    I'm studying for my PANCE and I absolutely love all of your videos!! One way that I was taught to remember where Mitral Regurg and Aortic Stenosis radiate to is: A man who picks up his child by the armpits (axilla) is a "Mister/Mr." versus a man who picks up his child by the neck (carotids) is an "ASs." I hope this can help someone else :)

    I used to use the mnemonic "S'ups and Downs" based on where you auscultate
    S'upS = Systolic murmur on "up" are Stenosis. So "up" meaning upper valves on ausucultation (aortic/pulmonary)
    DownS = Diastolic murmur in "down areas" are Stenosis. And "down" meaning lower valves on auscultation (Mitral/tricuspid)
    Hope this helps

    To remember Müller sign I always think of the principal yelling "Bueller!!" (from Ferris Bueller's Day Off); never forgot about the uvula sign after that for aortic regurgitation.

    If anyone else is horrible with remembering nmeumonics (like I am) I remember the diastolics based off of blood flow. So any murmur that has blood going into the ventricles will be diastolic! (pulm/aortic regurs has blood backflow into the vents, tricuspid/mitral stenosis has blood going into vents)

    hi! love your videos! quick question though- at 31:58, you say that PS murmur is best heard at the LeUSB but then you say that it is increased with inspiration. I thought that was only for right-sided murmurs? thank you!

    • @cramthepance
      @cramthepance  2 ปีที่แล้ว +1

      So this is a right sided murmur because pulmonary stenosis is a problem with the pulmonary valve that controls blood from the right side of the heart. Don’t focus on where it’s best heard on the chest, that’s not related to which side of the heart it’s affecting. Let me know if that makes sense!

    • @JohanaCeja
      @JohanaCeja ปีที่แล้ว

      @@cramthepance I had the same question. So, in this case, wouldn't Tricuspid Regurgitation also have RINspiration? since it is affecting the Rt side of the heart?
      Also, I just want to thank you for making such AMAZING videos!

    • @cramthepance
      @cramthepance  ปีที่แล้ว

      @@JohanaCeja Thank you! Yep, the murmur of Tricuspid regurgitation increases with inspiration due to increased venous return to the right side of the heart. I believe thats what was said in the video but let me know if not so I can edit it. Where the murmur is best heard though is the left lower sternal border. Rinspiration only refers to if the murmur gets louder with inspiration, not the location of where the murmur is heard. It can be a bit confusing, let me know if that makes sense.

    • @JohanaCeja
      @JohanaCeja ปีที่แล้ว

      @@cramthepance This makes sense. Thank you for clarifying. Yes, I just went back in the video, and you do mention that TR has RINspiration, I got confused because I didn't see it in the typed notes like you had in the other 3 (PR, PS, TS), but it is definitely mentioned. Thanks again. :)
