Quick Approach to Cervical Lymphadenopathy

  • How to approach a patient with swollen lymph nodes in the neck? How can we recognize signs of serious illness, like cancer, lymphoma, or infection? Most of the time, the cause of lymph node enlargement will be benign, usually a mild respiratory tract infection. But, on the other hand, if there are signs of lymphoma or cancer, we want to ensure we don’t miss them. This video offers a quick approach to cervical lymphadenopathy for students and junior doctors. Every time you see a patient with enlarged lymph nodes in the neck, you should pay attention to what I list in this video. It should make your life a little bit easier. Good luck!

    I've spoken to many people who had little to no symptoms and minimally enlarged lymph nodes that ended up with stage 2,3 and some even stage 4 lymphomas. I hope more doctors listen to patient concerns, especially when symptomatic.

    Excellent video. I had an enlarged cervical lymph node (15mm) a couple of years ago. Saw my doctor, who felt it was nothing. It persisted about 10-11 months, then fully went away, never to be felt again. With an extensive family history of cancer, this had me anxious for so long. I believe I was bitten by a tick a day or two prior (back of my neck) and always wondered if that mat have been related.

      All good. It never came back. Zero issues, so likely was an infection that resolved on its own.

      No, it stayed steady from its original size for probably 8-9 months, then gradually went down. It’s totally gone now, can’t feel a hint of it.

    I watched the very informative video.
    I have a lump along my left jaw.
    It just appeared out of nowhere.
    I haven’t been sick ever.
    But interestingly I have 2 adult cats. I am taking ibuprofen it helps a lot. I actually thought I had mumps . I’m 72 . I take no medicine prescribed by a physician. Thank you for this video.✝️👍

    he had a swollen node big as a plum for years and then it disappeared. why did it do that? I suspected tb. now he's on levaquin for bilateral pneumonia...well never know I guess if he has tb.

    Sometimes I feel the left side of my neck below my jaw and think that a lymph node feels bigger than the other side of my neck. I also have had a frequent blood shot eye on left side only.
    The pupil is often slightly bigger than my right as well. I went to an eye specialist who told me it was just benign anisicoria.
    I had a very bad ear infection after scratching my ear too hard back in 2019. The whole left side of my face swelled. I had to go to ER where they gave me ear drops and antibiotics. It healed fine.
    The reason I scratched my ear to begin with is that I have had chronically itchy ears since 2018. I think possibly it could be related to water softener water as it started a month after moving into building with it.
    I also have hashimoto’s. My thyroid feels fine though. No pain or lumps.
    I’ve been to doc for itching ears they don’t have anything conclusive.

      My left side swelled up from teeth issues, antibiotics helped, but since then (three years ago), I have a soft enlarged lymph node.

    I am waiting for 1/1/24 to see a doc. I have a huge lymph node in my underarm and some smaller ones. My neck keeps popping up more and more day by day. Those are smaller but they are very palpable. I have been fatigued for a year plus, and my oxygen sats drop with small exertion.

    I have a large mass near my armpit and several swollen lymph nodes in my neck I had an ultrasound done they are not worried about my arm but they want me to see an ENT for my neck they also scheduled me with a hematologist/oncologist I’m not sick but what made me go get checked out is the fact I keep feeling bad like I am sick it’s weird I’ll feel like I’m getting the flu one day and feel ok the next day I have had the mass in my arm for a long time it just randomly started getting bigger then I have had the larger ones in my neck around 1 cm for about 6 months now I can’t tell if it has gotten bigger because I only had the ultrasound recently so I wouldn’t know if they have grown much however the one in my armpit area has grown a lot it’s about 3 1/2 inches rubbery no pain so I’m not to worried about it hopefully at my next couple dr visits they can figure out why I feel so bad!!!     Hey Doc what about salivary gland swelling? I have this periodically. A small stone appears under my tounge, falls out and the swelling disappears.

      You need to alkalize your body/blood. Raise your immune system. Quite sugar and baked/fried foods.

    Are there lymph nodes in the back of the neck? I noticed a bump about halfway down my neck, just to left of my spine. It feels firm and tender/slight pain when I press on it. I'm wondering if it's a swollen lymph node, but not sure if there are nodes in that location.

