Hello thank you for this information. I just recently did a quest diagnostics drug screening for the Coast guard. Now the Coast guard requires the medical review officer to sign form cg-719p. I have to send that form back to the Coast guard along with my other documents including my physical to get my merchant Mariners credentials. My question is if I did a test in my urgent care with quest labs for a coast guard physical, how do I get the medical review officer to fill out this Coast guard form cg-719p and have the medical review officer send this back to me so I can send it to the Coast guard with my other documents?
