Had this dish last night in Fiji served with chocolate caramel sauce and ice cream. Can't wait to get back home and try it. Of course the coconut cream here was fresh.
I have heart and cholesterol issues, so may try this substituting crystalline Stevia for the sugar. Stevia is a plant and has zero calories. It's available from Australian supermarkets both as white crystals like sugar and in baking, also in crystalline form. To make it even healthier, I might also substitute the white flour for almond or coconut flour.

I add a little ginger , it gives an awesome kick.
I don't add anything else otherwise it will spoil the taste of coconut or it will taste like cake😂