    I have a swelling on one side of my neck but no hard lumps. Just swelling. No night sweats or fever either. It have a pinch of pain sometimes but most of the time, it's painless. The swelling doesn't increase too. It subsides if you put ice pack on it. I recently stopped my medication for acid (like lansoprazole and antibiotics), and I have gallbladder stone (based on my latest ultrasound). Is it still fine?

      I just found out that I have a bit of ear infection so that's why my neck is swelling but after a few weeks, it's now gone. Maybe tootache, ear infection, etc. can cause these swellings.

      @kathleenesomera3369 yes they told me mine was pharyngitis but I already took the medication and my sore throat is still there and my lymph nodes are a bit swollen and I went back and they took x rays and they said everything looks good so idk 🤷‍♂️ maybe it's my wisdom tooth

      @@jgrove2169 It will take some time before the swelling go down. It took me around 3-4 weeks before the swelling disappear. Just observe your swelling if it grows or not. Hope everythings fine. 😉

    My axillary lymph node is 3cm since November 2023 , it got biopsied via nail aspiration with 8 samples taken, no malignancy found but it's been there continuously for 7 months. I have a fibroadenoma in the breast same side tested negative. I am a 6 year breasts ductal carcinoma survivor with left total mastectomy. Don't know what to do, my oncologist is dismissive about it

      Im so sorry. If your doctor is ever dismissive with you, it may be time to find a new one. You have to remain your #1 advocate. Sometimes i think doctors become numb to it all. That's not a doctor that you want or need. Prayers be with

    So ive had enlarged inguinal lymph nodes, now for the last 10months. The hospital found a iliac crest tumour on my hip at 55mm big. The lymph nodes grew, from my hip and measured about 8 inches as a solid matted mass. My skin is burning to touch, and is also really dark purple. The discoloration has also travelled across my pelvis and into my inner thigh. The area is very large and is extremely painful. Im due to go for a 3nd biopsy, as the first and 2nd one come back as benign. Could the tumour itself be cancerous? Even though drs say its a bony benign tumour? Could there be a chance of cancerous cells, not reaching my lymph nodes?
    Im stupidly worried, as its completely changed my life. Plus the pain is unreal. Any advice would be great.

      @shreyasj9611 my 3rd biopsy also come back as benign, and my lymph nodes where of a normal size. That doesn't explain my scans, or tests,which show enlarged internal lymph nodes and high white blood cells. Also the size of the growth In my groin, which has now started into my leg. I've lost the whole top layer of skin, where it's burning so hot. I've been in serious pain and the mass has recently grew more than twice the size. In as little weeks.
      So now the hospital are going to take me in for my 4th biopsy of the tumour/fatty swollen material, (which I've been told for near on 12 months, where my lymph nodes).
      Still no diagnosis. They come up October last year and I thought it was a hernia next to my hip. By December, I was being told it was cancer. 10months on. I still have no idea. But I'm now bed ridden for 95%of my time. I cant use my leg and apart from laying down I'm in severe pain. The thought of a 4th operation, when ive had one the beginning of September, and the beginning of August, just seemed crazy.
      I'm sorry I've got no more reassurance. 12 months for a diagnosis is terrible in my eyes. I'll let you know how my 4th biopsy goes.

      @shreyasj9611 I did get told it could be Lymphoproliferative syndrome. Yes, a big mouthful. It'll be easier to Google it instead of me explaining.
      It sounds like a cop out though.

      @@tommayne1927 i know this was posted a while ago but i hope you are ok, doctors can be so frustrating. i have gotten no answers to why i had drenching night sweats for over a year with tons of other symptoms...and i just found a hard pea sized lump on the side of my neck. We'll see, i guess, when i call the doc.
      BUT.. i really hope you're okay, feeling better and getting answers🤞

    I am on my 2nd doctor and ent... no relief since January. Stagnant lymph in the front, but the nodes don't enlarge. They just ache and affect swallowing. I feel like I can't swallow and I have thick glue like mucus. I haven't had ulcers in the mouth.

    I think I have a swollen node, but I'm not sure. I can feel it when I push in my neck, but there isn't a visible bump. The bump is also not visible when I turn my head to the sky. I can now feel it for around 2 months, there are 0 other symptoms.

    Hi, i have several, 3 lymph nodes on my neck, thabt one is around 1.5 cm and the other are less than 1 cm. They showed up one year ago when i had strep that lastet almost 1 month, since than the nodes haven't change, they are the same as they were form tbe beggining, they get more solid when i get a flu or tooth pain, i havent had any b sympthoms for now. Could they be bening, like permenent lymph nodes, or cancerous beacuse they're still here, and baven't changed?

