How To Test & Heal A Torn ACL Without Surgery

    ACL (anterior cruciate ligament) injuries are all too often sent for surgery when most have a really good chance of healing 100% naturally.
    They can even get back to being active with sports and running if they choose!
    In this video, we cover 3 things to check on your knee for an ACL tear, then we explain all the different types of ACL tears and how to begin to treat an ACL injury naturally.
    A scary statistic you need to be aware of if you’re planning to get an MRI (or if you already have), is that one study reported over 40% of ACL tears found on an MRI can be wrong!
    That means the MRI showed a torn ACL but when the surgeon went in to repair it, there was no tear. It might have healed or it might have never been there...either way the surgery was not needed in those poor patients.
    There are endless ACL rehab exercises out there and nothing special about a single one that will help you most. What is most important is getting the right kind of stability so that your ACL can heal.
    That’s why we recommend getting a hard hinged knee brace on as soon as possible so that you can give your ACL some relief.

    You just described my pain to the tee.Hurt knee 8 years ago had surgery Dr said I had torn cartilage and that I would probably have to have knee replacement but I refused .About 3 weeks ago of writing this the pain swelling and popping of knee started to lessen with each day .I researched the area of pain and symptoms that I’ve been having.Your website popped up and let me say : You nailed it!Your description of an injured mcl is spot on to what I’ve been experiencing since 2015.Now I know I just wish I would have researched this before .

    My husband was clipped on December 25 by a wild snowboarder while skiing. He could barely walk for months with the swelling but now there is less clicking/popping and now just more achy from time to time. Walking for extended periods was hard. I’ve given him glut strengthening exercises and gentle shallow squats. He had a McDavid hinged knee brace in the early months but stopped wearing after a few months as he started feeling much better. He is much better but his MRI at the end of May was read as a completely torn ACL and both meniscal torn and MCL partial tear. But I was the only one as a chiro who did the basic orthopedic tests and the anterior drawer test was NOT positive. However, medial gapping test was hugely unstable. So I was stunned when the results in MRI complete ACL tear were read. Up to 40% are read wrong? I think it is MCL because the goalie squat was painful for him but is not anymore.
    So are you saying he should wear this other brace you recommend? Or just continue with the McDavid brace? All the time? He is much better as the knee only aches from time to time. I’ve been advising him to fix the muscle imbalance and strengthen the gluts. Standing in sand on the beach makes him feel unstable. They recommended orthopedic consult for ACL reconstruction and a custom ACL brace but so far two Orthopedics have turned him down.
    Should we just get this brace to recommend at this point? How long should he wear it for? I talked to him about doing your online knee rehab plan since it would clearly give him a guided program to help him fix his muscle imbalance. He has a history of loving biking and leg press and always had strong quads. That’s why he likely injured a meniscus many years ago. Sadly this was a freak injury on the ski hill.

      There’s still a possibility your acl is torn even if your leg is improving (something else could’ve caused that pop though) because my acl was fully torn for 10 months and I didn’t notice because after the incident that tore it (I didn’t know it was torn at the time) my leg improved back to the point where I could run full speed, so I’d get an mri if I were you im saying this so incase it’s torn you can catch it early

    I fell out of a trampoline at defy 10 feet up in the air . I looked down and the trampoline was gone and I stopped my fall with my right leg. My leg completely came out of my knee. I was lucky when I wiggled my leg for it to go Back in place. I was in horrible pain. My 2 year-old asked me if I was OK. Never have I heard her ask me that. My heart was broken. My first thought was how am I going to feed her, how am I going to go grocery shopping ,cook , wipe her butt etc.. my life is over. . Anyways, so finally, the pain was much better so I figured I’d get up and walk. Unfortunately my leg completely gave out and I fell to the ground. Instantly I knew my life has completely changed forever. I’m waiting to see my Orthopedic in a few days. In the meantime, I want to see my options. I really enjoyed this video , I very much appreciate you taking the time to make this . According to the ER, they think that my ACL and my PCL is gone. I don’t have any pain with my leg gives out but I feel the bones hitting each other and that feels crazy and I just feel like someone kicked me in the knee but the pain goes away. My foot also turns blue when I am on my crutches, so I remain with my leg up on my wheelchair. Would there be a chance for me to give this a try you think? I cannot put my foot down on the floor or else my leg will give out and he goes sideways not front ways..

    *Minimal knee joint effusion.*
    *Grade I tear - Posterior horn of medial meniscus and lateral meniscus.*
    *Posterior horn of medial meniscus and lateral meniscus.*
    *Anterior cruciate ligament-Partial Tear.*
    *Posterior cruciate ligament - Strain.*
    I shown these results to an Ortho Doctor and he suggested for Arthroscopy as next step..
    How severe is my injury? and also trying to get a knee brace for myself after seeing this video.

    I have a recent full ACL tear possible meniscus damage as well. This sounds great at least worth a try and the orthopedists I spoke with did advocate at least for a middle ager a wait and see approach as you mentioned, at least 2 months (which thanks to your vid is likely not enough time to know if it will heal on its own) then consider surgical reconstruction with hamstring tendon. However my concern, there was a study done by HSS in NY, that showed those who did not get ACL reconstructive surgery were much more likely to suffer from osteoarthritis 7 years after injury than those that got the surgery. Is this something you have seen at your practice or are aware of?

