the issues...other parts of the knee could be damaged... the ACL could be completely torn... you could further damage it with heavy use, etc.

      @@forzaaldi I'm in Canada, MRI take a long time if you go the free path (I did, as it's not a work/auto accident). Otherwise, it'd be around $2k. Anyway, I went to a physio that specializes soccer injuries. To her amazement, the injured knee was full of support and looked really healthy. It was the healthy knee (right one), that had quite a bit of ACL give. The injured knee passed all tests and exercises with flying colors. Despite telling her that the knee only shifts sideways when foot planted and possibly changing direction (I play defense, so like a squirrel, I change direction very fast), she still can't fathom why and how it's happening. I can squat really now, balance, side step, resistant band this and that, no issues. MRI appt is in late Jan/Feb 2020. Injury happened May 2019. Go figure, free healthcare lol. My only knee pain is on top/above the patella when really low squatting and on the inside portion of the knee , on the bone part...like getting a kick to shin bruise pain. Weird...but fascinating in a way.

      @@forzaaldi MRI disclosed partially torned ACL. Options were 1. wear brace for any physical activity, or 2. surgery to bring me back to previous level of activity (competitive soccer). No other damage to the knee. Not all ACL injuries lead to meniscus. The doc I saw is a knee surgeon, specialist in this, and teachers/runs own physio as well.

      @@LycanPaw You're lucky you didnt blow out your ACL completely. Even after 3 attempts lol. Did you have any feeling of locked knee? Where you can't bend or straighten it because something is in the way?

      @@charon_lix i can walk, and climb stairs and sprint without issues. Squats put a bit of strain on it. Side running and pivoting buckles my knee. I can live with a brace for when I'm playing. But since i can have the surgery and paid for, I'd rather live and play like i used to. I'm 31 lol. Surgery means 1month no driving, and 1yr no game time (physio and rehab). After, I'm good as new.

    So I had an injury to my knee several years ago. I had an MRI and was told it indicated a torn ACL. When I went to an Orthopedic surgeon, he evaluated me doing tests that I remember much being like this video and said he felt it was just a badly strained muscle and should heal with rest. It did get mostly better, but I will occasionally have some instability. In particular, if I take a step backwards and my heel on the side of the injury lands on something where my foot doesn't come all the way down, my knee will almost always shift and give. Should I have this reevaluated? If it was an ACL tear and has been left for several years, are there even repair options that could be considered?

    I wish, our doctors would know those methods. I have torn ACL in 1996. Reconstruction made in 2004. Before reconstruction they were always sure, that ACL is OK. After reconstruction the same. In fact, the ACL graft was torn again in May 2005 and during 14 years and tones of consultation they assured me that everything is fine :)

    Thanks.. I still play soccer and had a bad tackle 9 months ago and still suffering. MRI shows some tares to MCL and Meniscus. I developed a cyst and had it lanced and injections followed but after a couple of months the pain came back and I am not sure what to do next. Do I have an op or what?

    I twisted my knee on artificial turf around a month and a half ago and didn't allow it to fully heal, I'm inpatient and a month without football (soccer for Americans) with my team is just out of the question (It's bad I know)
    In the past I have recovered from rolled ankles and ligament damage fairly quickly and because this was the first time in 25+ years of football that I had twisted my knee I was totally un-educated in the rehabilitation side. I gave my self 3 weeks off and immediately started rehab, at'least what I assumed was good for it, icing, elevation, compression when needed and light walking/jogging in the pool.
    I went back way too early and was very weak on my knee but each week i could feel it getting stronger. The last 2 weeks my knee has felt totally fine/normal. I would use KT tape for a peace of mind and mild stability but 3 days ago twisted the knee on the exact same pitch, exact some patch and at almost the exact same game time (spooky) . So, the firs t time I did it there was no pop, no bruising but a small amount of swelling. The second time was the same, no pop, no bruising and a small amount of swelling. I know i have twisted my knee bad because it hurt like a son of a bitch for a good 1 minute and i removed my self from the game immediately...however I am not seeing/feeling the major symptoms of an ACL tear. I hurts across the back of the knee and just underneath the knee cap. Am I very lucky in that i may have just hyper extended it and not recovered enough? Fortunately its the last game of the season and I get to sit off it for 6 months.
    I'm 33 years old and the thought of possibly giving up football on the weekend is daunting and quite frankly has brought me down to my lowest since the second Injury.
    Its uncomfortable, bending my knee past 45' degrees is painful but i don't feel the "grinding" symptom or an inability to put weight on it...I can't stand on one leg with it fine.

