Both, no. :-) It was dinner and there were no limits. Now, I have only ever ordered multiple appetizers, so I'm not sure about getting multiple main dishes. I've heard from other people that you can, but I don't have any personal experience with that.
The “Speciality” restaurants are not included in the fee for the cruise. You can get a dining package (an add on, like the different drink packages) that would cover it.
