  • We have an update on Conor McGregor and the broken leg that he suffered at UFC 264 as the fighter shared that he has undergone successful surgery. In this video we'll talk in more detail about the injury and what actually could have happened with the kick checks that Dustin Poirier said caused the break.

    For those saying it was Poireir's left (hidden a bit) elbow they were referring to, I can't see any way the angle of Dustin's arm in that position breaks a tibia. The forearm is nearly parallel to McGregors leg and I can't see how Dustin's forearm wouldnt have actually broke instead if that were the case. Either way, well never know for sure, this cumulative mechanism is just my opinion.

      That’s a proper analyzation I think and sums it up nicely. What I wonder is, if that specific spot was wearing down overtime, not only throughout the fight, but possibly in training. I’m not sure if a bone can accumulate damage little by little until it breaks due to the leaf that tipped over the full glass. (if you will) Somebody let me know if that’s a possibility and share your ideas as to what happened!

      I think it could have happened from the up kicks Conor was throwing or when Dustin was stacking Conor on his back and fell down into Conor's guard. At one point Dustin grabbed the same leg right above the ankle and kinda pulled/twisted it out the way for him to get in conors guard to throw some ground and pound. A few seconds later Conor stood up and then the break happened.

    He’s been training a lot of leg kicks during his camp, so I imagine wear and tear was a big contributing factor to him breaking his leg. I would imagine the hard kicks he threw at Dustin destabilized it even more (no shin pads in a real bout), and him putting further weight on his leg was the beginning of the end for him. Great video analysis, definitely learned something from it.

      @@tocunha when did this doctor claim his strong suit was the technicality of fighting? He would be a cool addition to the UFC broadcast with medical analysis. There are guys like Stephan A smith who don’t know anything about fighting yet they find a seat on the broadcast so why not a professional doctor to break things down when there’s injuries?

    I had the same injury from soccer in high school, didn’t get a rod, just a plate and 10 screws. Took a long time mentally to be able to trust my leg. Mobility was never the same. Conor has the best rehab, nutrition, etc to get his body back, but I don’t think he’s gonna move the same as before. I could see him going the boxing route where it’s less likely to re injure it.

      Even if he manages to get it back to normal again it'll take alot of time maybe like 1 or even 2 years? by then everything would be different in the division and moved on.

      I had a weber C fracture, worst ankle break you can have, I now squat 160kg at a body weight of 59kg. My ankle is strong...just not good at running. I had a plate and 11 screws. I haven't had to kick anyone though haha

    6 weeks on a crutch and then back at it again? my leg was "broken" at the same spot surgically and put back together with screws and plates. that was 6 years ago and im still in pain every day. if someone would kick me in that leg i would probably die

      Yea no way he is back doing anything after 6 weeks, my mate came off his scooter in thailand and broke 2 bones in his ankle 1 year before he was back in the gym again!

    I agree with you about the cumulative thing. It's just weird how it happened with the first break so innocuous before he stepped back on the ankle. The leg must have been badly damaged prior to that like 70% fracture, then Conor finished it off with the torsion step and the final backwards step. I still think Poirier is right though in that the major incident happened during the check when Poirier pointed at his leg, although Conor threw a bunch of kicks after this. So one big hit did most of the damage then combined with the cumulative effects, led to a complete fracture at the end of the round
    I really think it's worth investing in Dustin's opinion as a fighter can feel the difference between the impact of the strike and the follow through. Dustin clearly felt something different on one particular check, and claimed to hear a sound, otherwise he wouldn't take time to point at it during the fight in a period when Conor was on top. Interesting video Doc, as most of yours are!

    We need the overhead angle- great video as always. As an MD, I think 2:05 is where the leg breaks. Conor was vulnerable. He was trying to throw a hard straight kick at Dustin left thigh. To do so he is leaning far back with his trunk and snapping his left leg out there. But you can see Dustin rolls his right right knee to turn his pelvis away so Conor misses. This leaves his leg straight out and vulnerable. At that moment Dustin strikes with his elbow on the outside of Conors leg ( the fibula side) with a force that could snap a leg in this position. It may seem like a glancing blow from the angle we have because we don't see Conors body twist from the impact, but the force of his elbow is directed toward Conors lower leg and foot not body so that is the body part we need to see at impact. We might be able to make out some deflection if the lower leg were to fracture away from our camera angle, but this view is blocked by Dustin arm and body. An overhead view would clear this up. I hope the UFC has one and releases it.

    I do hope the nerves in his leg are okay, after my ACL surgery I’ve got a numb spot on my shin that gets severe nerve pain when it gets bumped or anything, hope that’s not the case with him!

      That's the case with most ACL's. They often knick that nerve in surgery that leaves a part of the shin numb. Conor's broken leg is absolutely nothing like an ACL injury and surgery is completely different. They can't be compared.

    Thank you for the video.I find it hard to believe an elbow ever wins against the shin. The function of the shin is load bearing and the elbow is not. The olecranon process is a small ledge serving as a leverage enhancer for extension of the elbow and it is relatively easily damaged in comparison to a brute like the tibia. Assuming they did not screw up the fibula and its role in supporting the knee and ankle, I can only assume degradation and fighting capacity after such an injury is heavily psychological.

