My main complaint years ago was that my head became incredibly oily over a short period of time and became extremely itchy. Ketoconazole shampoo was the only type of shampoo that took the itch away completely. So defo worth while for me.

The anti androgen effect is not through the inhibition of 5ar enzyme, it doesn’t not block the aromatisation of testosterone to dht. From theory it should locally bind to androgen receptors in the scalp, beating DHT and Testosterone, potentially halting miniaturisation of the follicle. However the theory around it’s localised presence can be debunked by the fact that if it can be absorbed it will inevitably go systemic. It is a weak non steroidal anti androgen… not a 5ar inhibitor. It may work for people who are not genetically prone to AGGRESSIVE androgenic alopecia (99% of males are prone to hair follicle miniaturisation to a certain extent) but it will not be an effective compound if used as stand alone mono-therapy for androgenic alopecia. Keeping in mind with that, the use of finasteride will also not successfully halt androgenic alopecia because even though you are running your DHT into the ground there is an increase in testosterone. Testosterone has a lower androgenic effect at the hair follicle. Meaning the speed of miniaturisation is decreased but not completely halted. This is where there is potential found in the use of better more potent non steroidal anti androgens to combat hair follicle miniaturisation on both the androgen receptor front and then using finasteride to decrease DHT and indirectly reduce the level of androgens the anti androgen needs to beat to the receptor

I've been using Nizoral for 20 years and I've still got thinning on the crown. I wouldn't recommend anyone relies on it alone.

What about the nizoral cream though? The shampoo is used only for 5 mins while the cream can be left to absorb in the scalp, wouldn't that reduce DHT to a greater degree than the shampoo would?

I stopped using ketoconazole shampoo because I think it is overrated. You have to apply min two times a day for a few hours and you wash off keto after 5 m

i am 22 and experiencing hairfall since I was 19.. an eye doctor prescribed ketoconazole shampoo .. i had no expectations tbh.. but to my surprise it stopped my hairfall completely within 4-5 days.. and ever since then I have to shampoo my hairs everyday. But it's not really working anymore..

Just a correction - inflammation does effect the Male pattern baldness not just a temporary shedding, it shortens the Anagen phase which accelerates the permanent loss of the follicle, also not sure if the comparison with finasteride/Minoxidil was necessary as Ketoconazole Shampoo's are a complimentary treatment best taken with finasteride.

Being a hairstylist of almost a few years, I can share with you you, that you're not supposed to apply strong hair shampoo on your hair.

The studies about how it works synergistically with finasteride and is better than just finasteride alone
