    I went my whole life with little to no acne and around when I turned 22 I started getting horrible hormonal acne. After talking to many aestheticians, trying different treatments, researching like crazy cures, spending loads of money etc. The thing that finally helped me was going to my doctor. My mom told me to just stop messing with it and it would go away which some truth to that… buuuttttttt I had to finally go on an antibiotic, start a cream and get on birth control. Now I know who to trust what products actually help and what is and isn’t normal. Wish I would’ve found these videos sooner but now I am managing because of this and other advice. Moral of the story is TRUST YOUR INSTINCTS I knew something was wrong with my skin that topical solutions would not help and it took me far too long and too much money to make the right call.

    • @gabbiehernandez419
      @gabbiehernandez419 ปีที่แล้ว

      from what i’ve seen/heard is that you can find ways to treat it like using dandruff shampoo, zinc, sulfur, salicylic acid, ketoconazole, etc. it goes away for a while but, it’s a condition that will reoccur. it affects people that have oily skin, are sweaty, or live in hot/humid places. antibiotics help but, once you stop taking them it comes back but worse. something to do with the yeast on your face.
      i’ve been on and off antibiotics for 3 years now but i always thought it was hormonal. now that i know this is caused because of yeast i’m gonna try other ways to keep it under control without antibiotics.

      Just like hair, skin is a complex complete system, these doctors have learnt it (it’s pretty intense!) Shouldn’t worry about it so much, for we aren’t as pursuant as dermatologists!
      Though, as advocates for our own skin (since dermatology has always been given the unfortunate reputation of it being “aesthetic” instead of actually really being medical: since it’s treating a topical layer instead of the root causes per say : while there is medications involved and prescriptions etc : it’s still ironically not insurance-covered because it’s “aesthetic” - “not medical” 😂🫠 so~ gvt terms: not us), we do our best.
      I feel if we simplify, and start to look at skin as how we treat hair… Everything becomes so much simpler.
      Cleansing. Even our hair benefits from some water & oil. A protective film or sealant (silicone: oil : hairspray and/or gel/hair-> skin/vegetable glycerin - squalene - oil - actual silicone or ceramides to hold in moisture - Toners like Thayer’s Rose Water mist before oil and/or moisturizer). Moisturizing serum (for hair - can be a moisturizer for skin).
      “Heat protectant”(UV). And any additional treatment : hair’s deep conditioning (spot treatment in example acne treatment like retinoid: Vitamin A: OTC Differin Gel if not prescribed Tretinoin for skin). ❤ That’s it.

    This makes so much sense. I've been trying to help my daughter who has similar acne issues as I did at her age, but even with the La Roche system, prescribed doxycycline, and clindamycin phosphate gel, she is still struggling. I noticed a significant taming of the issue on her face, but I have not had any luck with her chest and back. What struck me was your mentioning the issue with dandruff as well. My daughter gets a lot of bumps on her scalp and if I scratch at anything can see her skin begin to flake. She's a tomboy and into a lot of sports, so she could care less about any of this, but it would explain why so many of the dandruff shampoos, the acne meds, and everything I've tried doesn't help the way I thought it would. I don't expect miracles and thankfully neither does she, but we haven't been able to get it under control at all. With the current acne regimen and what you spoke about in the video, it would make sense that we are only treating one of the acne issues and not the other. This might give me something to ask about at our next peds visit. THANK YOU!

    For me, using 2% zinc pyrithione and avoiding oils/fatty acids/esters in products is what I need to do.
    Zinc pyrithione gets down in the hair follicle too. Oils/fatty acids in products can feed fungal acne.

I recently went to Sephora and their staff sold the Drying Lotion to me and I was suspecting it would work the same as the existing oxy benzoyl peroxide I got for my acne + cystic acne. But after seeing this video and coming across your comment, I'm happy with the drying lotion I got from Sephora (not cheap) as it's got salicylic acid, in fact very concentrated and painful when used on my acne. I asked the dermatologist if my widespread chest and back acne could be fungal as it is itchy sometimes, and he said "clinically it looks the same; there's no way to tell the difference." He just prescribed a topical antifungal and a couple topical for regular acne and left the room. I'm pregnant so I understand my options are limited. Just trying to get it under control before my wedding in like 6 weeks. Didn't have this skin issue when I chose a backless dress with a plunging neckline.😫

    Hello doc! What products can you recommend about the treatment of dark circles in the eyes?

    • @Doctorly
      @Doctorly  4 ปีที่แล้ว +38

      Hi Gerthelyn, it depends on the cause of your dark circles. Some people have dark circles due to genetic hyperpigmentation in this area, some have dark circles due to shadowing, and some have dark circles due to deep tear troughs. For true hyperpigmentation, daily sunscreen is definitely the first step. To fade some of the dark spots with time, niacinamide, vitamin C, alpha arbutin, and retinoids can help over time but you have to be careful with strong ingredients around the eye because this area is very prone to irritation.