    I used to fight amateur. Standing from southpaw, I caught a guys front kick and kicked his right leg with my left. I made contact with the lower part of my shin to the outside of his lower right leg. My fibula snapped up closer up by the knee and my tibia was fine. The break didn't occur at the point of impact. Maybe this information might shed more light on the situation?

      That's what I was thinking. I am sure this Dr is knowledge and extremely well educated, but calf kicks are a new phenomenon. Not enough data on them. They are executed so powerfully, many times right on bone. The force MAY not cause a break at the impact area, but may be transferred to the weakest link.

      I think this is a great clue because when McGregor got up with approximately 10 seconds left in the round, he threw a kick to Dustins middle right thigh and the initial fracture could’ve happened there if his leg wrapped around Dustins leg. If you pause or slow down that kick you can see his leg wrap around the thigh and to me I saw a warping of Conors leg. Seconds after that, Conor torqued to throw that left straight and his ankle gave out.

      the question i have is: supposing you just injured your ankle ligaments (eg this kick that was probably intended to connect with the shin). is it possible that when this rolls in a catastrophic inversion, is that total collapse of the supporting leg capable of fracturing the tibia?

    So now that conor has a metal rod in his shin, does that give him an advantage kver other fighters where he can letterally kill someone with his leg?
    Also i met a couole of guys with screws in their ankles just like the picture you showed and they all suffer from pain and can't excersise too well beciase of it, do you think that would affect conor's ability to train and fight?

      the left arm had nothing to do with it..conor had a scan a few weeks prior to the fight so something was obviously wronm with his leg from fight camp getting his kicks back

      It's hard to deny that any impact on the leg from kicking had some effect to the fracture when you realise conor had a leg scan 2 weeks before the fight so obviously he's had a tiny fracture since fight camp that juts got more severe the more he kicked dustin in the fight

    Watch the figjt again where he caught him with the foot. He absolutely hurt it there but it was likely a hairline fracture at that point. watch him step right afterwards the leg almost gives at that point which is why they ended up in the clinch with conor pulling guard.

He felt the impact to the both of them and knew Conor not only took the worst of it but that it was something more than a decent check.       @@handstwister You are completely correct, I meant probability/likelihood, sometimes I do make a linguistic mistake, I am very sorry i'm not perfect in that regard yet I don't see why you should see that as a clear indicator that I have no idea of what I'm talking about. As for your point, referring to Dustin's opinion about the matter is not a strong case for Dustin being correct. Again as the doctor said it is not probable (not likeable ;) ) that the check (which was not even a proper check and the camera angle hid the point of impact, even your eyes can't see through Dustin's knee) was the thing that dit it. We just do not have all the info hence me saying that you speak too much in absolute terms. Yet given the fact that you would dispute my opinion due to me making a mistake in speech I am not sure you would reconcile, but I am open to what you want to say :).

    I broke my tib and fib in a motorcycle crash in 2013. It was an open/compound, spiral, high energy fracture. I have had pain issues in my left shin ever since. There is a rod still in there, but they took the plate off shortly after surgery due to infection.
    I also have pain issues in my ankle due to the injury. I still ride motorcycles,do BJJ, handle a K9 etc but every morning I play pain roulette (deciding if my shin or ankle will hurt that day) since managing both at the same time would make me immobile for the day.
    (I also had to get a shoe insert made for my left foot since I lost over 1/4 of an inch in leg length after the surgery.)

    Hi Brian, on the “Weighing In” podcast they were saying that Conor’s inactivity over the last years caused him to fracture his leg alluding to lack of calcification of the tissues from continuous training. Is this true or a Myth?

      It wouldn't be calcification of tissues that would occur, but reinforcement of the bone. This is achieved through osteoblasts rebuilding and reinforcing bone after stress is put on them. This stress can be either micro-fractures in the bone from direct impact or stress from putting high weight on the bone. Direct impact training, as well as some other techniques, are usually called shin conditioning, which i assume is what they believe he has been slacking on. Weight training could be done to put stress on the tibia, through something like a wall-sit with weights on the quads, but is less common in MMA.

    There is a slow motion video I just watched that showed the finishing sequence. And when he throws that front kick, if you set the video on the highest resolution and slow it down to , quarter speed.. you can see that Conor kicks dustins left elbow so hard it literally flew up ,. With the exact spot that broke.. then a second or two later it's broke,.. it's bit the best view. But go watch it.. make sure you set the resolution and the speed .. it's irrefutable ... He fractures it with the kick. And the bad step broke it the rest of the way..

    Dr. Brian can you speak about why leg breaks are seemingly so common in the UFC (2 major ones in the past few months) as opposed to Muay Thai for example where low kicks are probably thrown even more often ?

    Doc, it was recently revealed that Tom Brady played this past season with a torn mcl.
    I even heard someone say a torn mcl doesn't need surgery because it heals itself.

    At @2:00 it looks like Pourier blocks the kick with his left elbow, you can see impact made his arm throw back a bit. Like you said this is more so accumulative damage and that looks like it lands on the elbow bone you mention when explaining the other elbow hitting. My 2 cents, great content.

      @@sub_zero8495 Exactly. In other words, Conor lost by having a leg weaker than Dustin’s. It’s clear now he broke his shin bone hitting Dustin’s thigh at the very end you can see it. How Conor can consider that to be “not” a loss is beyond me.