    I recently broke my right leg. Small fracture in the fibula by the knee. I'm being told that I need an MRI to determine if ligaments or cartilage were involved. My symptoms are, popping, brushing, instability swelling and soreness

    Your video was very helpful I had an ACL injury in 2017 and had another one just 2 weeks ago. I was prescribed medication and asked to wear a knee brace for 2 months. I am over weight have a 9 months old son and hurt myself while dancing, now I am scared to ever dance again as I fear the pain and doing any further damage

    I once had a ligament tear 12 years back in my high school but didn't get any physiotherapy afterwards.. now I have gained much weight and on the same knee I experience pain. I have swelling on the right side of my left knee, I have so many sitting hours I experience pain when I lay my legs down for longer hours... Is there a solution to this?

    As per my MRI reports
    1. Mild knee Joint Effusion
    2. Linear Altered signals along anterior cruciate ligament intrasubstance tear grade (ii/iii) along the femoral attachment with slightly Altered morphological orientation of PCL,Hoffa's fat- pad Impingement and mild postero lateral capuslar Injury & lateral femoral cortical notch (Width 2-3mm)
    3. Grade i signals involving the posterior horn of medical meniscus
    4. Mild bone marrow oedema/contusion involving the tibial & femoral conyles
    Please suggest whether I require ACL reconstruction surgery of it can heal itself with the time

      I would try conservative physical therapy first. If you don't see any improvement after 2-4 weeks then it's time to consider the alternative (surgery) to correct this issue.

    After I hit my knee on the airbag, it had been swelling for 3 months with no pain.
    At first it was only painful when I put pressure on it to get in bed. The swelling is outside the knee cap on the inner side

    Hello there thank you so much for your help! About three maybe four months ago I put my granddaughter on my shoulders and I was doing a little jog went about 20 steps and felt a little pain in my knee. I put her down. And finish the run here. I am three months later And my knee catches, in other words, a little pain will shoot through it. I have to stop and hop - what do you think?

    Dr , early last week on Friday two weeks back I fell and i injured my knee bone ,so it looks so swollen cause i have been fallen multiple times but last week was so pain full and now it better .     Was playing basketball, completely healthy. Spun around and felt a pop (didn’t hear one) and mild pain. Only been a day but my knee is unstable and has an aching in the front right of my right knee and back of it. There wasn’t any excrutiating pain and I didn’t even jump. Should i be worried?

    I was having a lot of pain in the back of my knee, I had no swelling I did not hear a pop my knee was stable, I was not able to bend without pain and walking was painful as well. I went to my doctor he advised me to get a MRI , I did and was told I had a tier in the ACL. I went and had surgery to replace it with a donor I been going to therapy for
    (3 months) now, my knee is still swollen, the other complaint I have is that the knee is still stiff, even though I go to therapy 3 times a week, and I stretch and also doing light workouts. Way is that? Is the stiffness normal it never feels better. I also have issues now with my feet (plantar fasciitis) I was told my doctors (two of them) why did that happen? I never had that problem before , until after the ACL surgery.
    Please give me your option of that I am experiencing.
    FYI I am woman in her mid 50.