    • @epmanualphysicaltherapy
      @epmanualphysicaltherapy  2 ปีที่แล้ว +9

      Hey cardo, sorry to hear that you've got an ACL tear :-(
      Yes, we've heard about that study. There are more like it. We have to question what kind of health and strength the people from the study had. We don't know what they did for all those years leading up to getting knee arthritis. The assumption we work under is that they kept doing exercise or activity that fed into a muscle imbalance. Muscle imbalances around the knee (over-dominant quads and under-dominant glutes) is what sets up many chronic knee problems, including knee arthritis. That coupled with the loss of stability from the torn ACL can definitely expedite knee arthritis.
      But what if you correct your muscle imbalance and maintain it?
      That has not been studied. In fact muscle imbalances have not been studied. We see people here in the clinic do very well after an ACL tear. It's up to them to keep up their strength for the long term.

    Thanks for this video! I am in category 3 and torn my front ligament completely, along with both meniscus getting injured and a bone bruise condition in my thigh bone. I started early rehabilitation training in form of weight lifting etc. with great success and I can run and lift fairly in comparison of before my injury, even playing some soccer 4 times after the injury. My biggest issue is that my knee has been sliding out of position about 12 times in 3 months. But it does not happen while I am doing something that requires more force like training and so forth, but when taking a quick step in the opposite direction boom there we have it. So I find it difficult to make a decision about wether surgery or no surgery is right. My concern with no surgery is that my knee might get new injury or get worn out by continues slides of the bones. But I have not been wearing any form of knee support as of so far. I will try that and give my knee more time to recover before I make a decision. Thank you for making me realize this just now. 🎉

      Ive had acl reconstruction on both knees, most recently 2 months ago with a partial meniscectomy. Its not easy and the recovery takes time and dedication. Based on your comment if you're knee is buckling regularly you are risking further damage to the other ligaments and meniscus. Living with a torn acl requires a lifestyle change for most people, soccer and other pivoting activities are risking more serious damage. Further damage can cause osteoarthritis down the road and result in a knee replacement. I opted to have surgery both times because I do not plan on living my life in a straight line. Take some time for your decision but please be careful in the meantime.

    Good video, gives me hope to heal my ACL. I did a complete ACL tear playing soccer 7 weeks ago when I hyperextended my knee. I found after 3 weeks it felt pretty good, so I played again and did a minor hyperextension. Took a week off and bought a brace like you have there and played 4 games since. Knee feels stable in brace. Did an online session with Jun Matsumoto today, he has had 100% success with healing ACL tears (16/16) with his exercises. The focus is bringing your heal to your hip to shorten the length of the ACL so it can reattach. He wants me to quit soccer for 6 months, tough ask. I will do the exercises and see how I go. I don't see the benefit of the knee brace when not playing sport as my knee is very stable.

    Thanks for such a wonderful explanation. I met with a bike accident and my acl got tared .Then I underwent ACL reconstruction surgery 6 years back. A month ago I fell down and my current MRI states that " diffused thickening and increased signal in the ACL" - diffuse increased signal intensity secondary to mucoid degeneration. One of the doctor suggested for the surgery again. But another suggested to proceed without surgery. Does this MRI statement meant partial tear in ACL?

    • @epmanualphysicaltherapy
      @epmanualphysicaltherapy  ปีที่แล้ว

      Hey Subha, sorry to hear that you're dealing with this knee ACL problem :-(
      If the MRI report did not explicitly read that there is a tear, even a partial tear, then there is probably not a tear. "Diffuse thickening," "Increased signal," and "mucoid degeneration" are not the same thing as a tear. Those problems are things that could eventually become a tear, but you probably don't have a tear yet.
      This sounds like you do not need surgery and you need to address the root problem so that you don't end up having a tear. You should check out our library of ACL videos if you haven't already. You can see it here:

    How, exactly, does an acl tear heal over time without surgery? Do the torn parts grow back together somehow? Does new new ligament grow? And exactly how does exercise help with that process? Did it tear because muscles were weak? Does strengthening muscles hold the knee in place so the ligament can grow back? Is there scar tissue? Does that make the healed ligament weak and vulnerable to a tear in the future?
    Sorry for so many questions. All I see is videos on how tears happen and how to do rehab.

    I broke my left ankle a week before Memorial Day 2021. A week later, I tore my right knee's medial meniscus due to falling while on crutches. Fast forward several months of rehabbing the knee on my own at home and having the full range of motion, but not wearing a brace all the time. It was doing ok, but it felt pretty tight and swollen most of the time. Thanksgiving came around, I stepped off a footstool reaching for a dish on a high shelf. Came down on the right leg and felt searing pain. I could barely hobble to a chair. My knee was very unstable. I finally got in to work with a PT in March of this year and worked on the rehab exercises, purchased the ShockDoctor brace you link to - it is part of my daily clothing now. I don't go out without a decent brace now. I also got their highest-end brace because my goal is to go backpacking again with moderate miles and a fully loaded pack.

