    I really suspect a checked kick or some prior trauma to the bone made it susceptible to that fracture, but the break actually occurred before what most of highlights are focusing on

    Great video as always Brian. Just to further add to what you've already mentioned about there being a pre-existing injury, I think he was landing hard leg kicks with the same leg without any visible discomfort. So I have a strong hunch that he broke it during the fight instead of camp.

      In the post interview Dustin mentioned that he might of felt something off when he was checking those kicks. And looking at the kicks, those were hard hits...reminds me of Silva's injury.

      Those leg kicks are known to do that, and it's part of why they do body hardening in Muay Thai- repeated microfractures lead to increased bone density and callus formation. The axe style kicks he was using on the ground are also high stress, though you generally feel them higher( mid shaft to tibial tuberosity area).

      Were there many checked kicks? I thought it was likely when Connor kicked Dustin in the hip bone. His leg seemed to flex unnaturally a little.
      And that was just before this happened.

      It was not his ankle! It was his shin bone. He kicked Dustin's leg moments earlier, but he kicked improperly, with the inner side of his shin. Similar to Anderson Silva's injury

    2:03 The initial fracture happened earlier in the round (like Dustin said) when DP checked a leg kick. Dustin could tell by the way their legs struck one another that something had happened to Conor's shin. During the fight DP checks the kick then points at Conor as if to say "did you feel that?"

      No that would have been seen on his MRI. Ankle injury's are sketchy, hard to see. Tibial fibula fractures are blatantly evident!

    It appeared to me- that it was the front kick (checked by Poirer) to the elbow with 10s left in Rd1 which initially caused a fracture or injury compromising the tibia’s integrity. Then when he went to step back the twisting motion to the already compromised bone caused the more severe break.

    Thank you for posting this! Very informative on bone health to be aware of, especially while exercising. I tend to roll my ankle from time to time and would hate for something like this to happen. Could it be that he was doing leg presses too heavy and getting shin splints and weakening the bones?

    He didn’t have enough time to condition his shins properly. It takes a good few months and years to have strong shins. And he was throwing kicks really hard. He most likely had maybe mirco-fractures in his fight camp

    Good video! I had a spiral compound fracture on my tib/fib 3 years ago and my leg still throbs. Had a rod put in followed by a bone infection. How likely is it for him to actually come back? I currently can’t crawl on all fours and the thought of doing leg day is a nightmare.

    Awesome breakdown. Really cool. Makes sense now. Just wonder how Conor may have fractured the leg prior. Was it a checked kick? Pre-existing injury from training? MMA is crazy. Anything can happen. Really unfortunate end to the fight. I'm a fan of both fighters and ultimately this doesn't really prove whos the better fighter. Would have loved to have seen a 2nd round.

      whether it was a check or not, that kick was enough to break an ankle ligament and cause the instability that led to a break as everything collapsed. the pop of a big ligament breaking is also a fair bit of energy, so it might be something dustin would notice as odd. a large sprain or a small break could both produce the result we got

    Really interesting video. McGregor was definitely using that left leg for quite a lot kicking from the get go, he landed a good few kicks to the outside of Poiriers knee and shin, and that must have been the cause of some kind of trauma or hairline fractures before the actual fracture you're pointing out. Poirier was definitely checking those kicks, his knee was bent forward to take the blows. There were also a couple of moments where McGregor was sensitive with planting on his left foot at different points during the fight, so it might have happened early on too, and the subsequent kicks may have made it worse until the break.

      Yes probably. He kicked a lot on the heavy bags in camp. So he probably had some micro fractures that hadn't heald before the fight wich may have made it worse

      Conors shins are conditioned. Hes been kicking for years, if it was the check, then he would've been limping immediately. It wouldn't have took so long, he possible was already sort of injured before the fight idk.

    Hey doc! Thank you for the video. I was watching some highlights and there was a moment when things seemed to shift for McGregor. About 3:40 Poirier begins connecting with multiple left hands. One lands straight into McGregor's teeth at about 3:33 and McG stumbles backwards crossing his left leg over his right. It happened so quickly I didn't think much of it in the moment. Those missteps after a stout shot happen all the time. I suspect he may have been hurt during that awkward step.
    At that point McG seemed unable to keep pace with Poirier in the clinch and then gambled with a guillotine, which seemed an uncharacteristically bad decision for such an experienced fighter. I think something may have already been wrong at that point and he was rolling the dice to end the fight with a submission.
    The cascade of decreasing stability you described makes a lot of sense. Nightmarish sense. Do you think it's possible the cascade started even earlier than that foot plant and tweak? In the heat of the moment it can be hard for a fighter to know how bad an injury really is or what it can lead to. Nobody can predict something like this. Awful stuff.

    Great review, what needs to be pointed out are those kicks Connor was throwing at Dustin, he threw some kicks at his thigh that may have resulted in twisting the ankle and making it more susceptible to injury.

Even DC the commentator said that he checked it and made Connor’s kick impact with the front of his foot onto poirer’s knee and this likely caused the first mini fracture in the tibia

    Dustin said Conor most likely fractured his foot initially when Conor threw a leg kick and Dustin checked it. I didn’t even see that first little break either. That adds a ton of credibility to Dustin’s claim. Conor fractured it on the checked kick, furthered the damage on that first step, then totally broke it when he stepped back on. Excellent video, dude.

      Except if you go back and watch the kicks, especially the one Dustin said he, " checked ", you will see he never actually checked it, or any kick for that matter. The kick in question actually lands behind the knee. My guess is Conor had a bunch of micro stress fractions going into the fight and all those hard leg kicks he was throwing just worsened it to the point that it just eventually gave way.

    Hey doc I’m an mma fighter from San Jose california. I’ve been dealing with wicked knee pain and I developed a bakers cyst from an injury I sustained during a fight camp. Long story short, my leg ended up crooked and knocked off alignment by like 40 degrees or something.
    Doctor suggested an HTO to prevent a knee replacement however my concern was if I was going to be able to return to my combat sport after this and how long the recovery realistically could be? Also if it’s anything like Chris weidman or Conor’s injury??

    Hi doc Bryan, thanks for this video! During post-fight interview, Dustin said that the trauma could have happened when Conor checked Dustin's kick early in the round. I think you need to have a part 2 session on this to dissect the cause of the injury further. :)

      You're right I just replayed the fight and at mark 4:40 of the 1st round, Conor landed a leg kick on the knees of Dustin. He landed at least twice on the knee and Dustin did point at him as if to say that he kicked on a hard spot

      @@burekskremicem that would put quite a bit of force on the exact spot that his leg broke to be fair. They weren't clean checks in any way but with the way the end of his foot hit the knee it could have put a lot of strain on the bone and caused a hairline fracture that broke as he went to push off hard again

      @@burekskremicem sooo, connor just broke his leg because of lack of calcium?????? ..... or maybe has osteoporosis?????....Dude if connor was kicking and according to dustin he was hitting the knee full force, don't you even entertain the idea that is possible that the kick cause a bone injury that was later exacerbated by the sudden movement,???? Don't know men imo its possible.

    Consider the entire chain of events leading up to the acute injury: full training camp, with ample focus on kicking the heavy bag, due to the manner in which the last fight progressed (Dustin using calf kicks to compromise the leg before finishing with strikes), opening the rematch by throwing naked kicks, several of which were checked by Dustin, leg/ankle is already compromised before the tape highlighted in the video.