      Me, too....I had mine watched and measured by an ENT for 2-3 years; they finally got so enlarged (after my 2 days of mild COVID-19), but multiple needle biopsies missed getting enough tissue samples, and an MRI showed them enlarged throughout my body, so I had one removed in a simple outpatient surgery for diagnosis...is a benign condition, Rosai-Dorfman, that is supposedly self-limiting in about a year; mine have reduced down so they are not visible to other folks, but are yet larger than they were pre-COVID. It's been a little more than a year now. Could be more quickly helped with Prednisone or other such drugs that I don't want to use, esp. since with those, there is the stated possibility that the condition returns after it gets better with the meds.     Dr. Would HS for 25 yrs mean something, I have 5 hard lumps in my left neck. I have had 8 skin grafts , all my bilateral artillery glands removed. My groin nodes removed. 28 staples and 4 more skin grafts. Had a Vac system at Moffitt Cancer ctr. Now I have 5 hard lymph tumors. Very scared!

    I have had one that has grown since 2014. Two ENT surgeons here in AZ has not known what to do , what it is. Actually, I got a surgery we're the doc supposedly was going to remove it and I believe he did not ..he removed another thing . Which expert should I go to? I am getting tired of these doctors nowadays..no one wants to research .just want to remove, give med and or replace that's it ...

      biopsy...inconclusive, blood tests....50 drugs kill HIV , get HIV test..dont panic you will be ok

    I have 3-4 swollen lymph nodes in submental/submendibular(below the chin) area for 1.5 months and I have chronic pharyngitis ..Two ENT doctor said not to worry about it. Now I don't know what to do

    Hi Doctor, this is me Aadarsh Chaudhary. I am from Nepal. And I have multiple (4-6) right cervical lymph nodes since I think for last 4 or 5 years. I did several investigations including FNAC and the doctor diagnosed me with reactive lymphadenopathy. My right forst premolar teeth is also destroyed by infection i guess. Now I am wondering if I can do surgery to remove all these swollen lymph nodes if it is possible because sometimes I get stressed too much thinking of it. Please Doctor do reply to my question. What should I do?

    I hv enlarged lymph node on left side of neck , i went for usg and fnac , it showed reactive lymphoid hyperplasia , it is there since 10 months, what should i do now

    My two neck lymph nodes underneath my jaw have been swollen for about 2 months the right one I can feel but the one on the left is slightly swollen they seen it on a CAT scan my doctor called me this morning saying that the biopsy that was done 4 days ago nov 24 2022
    they can't really make a determination if it's cancer or not so they WANT TO COMPLETELY REMOVE it,,to test it for cancer.
    I HAVE NO OTHER SYMPTOMS!!
    COULD IT BE FROM THE COVID SHOT!!
    I scheduled the surgery for January 17 2023,,
    I'm not sure if I should go through with it,,
    my question to you is is it safe to get my lymph node on the right side underneath my jaw in my neck removed,,
    Or should I naturally try to cure the cancer in my body which I'm not sure that it is but I have a good feeling that I have cancer in my body

      I would not do anything at this point not intel I'm sure what I'm dealing with. I believe you need another opinion 🙂

      @@meshell1399 i have constant abdomina discomfort at right upper quadrant of my stomach i lose weight and have fever recurrent with a slightly swollen lymp node in the right side of my neck below my jaw

    A cervical Lymphadenopathy is serious ? I have a lump in my neck. diagnose cervical lymphadenopthy. my ent doc. said nothing to worry

    One major thing you’re missing. Ask the patient if they had a CT scan in that area. It’s not a small amount of radiation. It’s enough to cause cumulative DNA misrepair which causes cancer eventually

    What about hard, movable, painless, & no other symptoms? Like no night sweats, weight loss etc.
    Just a hard movable lymph node on the right/front neck where people usually feel their pulse.
    And it hasn’t grown or anything.

      @@alexthemoosefinder yea I had a sore throat and still have swollen lymph nodes arround my neck. Was freaking out because I thought I had cancer but turned out to be acid reflux and like a small infection so no worries!

    BRO this is the second time mine has swollen I have a small cold rn with a runny nose but this is not the first time and my last one went away by time