    Family doctor referred me to a specialist. I went to the hospital where a specialist saw me. Then he referred me to a surgeon who was running a physio clinic. Saw the surgeon at his clinic and then I went under the knife. One day surgery, in and out. Next day I was doing squats using my couch for assistance. Entire thing happening in Mississauga, ON, Canada. Dr. DiPasquale. An emerging top orthopedic in North America. I got lucky. Real lucky to have him. I went to my own physio clinic closer to home, but had regular visits at the hospital where in 10mins he told me I'm the fastest recovery he's seen. 4months I was running straight. 6 months I was able to play with brace. 8months I could've taken it off, my I was too afraid. At 9months or so, I damaged it and forced to play without it. No issues

    There are times where I feel and hear my knee pop but I have no pain, or swelling. I can still walk and run after. I guess it is just a bunch of close calls

    I use to wear hills a lot when I was younger. As I got older in my mid 20's I started gaining weight and didn't realize it until I put on some hills and tried to walk down stairs. I'm not sure if it's ACL but the pain is deep behind my knee caps and on the side of my knees. I can't squat any more. I lost weight now but the pain still comes back every now and then

    7 years ago I ran and twisted my right knee..I had an MRI and was told I snapped the hammstring ...my knee popped and the kneecap moved to the right side of my knee which I wasn't told..and I had a lump behind my knee and a bone protruding on the inside judt below the kneecap...I went for 3 weeks of therapy and weeks later my kneecap popped back in place..my knee swelled but didn't bruise..I couldn't stand..but as time went on it became better..recently however it's become stiff and painful and my leg is crooked..when I first jnjured it it hurt on the inside by the kneecaps..now it hurts all around the knee..My leg also aches above behind and in front if my leg..

    sir I had torn my acl but i can run even can sprint but sir as doctor said by seeing Mri that i am having complete acl tear . I have only problem in landing my leg straight. Sir please guide me

    Not sure if you can help since I can't afford a doctor to diagnose my issue, but thought I'd ask anyway. I've cleaned houses for 30 years, so a lot of up and down and working on my knees. I'm 58 was over weight for a lot of years, still 30 lbs over but working on it. I'm borderline diabetic and HBP w/ fluid retention. My knees appear to stay swollen and very stiff but now I can hardly bend them. Feels like they will break if I push them too far. Is there an exercise I can do to help? Thank you. Sherrie.

    Hi I tore my acl and I was recommended surgery. The problem is I'm only 14 and I'm afraid it will stunt my growth and/or I'll have uneven leg lengths. Thoughts?

      As long as your growth plate wasn't damaged in the injury that tore your ligament you have nothing to worry about. Good luck!

    Sir I got a knee injury on 21st of April evening while striking football my right feet clashes or collides with that of opponent player's feet , at that time I got the knee injury & was very painful in the knee & also develop swelling, so I took complete 2 days rest with move and a crap bandage applied on it and then after 2 days I I was like able to walk with more of strength used from the left left , so I joined the college for one week, the mild pain & complete recovery didn't happen so I visited the Dr, he took x ray and did some mechanical ligaments testing, said x ray it's fine no, fracture, Also major ligaments fine minor might be injured he said, so asked form complete 1 week bed rest, so now it's been 18 days, I didn't recovered, I still can't use my right leg normally, Just with help of left leg I walk.
    Also sir I tried your test for acl , I can bendy knee beyond 20° , I can't do the 90° test
    So sir is there chances of ACL injury & I need surgery..?

    i got my knee injury last nov. 29.. it popped.. up until now, i can't walk normally.. and my leg were like no muscles at all when i try to walk.. sometimes, the right part of the knee hurts up to my thigh.. it's almost 1 month 😭 but i can get it straight now 😐 its just, i can't walk just like i used to.. my thigh's too weak..

      my family made me walk everyday even though it hurts.. even made me look after our niece 😂 funny and ironic as it seems but i can walk perfectly fine now.. but i have difficulties walking up and down the stairs 😐 i still needed support..

    I had an ACL surgery replacement, the replacement was taken from my patella tendon and meniscus torn surgery all at the same time 10 years ago. I have never had any issues, but two days ago after 40 days of extreme and high impact workout it started hurting a little. Nothing popped but I do feel a little pain and it seems a little swallen. Should I be concerned? Give it a break and ice it? Any advice? Thank you

      I did my meniscus in and when I get bad knee pain I immediately pull back on my workouts and work my way back up more gradually. Magnesium/electrolytes helped me with my chronic pain too, if that helps!

