    I am level 5. I have been seeing a neuromuscular tmj specialist for a few years and while I have made progress, my right jaw joint is so damaged that it keeps shifting every week or so when im eating. Im ready to take my own life im in such distress.

    Dr Mistry, what is your take on the NIH and TMJA recommendation to not use orthotics during the day for longer than 6 weeks due to bite changes that can happen? I was treated using this type of therapy and now have an open bite. I know many TMJ practitioners use this method as well, just wondering your thoughts on the warnings against bite changes

      The TMJA is not an association I would recommend listening to in any way. They have hindered progress in the TMJ field for decades. The crux of TMJ issues for many people is that when the muscles and joints are in their optimal position, the teeth do NoT line up. Essentially the teeth/bite can be the cause of TMJ issues. Therefore, yes, the bite can change when the jaw is put in the correct position. Because of this, phase 2 is essential for some people - this involves bringing the bite back together with braces or full mouth reconstruction.

    Do you usually do CTScans only or also do MRIs ? I watched your video regarding the same

      I do CBCTs ALWAYS (unless the patient is pregnant) and MRIs as needed.

    I’m in Canada. Finally got an mri done on my jaw and it showed a rip or hole in my cartridge in my TMJ. 3 oral surgeons have turned my referral down fromy family doctor. Not sure why and what else I should do. I do not want surgery but maybe some kind of physio. Currently only have a night guard from my dentist. Always have headaches, jaw pain, tired jaw and tongue feeling. Sometimes jaw clicks/pops. Frustrating

    Thank you for the video, I am actually a bit confused as I can’t find myself in any point of classification, because I have discs displacements from childhood on both sides (with reduction on one side and without reduction on other side), but absolutely no pain and no locking episodes. Dear dr. Priya, could you please tell me if I should seek treatment in my case?

      It’s hard to say. Many people seek treatment because TMD affects their quality of life.

    What is the success rate for these orthotics typically? I am very hesitant as I have heard they make it worse for some, and my TMJ dentist quoted me at 3,000. I’d be level 1 in your video. Thank you!!

    Can level 1 or 2 cause extreme/debilitating pain in the center of the teeth/gums that radiates up the center of the face? The type of pain is severe pressure/crushing pain. I have been a long time teeth clincher, but this pain started after a far back bottom molar was extracted. I was thinking possible nerve damage that led to Persistent Ideopathic Facial Pain, but am now wondering if it’s TMJ related. Thank you!

    Hi Priya, would you be able to say what a healthy and normal opening is? I have been going to the University of Wa Oral Medicine clinic and have been receiving some help, but am still very interested in your work. My jaw opens 29-30 mm currently and I’m hoping it will continue to grow but it’s been a long journey over the last 2 years.

    Can the joint have effusion? I have ear crackling and it is super loud. Can this ear noise go away with orthotics.

    Is there a simple explanation as to why one may be only able to open say 1.5 finger widths but can open much further when yawning?

      Excellent question. My mouth opening also like 1 to 2 fingers but when yawn it opens wide even we control.

      Potentially - it depends on how they’re designed. The nighttime one is usually bulky so it’s hard to speak with

    I am able open my mouth only one cm to two cm only
    No severe pain
    But at times one sec shock pain in year and a pain in toe
    I suffer from tablets controlled epilepsy too any advice

    I've had terrible TMJ pain for 4 weeks and lost a lot of weight as eating us so painful. I can't put the gum shield in as my teeth are too painful, it makes everything worse. I have terrible toothache on all my right teeth top and bottom. I've had a scan and am waiting for the results. The jaw pain has lessoned as I have been having the cranial therapy but the toothache is getting worse. What can I do? Ps it started with electric shock pains through my teeth and jaw in the middle of the night. I've never experienced such pain before, Many thanks, Jenny

      @@theTMJdocthank you but the problem is I'm in England. I had 2 jaw joint wash out operations in my 20s and I've been ok until 4 weeks ago, I'm aged 44 now. I've had clicking and popping for years up until now.

      @@theTMJdoc I am seeing a maxillofacial specialist in 10 days time but they cannot see me any sooner. I am seeing twice a week am osteopath for cranial massage which is helping. I just can't cope with the tooth pain.
