Perhaps the origin of sugar coating something?

Yes. Pills still are made with sugar coatings-the decongestant pseudoephedrine hydrochloride, for example. To sugarcoat something, literally and figuratively, is to make it more palatable.
More precisely, not all pills are pills. The distinctions have to do mostly with shape. Those that are balls are true pills. Tablets are discs. Caplets have the shape of capsules, but are solid. Then there are lozenges. Of course, nowadays, to help in identification, we have pills of many colors, sizes, and shapes, including ovals, diamonds, and triangles.
