Creative Connection worked with the the Royal College of General Practitioners to create this animation that explains the benefits of Person Centered Care Planning to both GP's and patients. The central message is that while setting up a new system requires work in the short term, in the long term both their lives will be easier, more rewarding, more manageable and ultimately, healthier!
