    Nice to find a Dr who gets it! I've lost 45lbs but often get stuck at various weights. What I'm doing now that's working exceptionally is warrior 20 hour fast, mostly high protein, low carb, lower fat and using berberine. I'm also using niacin+sauna, lifting weights a lot, walking twice a day and often Rucking with 45lb. Losing 2+ lbs each week easily.

    Dr Stirrett, something I'd love to hear your comments about: starvation mode. Is it a thing? Is it not a thing? If it's NOT a thing, any idea why weight loss may stall when eating too FEW calories, at least over a short term? Even when focusing on macros I've noticed that my most successful weeks (I AM on sema) are the week when I actually manage to get a couple meals in.

      To answer your question, watch Naked & Afraid. They basically starve themselves in the name of "survival." You can see them transform over every episode. They lose 25-40 pounds in 3-4 weeks. There is no such thing as starvation mode. If you aren't eating you will lose weight. What usually happens is people "think" they are starving themselves, but they aren't counting the calories properly. They misread serving sizes on packages. They forget to factor in the cooking oils and condiments. When they do eat, they unhealthy foods that cause temporary bloat. Etc etc. Now if you are actually starving yourself, you will lose muscle with fat, and that's counterintuitive for long term. The more muscle mass you have the more calories you burn even at rest.
      But long story short, if you count your calories properly, eat a high protein diet, and lift weights regularly, you will lost weight. Cardio is not needed at all.
      And if you're wondering who this random stranger is on TH-cam, I cut from 265 down to 190 in 10 months. Saw abs for the first time in my life. I did it from lifting weights 4-5 times a week, keeping protein high as possible, and ate in a 400-500 caloric deficit. The only cardio I did was walking my dogs...

      @@SouthernDrinkSlinger good on ya and congrats on the life change! I'm down from 230 to 190 in 4 months and going to the gym 3-4 times a week myself: would love to see an ab, though my real goal is to be able to do but a single pull-up lol
      As for the rest, I have watched the show but I have to believe there's more to it than that - simply because I can attest to the fact that when I started on sema, I would go days at a time without eating anything but water: I just couldn't put it in. At best I'd get a single protein shake down at some point, or a lone banana...we're talking sub 300 calorie range. I was taking supplements to ensure I didn't suffer from malnutrition...but yeah...at 300 calories intake a day, walking 5000 steps, I should have been dropping MUCH weight (as I recall I think I read you need a 500 calorie deficit to lose half a pound - I was probably closer to a 900 calorie deficit PER DAY).
      Everybody's body is different - which is why I like Dr. S' take on things. If all it required were less calories, he wouldn't be saying "calories in/calories out doesn't work" - and I've definitely found that to be true.
      Again , congrats on the amazing turn around - so glad you were able to find something that works for you and I hope, hope, HOPE I can have similar results when all is said and done (my goal weight is around 170, though my doctor has pushed me to think a little higher...we shall see how it goes)! And thank you for the insight from your experience!

      You stop losing weight when you reduce your food intake too drastically, because your metabolism slows down. It is not so much "starvation mode" as "survival mode." A slower metabolism does not need as much fuel. Dr Jason Fung has a very good video about this concept on his channel.

    My fasting insulin is 29 and my A1c 5.5% I’m not diabetic but overweight. I’m nervous about taking a gap-1 can I take metformin instead?

      Try good plain Greek yoghurt it's full of prebiotics and high protein

    I can attest to CICO not working. I’ve cut carbs, keto, down to OMAD. My insulin sensitivity has increased significantly and my inflammation is way down. 30 lbs gone. I did try a longer fast 48 hours ending today and had a syncopal episode at the grocery store and almost fainted over by the organic strawberries. 😂 😅I came home and ate but felt bad all day. I think I wont be doing a longer fast for a while.

      I used to do OMAD for over a year. I was averaging 900 calories a day. I loved OMAD and I felt good and I was never hungry. However, I still had weight to lose and I had stopped losing. So I started trying to eat every other day. I ended up with very low blood sugar. I stoped doing OMAD and gained all the weight back. Now I can’t seem to lose no matter what.

      When you do longer fasts you MUST get extra salt and other electrolytes. Lacking those will cause a drop in blood pressure resulting in fainting.

    I’ve intermittent fasted for 90 days eating one meal a day and I didn’t lose any weight. Can you possibly give me some reasons why

      I’m not sure how old you are, but I’m over 44. I also tried OMAD for 2 months and didn’t lose weight. I started watching a few videos on TH-cam for IF for women over 40lbs. Especially premenopausal women. I then changed my IF window from OMaD to 6-7 hrs eating window and watched my carbs. I didn’t do keto , but I did watch my carbs and didn’t eat the processed foods. I lost 13lbs in 2 months. I’ve been losing since . The omad may be too much on your body, for various reasons. I also increased my protein. I ate things such as quinoa, salmon, veggies. Etc. Omad doesn’t work for everyone. Also, don’t think about “I need to lose weight. I can’t lose weight “ .. once I stopped thinking about losing weight, I started losing! I just focused on my eating window, ate when I was hungry, and started thinking , “if I lose, I lose. If not, at least I know I’m eating healthier than I was months prior.
      I think the stress of thinking of weight loss all the time, also causes people to keep weight on, maybe it’s stress? I don’t know- I’m not an expert- but just my thoughts and experience. Hopes this helps !😊

      @@KeCee1975 hi , thank you for your response, I am 66 & as I was thinking about it I think OMAD does not work for me. I cut my carbs but I didn’t exercise & maybe my body needs more movement. I do need to increase my protein as well because I’m not getting enough of that but if your eating one meal a day how in the world can I get in 80-90gms of protein for the day?. 18-20 hour fast didn’t work for me so I will do 16/8, increase my protein and walk or exercise 3 days a week and see what happens.       Loads of research out there which indicates that fasting works for weight loss by cutting calories. End of. As a Medical Doctor and PhD, I am not against any method which works to help you fight obesity ..... But, the big problem is 98% of people lose weight and then put it back on. The best method is a healthy diet that you can stay on permanently. Boring, I know, but a high protein version of Mediterranean is slow but very effective.

    What do you think of intermittent fasting or fasting in general with GLP medication? My carbs are usually under 30.

      Hey! Great question. I will do a video on this for you. In general I am not a big fan of IMF with semaglutide due to blood sugar stability and balance throughout the day being improved with high protein meals. But, that is not an absolute. I have had a number of patients do quite well with IMF while on GLP-1's. I think it has more to do with your cortisol and adrenal function at the end of the day.

      @@dr.stirrettI’ve asked you 1:26 this question on a video about GLP1s if because they make it hard to eat will that increase cortisol? And if someone has high cortisol is this soemthing to be aware of? Or if you are losing weight on the GLP1s then this proves cortisol is ok? Just so curious , because as someone with a cortisol belly and adrenal fatigue I know that fasting for me has to be planned and there are nuances to iMF. Like women should break fast earlier in the day than men.. cortisol is higher etc. please advise

    If you eat 1000 calories over what you need of the best food known to man you will gain weight. Calories matter.
