Great series. I am a family physician preparing for board exams. I have studied and used a number of board review materials, but I especially enjoy TH-cam videos while exercising and podcasts while driving. Among the many sources I have found, your videos and podcasts are the best. Concise with high yield material. I really love the series you have done with 50 questions on a variety of topics. If you are still looking for material to make new videos/podcasts, may I suggest that you consider making a second 50 question series on each of the topics you have already done (or 25 questions), and I would also really enjoy it if you would make a video with high yield medical triads, quartets, pentads, etc as those are commonly tested.
I am 18 days away from my PANCE and am losing steam with my other study techniques! Thank you so much for making these videos, they have been so helpful!

I had a quick question about number 41. Cold nodules (versus hot) have a greater chance of malignancy which is why if you have little uptake on the scan, you need to further rule out malignancy correct? I'm 99% positive that's what you were getting at but I was reading it as "what dx IS ruled out with a cold nodule with the answer being malignancy" and I learned cold nodules were more likely to be malignant compared to hot. Again, I think this was my confusion and nothing on your part but I wanted to double check. Really love the videos and looking forward to more of them!! Thanks!
Yep, just like you said, if the scan shows little to no iodine uptake (a cold nodule) you need to make sure its not malignant as these have type of nodules carry a higher risk of malignancy. just some additional tidbits for your audience question 49 - myxedema coma - before treating it with IV synthroid - ALWAYS CHECK for underlying adrenal insufficiency and if present - give IV DEXAMETHASONE 1st before giving IV synthroid -- do it the opposite way and the patient will go into adrenal crisis and die question 50 - for patietns prone to UTI and candida especially in urine for diabetics - AVOID giving SGLT2 inhibitors as they expedite sugar excretion into urine which = more sugar in urine = more chance of candida UTIs occuring.
On #45, the patient has unexplained weight loss and putting edema in both legs? I assume you meant he had unexplained weight gain, especially since you had just explained that TZDs can cause weight gain. Please clarify. Love these reviews. I'm taking my PANCE in 2 weeks and these are great!

I have a question for question #12 your explanation for the answer seemed to be in-revrse. Did you mean to say never give an alpha blocker before starting with a BB in pheochromocytoma because you were saying to never give a BB before starting with a alpha blocker? Just to clarify
Sorry if it was confusing. So in a pheochromocytoma, avoid giving beta blockers before you have adequately treated them with Alpha blockers first for at least a few days. If you do the reverse, start with beta blockers before alpha blockers, you can cause a hypertensive crisis. So correct order is alpha blockers first, then beta blockers. Hope that clarified it for you! Let me know if you have any other questions.
Hi for #17, if a patient has cushing disease wouldn't they show no change/no suppression with the dexamethasone suppression test because the pituitary is over-producing ACTH without attention to feedback?
This is true with a low dose of Dexamethasone, but not in a high dose Dexamethasone suppression test . When you give someone a high enough dose of Dexamethasone, Cushing Disease (Pituitary) will suppress, and it is generally the ONLY type that will (with some rare exceptions). From UptoDate: "By increasing the dose of dexamethasone four- to eightfold (ie, 12 to 16 times the usual daily maintenance dose), ACTH secretion may be suppressed partially in most patients with Cushing's disease (pituitary). In contrast, many nonpituitary tumors that produce ACTH ectopically (such as small cell lung carcinomas) are typically not responsive to glucocorticoid negative feedback; ". So if you yell loud enough at the pituitary, it will eventually wake up and respond, the other ones like the ectopic producing tumors, don't care. Let me know if that clarifies it.
