This is bad dentistry. Do not agree to this kind of surgery as the risks are too great. Artificial bone powder (Bio-Oss) could be used along with a membrane to preserve the socket in preparation for an implant perhaps 6 months after extraction. It could also be used to try and patch a maxillary bone defect, ideally mixed with concentrated growth factor and/or mesenchymal stem cells extracted from the patent's blood or marrow. The vascularity of this mandibular bone is not particularly good. There is no point placing bone powder on top of the cortical plate. The powder needs contact with cancellous bone. Small holes could be drilled in the area of bone on which the artificial bone is to be placed to provide this minimal contact and blood supply to the artificial material. The artificial bone does not induce the production of real bone. The environment in which the artificial bone power is placed induces the powder to integrate or not. Unless a sufficient blood supply to the artificial bone exists there is a very real chance of failure with insufficient bone on the buccal (cheek) side, which may result in recurrent infections, further bone and gum loss then implant failure. The patient could later face more radical bone augmentation and tissue grafting. The only acceptable thing I have seen here is the placement of a membrane. Some dentists will argue this kind of bone augmentation is an acceptable option when placing implants but it is not. It is a very risky option...risks the patient does not understand. Responsible dentists will ensure 2mm of bone on the buccal side, which is crucial to long term success. In fact some dentists opt for a split ridge procedure, which splits the ridge of the bone to ensure adequate bone volume on the buccal side. The procedure you see here may look like a more palatable option than a split ridge procedure but at the very least allograph or autograph bone should be used in the mandibular position to ensure there is sufficient bone volume at the ridge BEFORE an implant is placed. Results using bone powder like you see in this video are highly UNpredictable. Get that indemnity insurance ready!
oh my goodness, that sounds so scary....I lost a front tooth, which left a lot of bone loss. I randomly popped into a dentist, who said we can do an implant, all sounded so good!!
I walked out with what looked like I had 3 front teeth, it was a joke. Its caused me no end of problems, I did eventually get an "OK acceptable crown, but still have the the bone loss, and a the pocket just fills with puss,its soul destroying...I go to the hygienist regularly, and had 2 option to fix the problem, so again,confused,and lost so much trust in dentist.
The implant is stable, could the gum be opened and the pocket filled with artificial bone or does the implant have to come out to fill the pocket.
The pocket has made the tooth next to it unstable now....
Thatcher Washington exactamente what they did to me and the bone is gone. It recent I hope I not loose my implant cause of this.