I'm confused because I'm not having pain & for the most part I've gain most of my mobility not 100% but I'm very close to it (injured 02/09/2022). Is this something that is considered normal for a complete ruptured ACL? Keep in mind right after I injured my knee I iced it, put a brace(still wearing it) & have continued to elevate it.

    • @epmanualphysicaltherapy
      @epmanualphysicaltherapy  2 ปีที่แล้ว

      Hey Haydee! Sorry to hear that you've got a complete ACL rupture :-(
      But that is great news that you've got nearly all your motion and no pain!
      That means that you have a chance at either healing the ACL or at least compensating with strength to go on without surgery. This does happen to a lot of people with ACL tears, but usually surgeons push patients right into surgery. Keep going with the brace and start on the recommended exercises. You might even find a local specialist to guide you on healing without surgery.
      The best case scenario is that you recover adequately and get back to normal again. Worse case scenario is that you get strong and put yourself in the best position to have surgery and recover as quickly as possible after you have surgery. There is no rush if you're feeling good and improving.

I went to the Emergency ortho near my house - they performed an x-ray and tests. It was a PA but she said I sprained my knee and to wear a brace. I told her my knee buckled walking into the garage step about an hour after injury. I injured my knee at 8am and then went to the orthopedic PA emergency clinic at 2pm. The instability happened at 9am after the injury. Question…she said for me to come back in 6 weeks if not feeling better. She had me jog in place diluting examination and then did some tests. But it’s been 2 weeks and I still don’t feel 100% confidence in it. Wondering if it is an acl or like a grade 2 mcl. I definitely feel pain on the inside portion of my right knee. I had worked all day on my farm planting trees on steep ground in the late afternoon I turned, fell over backwards to what sounded like a gun shot. My left leg was buckled inside towards my right leg and patella stuck on the outside of the joint. I picked up my lower leg lining it up with the thigh bone and thumped the outside of the patella to get it back in place. My thigh instantly ballooned out and I couldn't bend my leg. 20 minutes late I went into shock, I was by myself on our isolated property. With intense pain I stood up collected my chainsaw loaded the 4wd and took 5 minutes to get into the vehicle. When I went to the doctor he didn't believe I could have done it. The orthopaedic almost ripped my knee joint apart and decided I may have dislocated my knee. Intensive physio had me walking after a couple of months. After 4 months the orthopaedic operated, cut my quads off the patella taking out scar tissue and pieces of meniscus. He then said it was my fault the quad was wasted/detached. 3 more months of intensive physio and had to see another surgeon. Splinters of bone were growing into the joint under the patella which was shaved, bone chios and more broken meniscus was taken out. Not one dr told me anything about the injury which I am sure was acl damage as well as torn quadriceps.

    A torn ACLs doesn't heal by itself as it is inside the knee. There are scaring processes that can stabilise the knee somewhat, also a strong muscle base helps, but by no means can those processes replace or compensate significantly for a missing ACL. Most people also have accompanying knee injuries such as meniscus damage that needs to be surgically adressed anyways if you want to avoid osteoartritis in a few years.

    Long story short, I went to a MRI, new injury was a cracking of lateral menisc.
    So to conclude my acl is on 60% and medial and lateral menisc are cracked withou any movment, I've got full motion of my knee, havent any pain but, some funny felling is ther. Actualy I run today with change of diraction and pu some realy high pressure on my knee it felt good, in gym I can do everything without any problem.

    hi doc,
    i had an acl injury while playing football about 7-8 months back, and i did an MRI and the doctor confirmed it as an acl injury. for a 2-3 days it hurted a lot i could not bend my knee but after a few months without using any knee brace i can walk, i can run on my treadmill and i can even bend my knee, the only activity i didn't do is play football. while i was watching your video i tried your test and i could do it with ease, so doc what should i do now. should i wear a knee brace going forward or should i frequently do exercise or should i consult a doctor and get an MRI again. i just want to go back and play football but i am scared to do it because of my knee. please reply to this comment

      I would not play football any more but start coaching. An ACL tear I really believe your knee will never be stable enough to risk reinjury. I had complete reconstruction of Mt ACL and it's only good enough to live a normal life. I hope you're ok

    I tore my acl and decided to not get the surgery and almost 4 months into it my knee starts to feel better it’s more flexible and I was playing basketball I stopped with the leg I tore my acl on and it popped out of place and the pain won’t go away and now I finally decided to get the surgery so it doesn’t pop out of place again.

    I still struggle with pain with muscle strength. Either way its growing arthritis with or without surgery!

    About twenty five years ago, I fell on the on the ground and received a severe internal injury on my my left knee. I consulted many doctors. They prescribed pain killers and gels but a little betterment in pain. So, I decided to do MRI by myself which showed fanning of the fibres of ACL of left knee. Moderate sprain and partial tear in ACL. I can walk easily but I cannot run. What will be its treatment?

      Bruh. Get a medical card, strengthen your leg muscles for now, invest in a spin bike like yesoul m3 , then get a surgery in 2 years..or if you can't wait get a surgery at a cost of rm30k or around 12k at a government hospital
