  • Pityrosporum or Malassezia folliculitis is a condition that is probably under-diagnosed. It is mistaken for normal acne, but has different clinical features and the treatment is distinct too. This video looks at this entity including its clinical features, predisposing factors, and management.

    I had this for two years before figuring out what it was. My whole face was covered and at 51, that’s not ok. What worked for me was fluconizole 200mg daily for 7 days along with ketoconizole shampoo. It still didn’t go away, so I used a sulfur face wash every other evening, followed by benzoyl peroxide cream. On the off nights I used a ketoconizole lotion at night and a mix of apple cider vinegar and 3 parts water in the morning. I believe the apple cider vinegar, benzoyl peroxide and sulfur wash were what really helped turn the corner. I also avoided oil face washes and sunscreen. I switched to oil free sunscreen, foundation and vitamin C serum to clear up hyperpigmentation. It took about a month, but after suffering for two straight years not knowing what was going on, I’m thankful it’s gone. The scary part is knowing it could come back since I’m in a tropical region in Florida. I know now to shower immediately after working out, outdoor sports and any time I’m sweating. Fungal acne thrives on sweat and oil! ❤

      All the points you mention are perfectly right! Sometimes we use low dose Isotretinoin to treat it as well. It may need to be continued long term though Another natural option to isotretinoin is oral vitamin A. Here is a video on it. We may be able to use lower doses than that used for acne.

      I was wondering what exactly is progesterone folliculitis and how does it relate to Malassezia folliculitis? You mentioned it with isotretinoin. Thx :)

    Hello dr,Im 21 year old student and I had scalp folliculitis about 3 years and i consulted by 3 dermatologist. Last time I had doxycycline for 30 days.Two months there is no scalp folliculitis and i had fever and throat infection and reappear the folliculitis and things get worsen. Not only me there is so much people suffering from scalp folliculitis about years. There is permanent solution dr? And why difficult to treat scalp folliculitis?

      There are multiple reasons for folliculitis and that is the reason why it is hard to eradicate. For example, in pitysosporum folliculitis, the yeast lives in our skin and therefore eradication is not possible. The treatment options mentioned in the video can be tried. Another over the counter option is to consider Vitamin A as it mimics the action of Isotretinoin. It will need to be continued long term though. Here is a video on how it is useful in acne, but the principles mentioned are the same for folliculitis too.

      @@DiscoverDermatology thank you doctor for valuable reply. The ketoconzole , salicylic and sebo wash is not working on my scalp.Last time doctor prescribed betadine ad shampoo and it gives little bit relief. Ok , I'm going to see a good dermatologist for a longer treatment

      Hope you find a treatment that helps. Low dose isotretinoin can also be used for folliculitis. Here is a video that explains how it works in a similar condition called seborrheic eczema. Isotretinoin may help in folliculitis due to various causes

    5:20 i dont know much about this, but should i buy medicine with this ingredient(itraconazole) or should i go to the derm?

    TY, nurse diagnosed me with eczema and bacteria. She gave me steroids and anti biotics which made it worse. So itchy!

      This is a commonly missed diagnosis. As you mention, steroids and antibiotics only worsen the situation.

      @@DiscoverDermatology Going to dermatologist in an hour! Wondering if scabies from the cheap Chinese Foot massage places? (I am a woman, 57 year old in Connecticut). How itchy it Malassezia? This is unbearable sometimes. Mostly at night.

      Malassezia tends to be only mild itchy. Its v unusual to have the symptoms you describe. Hopefully your dermatologist has been able to diagnose your skin condition. @@lf9341

      My friend's retired cousin who I guess is one of the best Dermatologists in the country wanted me to ask her if it could be this. I insisted on a biopsy.
      Prurigo nodularis is a chronic skin disorder characterized by the presence of hard, extremely itchy bumps known as nodules. Though the cause of the condition is unknown, the nodules are the result of persistent, intense scratching and rubbing of the skin. They tend to be found in easy-to-scratch areas such as the arms, legs, abdomen, and the upper and lower back. Difficult to reach places like the upper portion of the middle back are usually spared, and nodules generally do not form on the palms or soles of the feet, or face.
      THIS is Exactly what I feel it is.

    Sir i have suffered from Malizalisa Foliculities on face 3 months ago again I'm getting small small red pimples and rouph skin for touching and whiteheds and some icchiness and i have drandraff also..doctor prescribed me muppirocin ointment ladt time also this time also for one month for face.. My skin burning itching while using muppirocin ointment..i need to get rid of this problem please help me sir what ingredient will stop this problem from coming again and again is salicylic acid is good or glycolic acid or retinol which will help me to get rid of this infection

      I think acids can irritate the skin further in M folliculitis. The treatments suggested in the video, including anti yeast tablets like Fluconazole may be an option. Low dose Isotretinoin could also be considered.

    Hello, i wanted to ask if i use itraconazole will it cure my folliculitis permanently?

      It is unlikely that any treatment will cure this condition as it is caused by a yeast that lives ion our skin. We can control the condition, but cure is something that occurs on its own with time.

      There is no cure for it as the yeast lives in our body. We can use low dose Isotretinoin but it needs to be continued long term. Here is a video on isotretinoin used for a similar condition called seborrhoeic eczema that is caused by the same yeast.

    That's amazing info it's very helpful.....i hv this issue since Manu year the infection will b more sometimes and sometimes it feels it's going...but the issue with me is the marks left behind on my back and chest...can u plzzz suggest a lotion or any cream for marks🙏

      The marks take time to heal in Asian skin. Usually, time is the best healer. There are a few creams used for facial pigmentation that you could try. The ones mentioned in the latter part of this video are over the counter and may be beneficial. However, time is the most important factor in the dark patches settling in Asian skin.

    hellow sir. really very much informative video. if its possible may i share my prescription with you in any way? im suffering with seborrheic folliculitis and two doctors precribed me completely different medications involved both topical steroids and antibiotics. im so worried and confused. looking for a guideline

      @@sayemdewan5030 The first covers a bacterial in section whilst the second script covers both bacterial and fungal infections. So the diagnosis is still not clear to the doctors treating you. It can be difficult and thats the reason why we sometimes treat both bacterial and viral causes of the rash.

      @@shaikthaslima4412 It may look dry, but seborrhoea eczema is caused by oiliness on the skin that causes a yeast overgrowth. Most of the treatments mentioned in the video including anti yeast creams, shampoos and tablets may be helpful for your condition too. You could ask your doctor to prescribe them for you.
