Technique
Multiecho & multiplanar sequences are acquired through lumbar spine with I/V contrast. Images were reviewed on Osirix work station
Report
Loss of intervening disc height with abnormal endplate signal intensity involving L3-L4 level is appreciated returning T1W low and T2W high signal intensity. Thickening of the posterior vertebral soft tissues along with anterior peri-dural subdural thickening demonstrating right foraminal encroachment at L3 and bilateral lateral recesses at L4-L5 level. Edema is appreciated as T2W/STIR high signals in bilateral paravertebral soft tissues. Multilevel disc dessicatory changes noted.
No other disc bulge or protrusion is appreciated. Dural sleeve is well capacious. Conus medullaris is terminating anatomically.
CONCLUSION-
Loss of intervening disc height with contiguous involvement of the vertebrae at L3-L4 level and thickening of the prevertebral and paravertebral soft tissue with anterior peridural subdural thickening likely signifying spondylodiscitis may be inflammatory or caries as described.