    Hi there...i had an acl surgery about 4 months ago and now im so good but some pains are still with me....i cant run maximum like before but its ok...i have sometimes a pain under my kneecap and it happens when it has a little twist of knee.i jump about 1 month and its ok but i cant pivot quick stop quick moves and yet i cant have change in direction just straight ways...thanks for reading.and thanks doctor for your good videos.

    Thanks for the helpful tips but I had a knee injury a year ago from football and to this day it still makes a clicking noise when I kind of turn my knee. When I try to play on it, it feels like it’s gonna pop out again, I don’t know what to do for it to stop clicking or who to go to. Please help me out!!!

    My knee got twisted a month ago and it was currently recovering. when while I bend my knee to sit, I just heard some crack on the back of my knee and it hurts even more than what I felt the first time it got twisted..I began to make steps again after 3 days..but I still feel the pain whenever I force to walk..l haven't consulted a doctor yet and I'm also scared to go to get it massaged because I'm afraid that it was something else..what should I do?     I had a meniscus tear two years ago and upto date its still unstable although i can run even sprint , then two weeks ago i do not know what happened to my other knee its was painful felt like giving out it pops when i stretch it still i have maximum motion but a bit taught to the right side , would you know what myt be the problem or how i can test it wothout using my other damaged knee as reference?

    That’s exactly what I did yesterday when I twisted my leg to look round and it felt like an elastic band snapping in the side of my knee. The pain was excruciating and I couldn’t bare weight on it, people had to help me walk to my house. X-ray suggested either avl or pellegrini stieda. I’m having to walk with crutches, how long will it take to heal?

    I used to dance, but one day while performing twist movements I felt my knee is getting shivered. But I continued dancing for more 1/2hr. Later, I had severe pain in my left knee and I wore knee support. I massaged my knee and also went for traction treatment. However, those were temporary, then during this lock down period I consulted an orthopedic in my acquaintance. After hearing my problem, he told me that symptoms are of ligament tear which might be on first stage. But, still we need to go for MRI then only we can go ahead. Until, you wear knee support and do not fold your knee. As this lock down period is going on, I can't get my MRI done. What do I do? I feel crack noise when I used to straight my knee after bending.

      Hey I had the same problem and had the same symptoms, sh8vered feeling was at the bottom of the patella. After that i walked for some 10-20 kms and then in following two days my knee swelled and i was not even able to stand up. There is clicking noises sometimes when i bend or straighten. Couldnt see the doctor due to lockdown but had mri, tried to read it and it looks like there are high signals in the acl area. You must be having a grade 1 tear.

    Hi, about 2weeks an half ago I twisted my knee playing cricket. I was balling and as soon as I landed my foot on the ground my knee twisted i heard a pop sound and felt harshness between my bones
    Went to the doctor, no fracture and my x-ray was unmarkable.
    The doctor said i have damaged my tissues and I need to rest
    I still can't bend my knee without having too much pain or even straighten it up properly and i twisted it again which got me into worst pain ever
    Also I can walk and good with stairs but i limp
    Is it ligament injury?

    Hi, I’m 14 and I tore my acl, lcl, mcl, and meniscus. It was caused by contact during a basketball game. With intense physical therapy and surgery (which I already had) how long do you think that that would take to heal? This happened around two months ago

      Oh man! I'm sorry to hear about your knee, gabby! That's a BAD accident. Should be about 12-14 weeks to start jogging on it, 18-20 to start running and light jumping, 24+ to work into sprinting and cutting drills, up to a year before you completely trust it again. Ask your PT and ask your surgeon what their timeframes or "rehab protocol" is for this injury.

    I already had a double bundle surgery done for ACL... Is it possible for it to break again on a light - moderate contact during soccer/football?

      Unfortunately it doesn't ever heal as strong as it was before the injury. You're always going to be a little more prone to injury in that area. I would go get it looked at by a local professional for a diagnosis. Sorry I can't be of more assistance and good luck with your pain/problem!