    Hey there! I was treating my regular acne with topical antibiotics for about a year, and it started to stop working so my doc gave me doxycycline which ended up giving me itchy uniform little bumps on my cheeks, forehead (never had pimples on my forehead before) and back. I think it’s fungal from too much antibiotic and when I did nizoral it helped a bit but didn’t fully get rid of it. My doctor doesn’t believe me that it could me fungal though because she says it’s “too rare”. But I’m positive it is. My question is this: if it is fungal acne, when my body finally gets all the good bacteria back will the fungal acne go away? Or do I need to find a way to treat it and maintain it for the rest of my life now? Please help me I’m just looking for answers and my doctor doesn’t want to even see if it could be fungal and wants to give me more antibiotics. Also I cut out everything that can cause the fungus to grow and that did help it a lot and my forehead is completely smooth again but my cheeks aren’t.

    I suffered with this years, and didn’t know what it was. I went to the doctor once, and he told me it was fungus. I somehow never took him seriously, because of how he just casually glanced at me, said it could be fungus, gave me some trial ointments that were sitting on his desk (that worked by the way), and sent me on my way. All this time I thought he was not sincere with his diagnosis. Years later, I’m now understanding, he knew what he was talking about. The ointments worked, and then I stopped using them, and after a while it came back. I used a desperate method of exfoliating gloves/brush and some laundry detergent mixed with my regular Dove soap to finally keep my fungal acne at bay.

      Hi Lily, we plan to do more videos on acne soon. But just as a principle, the best treatments for acne are retinoids, salicylic acid, benzoyl peroxide, and azelaic acid. Since you have sensitive skin, start slowly with these (every other day) and go from there. If you are developing scarring, its best to see a doctor ASAP because scarring is very difficult to treat compared to just treating acne :)

    • @Doctorly
      @Doctorly  4 ปีที่แล้ว +3

      It depends on the cause. Acne, rosacea, perioral dermatitis can all cause bumps around the mouth. I notice a lot of people who have acne or acne-like bumps limited to the beard area may be touching their face a lot. Touching your face can lead to bacteria or oil on your skin and worsen acne. Starting with a gentle cleanser, like Cerave SA, and then using a niacinamide, or some other active ingredients. Also, although retinoids are amazing for acne, they can be irritating around the more sensitive skin of the mouth.

    I have been breaking out for the past year. I’ve also been diagnosed with an autoimmune disease that effects the liver. so I’m on several new medications. I finally went to my dermatologist and asked him about the possibility of fungal acne. He basically dismissed the even thought of fungal acne existing and said “don’t believe everything you read on the internet”. However I’m a scientist so am very thorough with my research and really think I have fungal acne. Should I trust him with what he’s recommended or should I get a second opinion?

    • @Doctorly
      @Doctorly  4 ปีที่แล้ว +12

      Hi Erica - So technically "fungal acne" is not a medical term, the correct terminology is Malassezia folliculitis, so perhaps he is just not familiar with the terminology! The term basically developed on the internet to describe this condition called Malassezia folliculitis. To be honest, "fungal acne" is pretty uncommon - so I would trust your dermatologist. Board-certified dermatologists have 12 years of training and have treated thousands of skin conditions.Try the treatment he prescribed for a few months and if that doesn't work then consider returning to him or if you're still not satisfied, then you can consider getting a second opinion :) I'm sorry that you're dealing with so many different medical issues right now - hopefully things get better soon for you!

    Thanks for these videos. Very helpful content. I have a couple of questions regarding anti-fungals and the face area. I have redness around my nose that seems to be separate from my regular acne. My GP suggested I apply topical anti-fungal treatments (Daktarin or Daktakort) around the area, and advised that Daktarin would be safer because there is no steroid in that. How long would it normally take to see results in reducing redness if the red bumpy area is a fungal infection? Is it best to avoid applying steroid creams to the face and use Daktarin not Daktakort in the short-term and long-term?

    I have skin colored bumps and when it is rainy, windy or really hot it goes ape shit and then itches so bad I can’t even stand my hair to brush on my face. I have seen several dermatologists and they just keep giving me antibiotics (doxycycline, monocycline and azithromycin all failed) AND tretinoin.05 which just makes it itch so much I end up not being able to use it. 😞 It’s been 5 years almost and I’m still trying to get answers.

    I highly doubt you will answer a comment for a 4 year video but i'll try, why didn't you mention Azelaic acid, I'm using it for close comedones/whiteheads?
