The developers "not thinking that far ahead" is total BS. In Redguard, a game that came out in 1999, there is a cutscene that has 6 books all titled after the 6 then unreleased Elder Scrolls games. In 1999, the names of the games Arena, Daggerfall, Morrowind, Oblivion and Skyrim were all name-dropped. The final and 6th one, Argonia, is thought to have been the ESO Expansion. So I definitely think that the developers were thinking "that far ahead" when talking about Syndicates in the Shimmering Isles.
“You move like a pregnant cow” is actually a reference to “Remo Williams” where his master tells him “you move like a pregnant yak” while training him.
For the Boba Fett reference I am guessing it’s a situation where the two took inspiration from the same source (real medieval helmets), thus there’s some similarities.
For Belisarius I would agree. Belisarius was a legendary Byzantine general irl , defeated the goths, vandals, and Sassanids. Before his legendary exploits, he was personal bodyguard of Emperor Justinian I after serving time within the Byzantine/Roman Army. He reconquered Rome even.
According to a certain member of the Oblivion Writing team(i will not name names) The line "The only good goblin is a dead goblin" was actually a direct reference to the Starship Troopers film, in which Casper Van Dien's Character, Johnny Rico, Yells into a reporters camera "The only good bug is a dead bug". The team were all Starship Troopers fans. (they chose to remain anonymous when they said this, which is why i wont name names.)
I'd still buy it as a loose connection. Surely the Adoring Fan was planned, but I wouldn't put it past some tired developer to tweak the model to match a recent movie character. EDIT: or saw the design from a friend. Lots of devs know other artists, and so forth.
I think the hobbit pants for the hobbit outfit are either the green felt pants or the breeches. Both heavily resemble the overall style we see of the hobbitian residents of the shire in the peter jackson films, though I also dont think that its inspired by any specific outfit, its just the overall vibes they got

It sound like an effective torture device. The nails should let just enough room to stand up without being able to move. But there are easier and faster ways to do that and a giant coffin with lots of nails would take a lot of effort to build.

"The only good commie/goblin is a dead commie/goblin." is more likely a FalloutTES reference than a historic cold war reference. Or it might just be that Bethesda is taking the stereotypical orc, which is sort of a creation of Tolkien (and most of his ideas are taken from preexisting folklore anyway)

Edit- I also think you might be wrong on the Iron Maiden/Manowar thing specifically because they're bands that are very commonly compared, and oddly specific since one is a theoretical torture implement and one is a ship. Considering how much game devs at the time were metalheads and with other references to metal groups like the Nightwish easter eggs in Shivering Isles, I think there's more to this one.

And there is *countless* times in the older games where something is referenced or foreshadowed that happens in a future game. *Modern* Bethesda might not think that far ahead, but the original Bethesda team certainly did.