    I hit my knee to a desk’s leg two weeks ago. No swelling, no bruise, no instability. However, it feels sore at times. It also is throbbing now having stood for over 4 hours a day. If it were a bone break, it would have been an aggressive reaction by the knee, wouldn’t it?

    As of now an mri shows I have a high grade partial tear. I am 5 weeks out from the injury. The doctor had done his physical exam 1x per week since injury, including Lachman exam and has noted the injured knee to be symmetrical to unijured knee. So far during pt I have done lateral jumping and agility ladder drills, without issue. So far my orthopedic doctor wants to try avoiding surgery. My line of work is a police officer, what’s your opinion?

      @@weixinvincent doing well thank you! Elected not to do the surgery. Just pt’d it. I’m back to work full time. Was out of work for approximately 2 months, don’t remember the exact amount of time. But, haven’t had any issues since returning. I can run and lift without the knee giving me any issues. It does get sore pretty easily. The only time I really have any pain in it is if I hyper extend it at all. Hopefully it stays that way for a while, but I’m sure surgery is going to be the only option down the road

    1> History :
    I had the kind of injury that you mentioned @ 2:22 . where someone hit my left knee form the lateral side and i twisted and bent inwards. AND I HEARD AND FELT POP. And for the next few seconds didnt feel my foreleg at all. after that i was fine an running again becoz body was warmed up . its been 2 weeks since. and now i cant bent my knee fully
    2> Lachman:
    There is no back and forth loose movement of the tibia. left injured knee is as tightened as right healthy knee. but this test causes NO pain in the ACL region. I guess multiple ligaments injured.
    3> Anterior Drawer:
    Back and Forth movement is tightened as the health legs and causes no pain in ACL region.
    Only Medial collateral ligament hurts.
    What should be my next steps ?

      Its still sometimes. hurts from posterior tendon. (form outside .back of the knee).
      Its never the same. But i think its more about losing confidence. i never tried hititng the gym and train with a lil load on it. i still think if i do i will be able to fully recover and the muscle will be better than ever.
      Theres also a chance that the tendon has retached in a slightly wrong position that causes the pain. but yes i had played footbal and ran a lot for other games and in those i never felt any pain which gives me confidence that i m ok.

    This video is extremely old but I have a left knee injury 6 years after a meniscal tear in the right… I thought it felt identical to my previous injury but now I’m not entirely sure. Do you have any videos explaining any differences? It happened the same way but now a few weeks later it no longer feels the same. I have a surgeon I’m working with but I’m quite anxious and knowledge is power!

    I don't know if it's a tear or no but my feet has gone flat when I plant it on floor after the knee injury from a quad strengthening stretch.
    So I doubt was it a sprain or tear. Knee is extremely wobilly.
    I had locked knee for a couple of dys N that's it.
    So what do I do if theres no pain N lockness at all but I'm not able to walk properly due to instability in the knee as if it seems there's no grip at all.
    N it is hyperextending. Even i feel like my leg muscles are shrunken a lot.

    Awhile ago I fell over a pallet backwards and my leg went one way and my left knee went the other way. I heard a loud pop in my knee and it felt like it wanted to give out. I can’t ice it it burns too bad and anything warm turns it fire engine red. I have tried everything for it and works. I suspect I may have torn my ACL. I’m not sure if I swell up or not. I haven’t had it looked at my insurance won’t cover me. What is your advice to me?

    Hello, i dont have bruise, and my swelling is not big, but i have difficulty walking (i limp), and i have difficulty flexing my leg, my pain in the knee is more on the right side, and I can't seat on my knees, its too painful, btw my injury happened during wrestling, but i dont remember if i hear a pop or not.
    Based on these informations and your experience, could you tell me if it's most likely that I have ACL or not ?

    Hi doc i really need help here , I have swelling on the inside of my knee and pain when pressure is put on the knee ,but when my doc advised an MRI the MRI showed that there is no ligament tear,and all the ligament are intact ,but I am an athelete and i really need to return to playing my doc said I rest my knee for a month and see what happens ,but he also said even though the MRI report said there's no tear he said he suspects there is a little very little tear / streched ACL ,no I'm very concerned about it , whether and when Ican i return to sports
