Here's how my local authority does it: do a carer's assessment and give no advice, type up the notes two months later and get them wrong. Then, because the cared for is self funding, give them an out of date list of care agencies, some of whom didn't cover the area and most of whom had no staff to come to the house unless mother wanted to get up at 11 am (she didn't) and go to bed at 6pm. They system in practice is an unfunny joke.
