    I found another way to remember PT and PTT and intrinsic and extrinsic pathway. PT think play tennis outside. PTT think play table tennis inside I never miss it now.

    Just 5 minutes ago did a patient presentation on iron deficiency anemia. My preceptor "what kind of anemia does she have?" Me: "uhhhhh, microcytic." Him "good." "What are the different types of microcytic anemia?" Me to self "think, think, think.....TICS!" Shapiro saves the day!

    I am confused with Q39 explanation - if it is Hemophilia A - X linked recessive wouldn't that be more common in females? Also how could we say this is not Vwf disease?

      X-linked recessive meaning if this is passed on to a male, since they only have 1 x chromosome, they will have the disorder, women, having 2 x chromosomes will generally only be carriers since they will still have one unaffected x chromosome and will not be symptomatic. Differentiating between VWD and Hemophilia A can be challenging since they have very similar lab findings and presentation. The key in this vignette is the gender of the patient, as well as the hemarthrosis, all specific to Hemophilia A (particularly the hemarthrosis, where as in VWD be looking for mucocutaneous bleeding). Another way to differentiate, not listed in this vignette, is bleeding time, generally bleeding time will be increased in VWD, but normal in Hemophilia A. Finally with Von Willebrand Disease, look for them to mention a family history since it is the most common hereditary bleeding disorder, and of course for them to mention decreased VWF antigen/activity. Let me know if that makes sense or if you have any other questions. Thanks!
